Mycofenolzuur: IMDPH II inhibitie• MPA is a selective, non-competitive inhibitor of IMDPH (inosine
monophosphate dehydrogenase) and guanylate synthethase.
• IMDPH is an enzyme in the ‘De Novo’ pathway of purine biosynthesis, where it catalyses the conversion of IMP to xanthosine monophosphate via the cofactor NAD.
• T and B lymphocytes are selectively dependent on this ‘De Novo’ pathway for purine biosynthesis.
• MPA has a higher binding affinity for the IMDPH type II isoforms, which is up-regulated in activated lymphocytes.

Conclusies• Specifieke (alsook algemene) bijwerkingen van immuunsuppressieve
therapie kunnen, althans ten dele, gunstig beïnvloed worden door dosisreductie, rekening houdend met de vereiste graad van immuunsuppressie
• Immuunsuppressieve medicaties behouden echter ten dele hun karakteristiek toxiciteitsprofiel spijts dosisreductie
• “Op maat gemaakte” immuunsuppressie, waarbij rekening wordt gehouden met het (veranderlijke) patiëntenprofiel en de intrinsieke eigenschappen van de immuunsuppressieve medicatie, kan de “cost-benefit” aanzienlijk verhogen
