DESCRIPTION
Devereux Colorado’s Community Based Services Program was created to fill a gap in the transitional process. With the program divided into four tracks: In-home Therapy Services, Individual Transition Services, Employment Services and Outpatient Psychiatry, Community Based Services gives clients a full spectrum of support as they enter the workforce. Working with the local community, Devereux Colorado has secured an assortment of jobs with local business owners to help get our clients on their desired career path. However, this program doesn’t simply provide clients with a job; the four tracks, outlined below, allow this program to accommodate a variety of different clients and potential issues that may arise while employed.

Career Based Services
Devereux Colorado’s Community Based Services Program was created to fill a gap in the transitional process. Devereux Colorado will provide individual and family treatment to encourage a cohesive and supportive family structure. Our clinicians are trained to use the following models: Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), Applied Behavioral Analysis (ABA) along with general concepts from major research based in-home therapy models.
Function of Services
Services begin with the therapist meeting and assessing the client and family. Monthly team meetings. Weekly contact with the client manager/case worker. Services typically last a minimum of 90 days, but can be tailored as
determined appropriate. The length of treatment will be established through discussions with the treatment provider and the client manager/case worker.
Individual Transition Services
Resource Identification and Connection These resources include mental health, free/discounted healthcare,
drug/ETOH groups, after school activities, sports teams and activities, social groups and activities and crisis support.
Drug Screening
Transition specialists are trained to administer a urine analysis and submit to the laboratory. Devereux Colorado will interpret and report the results.
Independent Living Skills Development/ Vocational Support The transition specialist will complete an assessment and discuss goals
with the client. Then with the treatment team, they will determine the vocational support/independent living skills needed for the youth to be successful. Skills are categorized in the following areas: housing, employment, education, life skills and health and safety.
Education Support The transition specialist will provide support, tracking and mentoring
to ensure the client has success in an academic setting. This will include post-secondary educational support, adult life functioning and health and safety.
Family Intervention If family therapy is needed, the transition specialist will work with the
case/client manager to determine if and how these services will be administered.
Mentoring Developing healthy relationships with adult role models is crucial for
youth to be successful in the community. Transition specialists will help them seek out and maintain these relationships.
Employment Training
Devereux Colorado’s Employment Training Program is designed to provide today’s youth with the guidance required to get and maintain employment. With varying degrees of guidance, the Employment Training Program includes three tracks to allow for the flexibility necessary to encourage growth in all types of candidates. From ten work sites on the Devereux Colorado campus, to working with businesses in the community, the client will be supported by a trained job coach to achieve maximum growth. The training process begins with teaching clients how to fill out job applications and prepare for an interview all while addressing the basic skills necessary to perform the job. The transition specialist will help the client identify possible employment opportunities within the community and will provide assistance for up to 30 days following job placement to help them transition and excel in the position.
Function of Services
- Upon admission to the program, a vocational/employment assessment will be conducted.
- The client will go through the established procedures of Devereux’s Work Experience Program to achieve employment- including: application preparation, interview preparation, carefully selected job placement and coaching/supervising while client is employed.
- Additional time can be added to include an independent living skills focus, which would include assessment and classroom time using the Tri-County Youth Council, “Get a Life” curriculum.
Employment Tracks
Work Experience Program The Devereux Colorado, Cleo Wallace Center residential campus has a
robust employment program that includes ten different work sites. The client on this track will receive one-on-one support from a job coach during every shift to help them develop the necessary skills to maintain employment in an internship or job within the community.
Work Internship Program Devereux Colorado has partnered with different organizations within
the Denver metro area to provide clients with a variety of different employment options. Devereux assumes responsibility for all client wages and associated employment expenses. The participating business agrees to support and teach the client all the necessary skills to be successful in the position.
Job in the Community The client will work with a job coach to find employment in the
community, independent of Devereux Colorado. The business will be responsible for paying client wages. The client will also have the opportunity to meet with a job coach on a weekly basis.
Outpatient Psychiatry
Devereux Colorado houses a highly skilled team of psychiatrists with extensive experience working with children and adolescents in residential, inpatient and outpatient settings. These services can be used to help prevent placement disruption, prevent out of home placement, transitioning youth into the community or for long-term need requiring psychiatric care and medication monitoring.
Function of Services
A psychiatric evaluation will occur during the first appointment. Medication management will occur at a frequency requested or
indicated.
