• Healthcare staff members accessing celebrity health records and other personal information without permission.

• Breach of HIPAA – Patient Information Confidentiality Standards

• Violation of patient information and confidentiality standards, whether intentional or unintentional, will be subject to disciplinary actions.

What is in place to avoid this situation

• All new employees shall receive information and training concerning the standards for Confidentiality of Patient Information and HIPAA at the time of employment and shall sign a statement agreeing to uphold those standards. Routine training will be held on an annual basis.

• There will only be a select group of staff members with security clearance to access celebrity information.

• Random audits will be performed on all computer systems to ensure that only employees with a need to know have accessed patient protected health information.

What is in place to avoid this situation

• Employees should not access or request from another person ANY information on ANY patient unless it is within their job function to do so or they have prior written authorization from the patient

• Employees should not give out or share login information with anyone. Login information should be kept in a secure location. Login information/passwords will expire every 90 days.

• Computer sessions should be secured while after from workstation and completely logged off upon departure from work.

How will this training on confidentiality be effective?

• Employees will be made aware of the importance of confidentiality, HIPAA, and what exactly is PHI.

• Employees will receive continuous training and knowledge to be kept abreast of confidentiality and privacy

• Employees will be aware of consequences resulting of violations
