This manual provides step by step instructions for a teacher to conduct a self-assessment, complete a preliminary professional development plan (if applicable), or professional development plan, and view observations, summary rating form, and record of teacher evaluation activities. All of the following steps that have the NC Educator Evaluation System icon next to them are completed using the online system.
Initial Login to the System
Log into: https://mxweb3.media-x.com/home/ncval/ Select the correct school district Type in the username that was provided to you (UID number) Type in the password that was provided to you: 123456 Click Login

Profile and Settings
You will only need to complete the profile and settings changes one time. These steps are completed online.
Click on the Profile icon.
Click on the Change Password icon.
Type in the original default password (123456) Type in a unique password for yourself that is at least six characters long. Click Save.

Click on mxWeb
Click on the NC Educator Evaluation System icon
Read and accept the End User License Agreement. Look for the check mark at the bottom: This occurs on your first login only. Be sure to scroll to the bottom to find the point of acceptance.
Remember to always Logout of the system to ensure the program is closed.
Do not close the program by clicking on the red “X”

New Self Assessment
Log into the system. Click on the NC Educator Evaluation System icon.
Click on Observe/Report. Click on Observations.
Check to see if a Self-Assessment already exists. If so, teachers may make changes to the current Self-Assessment. If a Self-Assessment does not exist teachers may create a new Self Assessment by clicking on New beside Self Assessment section or New Self-Assessment icon as shown below.

The fields are pre-populated but you may change the pre-populated name to any naming convention. Click Next.
1. Use the dropdown menus to choose the Start Time (the time you begin your self-evaluation) 2. Use the dropdown menus to choose the Teacher Status (Probationary or Career
Status Teacher), and the Year Number (if this information is prepopulated, ensure the data is accurate).
NOTE: The End Time must be entered at the end of your evaluation.
Begin reading and filling out your self-assessment by clicking in the appropriate boxes beside the descriptors that apply. Once you click in a box, a checkmark will appear (see sample below).

You may type in Comments, attach a File (.txt, PDF, .doc, .docx, ppt, pptx), include a web link, check the boxes of Example of Artifacts, and add additional examples of artifacts in the space provided.
SUGGESTION: During the school year if you revisit the self-assessment, type a date next to any comments that you have included.
Once you have completed checking all of the descriptors for each standard of your self-assessment, use the drop downs menus following the word To in order to choose your End Time.
After you have completed your self assessment, check the box next to I have completed my self assessment. You may give your principal permission to view your self assessment by checking the box next to The principal can view my self assessment.

Click on the Save icon.
You may click on the Print icon to print your self-assessment. (The print icon opens the self-assessment in a new window)
Click on the Done icon. You will be prompted if you want to save before leaving.
NOTE: Even before you completely finish the self-assessment, it is advisable to click on the Save icon to save the information that you have entered so far. Incidentally, the system will not indicate any skipped descriptors. It is your responsibility to ensure that the self-assessment is complete.

New Preliminary Professional Development Plan If you do not have a Professional Development Plan, then you must complete a New Preliminary Professional Development Plan. The New Preliminary Professional Development Plan is created by the teacher. The teacher and the principal have the ability to add content to the form. Once the administrator signs the form, it is locked. The teacher then may login and sign the Preliminary Development Plan. Please DO NOT create a new plan if you have a rollover Professional Development Plan from the previous year.
Log into the system. Click on the NC Educator Evaluation System icon.
Click on Observe/Report. Click on New Preliminary Prof. Development Plan
Teachers may also click on Observation and click on the Teacher Task tab. Teachers may create a new Self Assessment by clicking on New beside Self Assessment section or New Self Assessment.

1. Use the drop down menus to select your Teacher Status. 2. Fill in your Subject Area. 3. Select your Plan at the top of the page before filling out the rest of the form.
No one can type in Section A: Professional Teaching Standards or Professional Development Priorities. This section will populate if an observation from the previous year is marked Developing or Not Demonstrated.

You may add goals in the To Be Addressed by this Plan in the Standard(s): and Element(s) sections.
Complete Section B. Teacher’s Strategies. Type the Goals for Elements, Activities/Actions, Expected Outcomes and Evidence of Completion, Resources Needed, and the Timeline.
Click Save. Click Done.
You may click on the Print icon to print your Professional Development Plan. (The print icon opens the Professional Development Plan in a new window)

Electronic Signature Once an administrator has electronically signed a section of the plan (Preliminary Professional Development Plan, Professional Development Plan, Mid Year Review and/or End-of-Year Review) the teacher may login and electronically sign that section of the plan. Make sure the form is completed before electronically signing. The folllowing steps will be used for electronic signatures.

Click on Observe/Report. Click on Observations.

Click on the Teacher Task tab. Click on your Professional Development Plan based on 2011-2012.
Select Preliminary Development Plan, initial Professional Development Plan, Mid Year Review or End-of-Year Review tab. Scroll down to the bottom of the form.
Make sure the administrator has electronically signed the form. Click inside the box underneath Teacher’s Signature.
Enter the password in the dialog box that pops up to authenticate your signature. Click on Authenticate after having entered your password.

Complete a Professional Development Plan The Professional Development Plan will be automatically created based on the Summary Rating Form from last school year. The teacher and the administrator have the ability to add content to the form. Once the administrator signs the form, it is locked and no additional changes may be made until the next review. The teacher then may login and sign the Professional Development Plan.
Log into the system. Click on the NC Educator Evaluation System icon.
Click on Observe/Report. Click on Observations.

Click on the Teacher Task tab to view your Self Assessments, Professional Development Plan or your Preliminary Development Plan.
Click on your Professional Development Plan created from the previous year Summary Rating Form. 1. Use the drop down menus to select your Teacher Status. 2. Fill in your Subject Area. 3. Select your Plan (Individual, Monitored, or Directed), at the top of the page before filling out the rest of the form. Section A will populate with Standards and Elements rated Developing or Not Demonstrated from the previous school year’s Summary Rating Form.
The teacher and principal may add to Section A To be Addressed by this Plan in the Standard(s) and Element(s) section, if needed.

Mid Year Review The teacher and the administrator have the ability to add content to the form. Once the administrator signs the form, it is locked. The teacher then may login and sign the Mid Year Review.
Log into the system. Click on the NC Educator Evaluator icon.
Click on Observe/Report. Click on Observations.

Click on the Teacher Task tab. Click on your Professional Development Plan.
1. Click on Mid-Year Review tab. 2. Use the drop down menus to select your Plan. 3. Set the date for the Mid-Year Review.
Complete Section C: Evidence of Progress Toward Specific Standards of Elements to be Addresses/Enhanced. Complete Section D: Narrative – Teacher Comments. An assigned Mentor would complete Section D: Narrative

Click Save. Click Done.
You may print the Professional Development Plan by clicking on Print. The form will open in a new window.

End-of-Year Review The teacher and the administrator have the ability to add content to the form. Once the administrator signs the form, it is locked. The teacher then may login and sign the Mid Year Review.
Log into the system. Click on the NC Educator Evaluator icon.
Click on Observe/Report. Click on Observations.
Click on the Teacher Task tab. Click on your Professional Development Plan.

1. Click on End-of-Year Review tab. 2. Use the drop down menus to select your Plan. 3. Set the date for the End-of-Year Review.
4. Complete Section E: Evidence of Progress Toward Specific Standards of Elements to be Addresses/Enhanced. 5. Complete Section F: Goals. Click the drop down menu beside the goal and choose “yes” or “no”. 6. Complete Section G: Narrative – Teacher Comments. An assigned Mentor would complete Section G: Narrative
Click Save. Click Done.
You may print the Professional Development Plan by clicking on Print. The form will open in a new window.

View Observations
Log into the system. Click on the NC Educator Evaluator icon.
Click on Observe/Report. Click on Observations.
Click on the Observer Task tab. Make sure you are viewing the correct school year.
Observation: Click on the observation to view the observation.

You will be in the Viewing status meaning that no changes can be made to the observation. The boxes are grayed out.
If you have a Peer Observation by another teacher and the Peer Observer has electronically signed that evaluation you will be able to view the Peer Observation. You will not be able to make any changes to the evaluation rubric.

Sign the Observation or Peer Observation by clicking in the Teacher’s Signature box.
Enter the password in the dialog box that pops up to authenticate your signature. Click on Authenticate after having entered your password.
After signing the observation, the Add Comments button becomes live.
1. The teacher may type comment in the white box to the left of the button. 2. Type the comment and click Add Comments.

After clicking on the Add Comment box a warning message will pop-up.
Click OK.
NOTE: A message box will appear indicating an email has been sent to the observer to notify them of your additional comments.

View Summary Rating Form
Log into the system. Click on the NC Educator Evaluator icon.
Click on Observe/Report. Click on Observations.
Click on the Observer Task tab. Make sure you are viewing the correct school year.

In the Summary Rating Form section click on the Summary Rating Sheet.
The ratings can be viewed on the right-hand side as checked radio buttons.
One of the following ratings is assigned to each element and to the standards as a whole (Overall Rating): Developing, Proficient, Accomplished, Distinguished, or Not Demonstrated. Click on any of the + signs beside an element to view a summary of that element’s descriptors.

Administrator must electronically sign the form first as shown below. Once the administrator signs, the form is lock for the teacher and administrator. Sign the Summary Rating Sheet by clicking in the Teacher’s Signature box.
After clicking on the check box a warning message will pop-up.
Enter the password in the dialog box that pops up to authenticate your signature. Click on Authenticate after having entered your password.

After signing the Summary Rating Sheet, the Add Comments button becomes live. The teacher may type comments in the white box to the left of the button. Type the comments in the white box and click Add Comments.
After clicking on the Add Comments box a warning message will pop-up. NOTE: A message box will appear indicating an email has been sent to the observer to notify them of your additional comments.
Click OK and a pop-up message will appear.

The comment has been added with the date. Additional comments may be added by the teacher.
Once the Summary Rating Sheet is complete and electronically signed the teacher can still view the form. To view the completed Summary Rating Sheet click on the form. The ratings are grayed out but can be viewed.
To view next school year’s Professional Development Plan click on View Professional Development Plan for 2013-2014. The Summary Rating Form may be printed by clicking on the Print icon. This will open the form in a new window. The teacher may select to print the Teacher Summary Rating Form or the Teacher Summary Rating Sheet by using the drop down menu beside Print Format.

Record of Teacher Evaluation Activities
Log into the system. Click on the NC Educator Evaluator icon.
Click on Observe/Report. Click on Observations.
Click on the Observer Task tab. Make sure you are viewing the correct school year.

In the Record of Teacher Evaluation Activities box click on the Record of Teacher Evaluation Activities for 2012-2013.
The form will indicate when each activity was authenticated by the teacher and the evaluator.

This form will allow you to confirm attendance at orientation and pre-observation conferences.
Click in the appropriate checkbox(s) to sign. You will be prompted by a warning message pop-up to authenticate your signature by entering your password.
Enter the password in the dialog box that pops up to authenticate your signature. Click on Authenticate after having entered your password.
The Record of Teacher Evaluation Activities Form may be printed by clicking on the Print icon. This will open the form in a new window. The teacher may select to print the Record of Teacher Evaluation Activities Form as Standard Printing or as a PDF file.

Peer Observation
Log into the system. Click on the NC Educator Evaluator icon.
Click on More Click on Rubric for Observing Teachers
Select Teacher Box: In the Select Teacher box scroll down and click on the teacher’s name who you will be observing.
Note: You will ONLY be able to see the observation that you have conducted. You cannot see other teachers’ self assessments or the principal’s observations.

Click on the New button in the “Observation list done by yourself” box.
After clicking on New a warning message will pop-up. Click OK to create a Peer Observation form.
A new Peer Observation form will be created for the teacher.

1. Select the Teacher’s Status and teaching year. 2. Select the Start Time.
3. Click on the Calendar icon and choose the date you are completing the peer observation Click Set.

Click on the top drop down menu arrow to select the Teacher Status. Click on the bottom drop down menu arrow to select the teaching year.
Use the drop down arrows to choose the Start Time (the time you begin the peer evaluation).
Begin reading and filling out the rubric by clicking in the appropriate boxes beside the descriptors. Once you click in a box, a checkmark will appear.

Suggestion: If you conduct more than one peer observation on the teacher, type a date next to any comments that you have included.
Once you have completed all the standards in the rubric, use the drop downs to choose the End Time.
Click Save.
You may click on the Print icon to print a copy of the Peer Observation. The form will open in a new window.

Click Done.
Note: If you do not completely finish the Peer Observation, click on the Save icon and it will save the information that you have completed. The system will not indicate if you skipped any standards.
Once you have completed the Peer Observation and authenticated the signature the status of the observation will change to Completed.
You have completed the Peer Observation and the teacher that you observed may now view the observation.
