Summary Incident happened on the north west

corner of the 3rd bench of the overburden benches.

Summary – cont. Shovel called for a clean-up. Rubber tire dozer came around trucks to give

clean-up. First truck thought he needed to back up to give

dozer some room and made radio call to inform other trucks of his intentions.

Second truck acknowledged and started to back up to give first truck room.

Second truck did not see third truck in mirrors and back into third truck, trapping operator who was out and behind cab.

Third truck had pulled up about 30-40 feet behind second truck

Third truck was smaller than second truck (190 ton – 240 ton)

Summary – cont. Bed of larger truck

struck the top of the cab of smaller truck, pushing top of cab back

Cab pushed back brake cabinet, trapping the operator that was behind cab.

Operator was trapped, facing electrical cabinets, between brake cabinet and air tank.

Extrication took approximately one hour.

Victim was transported to the local hospital and Flight for Life was called in.

Victim passed away before he could be loaded onto the Flight for Life
