Barrett’s Esophagus
• Demarcated by histological
changes in cells lining the
esophagus.
• Occurs in 10% of GERD
patients.
• Incidence is 1% in the general
population.
• Associated with
adenocarcinoma of the
esophagus.
Lower esophagus lined by red-coloured tissue, not the usual white-pink
colour.

Alginates
• Sodium alginate is the main active ingredient in Gaviscon
• Sodium alginate is a natural product refined from brown seaweed
– component of cell walls and the spaces between cells
– provides flexibility and strength
• both important attributes for plants growing in the sea
• Sodium alginate in Gaviscon is from Laminaria hyperborea (type of seaweed)
– found off the coast of Norway
Laminaria hyperborea
Alginates
• Made up of ‘building blocks’ of sugars
• Three block types are possible in seaweed
– MMM, GGG, MGM
• Number and order of these blocks is important
– determine the biochemical and physical properties
• Composition varies
– between species
– between different parts of the same plant
M= mannuronate
G = guluronate

Alginates
• Alginates containing the highest proportion of GG blocks produce the strongest gels
– GG blocks can bind to calcium to produce a strong cross-linked structure
• The stem of L. hyperborea used in Gaviscon contains high levels of GG blocks
Alginates
• Alginates have a number of properties – thickening
– stabilising
– gelling
– film-forming
• Alginates are polysaccharides
– composed of 1003000 building blocks of
sugar :Mannuronate(M) & Guluronate(G)
Alginates
• First scientific studies performed in the late 1800s – extraction of alginate from brown seaweed
• Alginate and other ingredients formed a foam – floated on top of the stomach contents
– relieved symptoms in patients with hiatus hernia
• Gaviscon first launched in Sweden in 1965
– by Ferring Pharmaceuticals
• Gaviscon introduced in the UK by RB in 1970

Mode of Action Acid neutralization The sodium alginate reacts with the stomach acid to precipitate a low-density viscous raft, which floats on top of the stomach contents preventing the acid, food, pepsin and bile from flowing back into the oesophagus

Indication
Gastric reflux, heart burn, flatulence associated with gastric reflux, all cases of
epigastric and retrosternal distress where underlying cause is gastric reflux
Alginate-based reflux suppressant
e.g. sodium alginate, alginic acid
• Physical, non-systemic mode of action
– forms strong, viscous barrier (raft)
– helps prevent reflux of acid, pepsin and bile
• Does not affect normal physiology of the stomach
– no significant side effects
– pH below the raft remains the same
– is not absorbed by the body and can be given to vulnerable patients
• Rapid onset & Long-lasting effect
• Suitable for self-medication
