To foster economic vitality,

Snohomish County Health Leadership Coalition Strategic Goals
Improve community health, healthcare costs, quality of
place, and economic vitality and prosperity
Serve as a leadership forum and clearinghouse to prioritize and pursue healthcare priorities that support long-term health and economic prosperity
Foster and forge public-private partnerships that
combine a long-term view with short-term
results
Establish a scalable model that can be
implemented in other parts of the state and
region
First Initiatives
Advance Care Planning
Youth Physical Activity
• The Coalition developed two initial proof-of-principle initiatives in 2013
• We focused on both ends of the healthcare spectrum: youth activity and advance care
• Initiatives created the runway for a broader community effort to create long-term gains
Goal
• Provide accelerometers and technology to 5th graders and teachers to
see engagement and measure progress
• Promote better health, academic performance and lifelong healthy habits
• Reverse the downward trend of youth exercise/activity
Early Results:
• 12.5% increase in participant activity levels
• Countywide best practices identified
• Goal-setting with 70+ schools for 2015-16
• Promoted expansion into five states
Goal:
• Ensure people receive care in-line with their values when facing life
threatening illness
• Develop a self-sustaining community-based model to expand the
program countywide
Early Results:
• Tripled training in SnoCo
• 35% trained by community volunteers
• On path to train 4,500/year by 2017
