Th e African American church has and remains an important source of information and dialogue to the community. Churches provide its congregation with a wealth of knowledge and support; emotionally, fi nancially, politically and of course spiritually. Th is historically developed platform of trust and infl uence provide a grassroots connection to discuss the importance of healthy lifestyle. Th e issue of chronic disease is among the most challenging health issues of our time and the church serves as an excellent place to deliver eff ective health messages regarding good nutrition and exercise that can greatly impact disease prevention.

Th e National Kidney Foundation of Michigan’s (NKFM)mission is to prevent kidney disease and improve the quality of life for those living with it.

•Th e NKFM provides community-based prevention programs that focus on diabetes, hypertension and chronic kidney disease (CKD).

•Th e NKFM also provides patient and professional education and services regarding CKD.

•Th e NKFM uses its proven expertise in training lay health advisors and community partner collaborations to implement programs targeting individuals who are most at risk of kidney disease.

Body and Soul: Living on Th rough Love is a program designed to raise awareness of the Michigan Organ Donor Registry by targeting African Americans in church settings.

Our goal: •To increase the number of actual registrations on the Michigan Organ Donor Registry.

•To increase knowledge in the church community of healthy lifestyle options.

Th e Body & Soul: Living on Th rough Love Program is currently in the initial research phase. As a resultdata will not be available until the completion of this program in the next year. Although we are able to report that 23 Metro-Detroit churches are enrolled and 115 church members have been trained in administering Body and Soul to there congregation, data will be shared at a later date as it is submitted and analyzed.

EnhanceFitness® as administered by NKFM currently serves the Southeast Michigan area. Our three church locations have registered over 100 participants, while averaging over 300 (non-registered participants) drop-in visits to daily classes over the last three months.

EnhanceFitness® standard class format:•Warm-up (5-8 minutes)•Cardiovascular workout (20 minutes) •Instructor determines intensity of workout based on participant ability•Cool-down (3-5 minutes)•Resistance strength training (20 minutes) •Utilizing 5 – 10lb adjustable weights•Flexibility (8-10 minutes)•Balance training included in warm-up and cool-down•Music typically provided by instructor•EF participants’ level of functional fi tness measurements •Baseline and follow-up performance measures•Participant and program level outcome data collected, analyzed, distributed

Enhance Fitness®(EF) is an evidence-based physical activity program designed for older adults (All age groups welcome)•Accommodates all levels of fi tness•Certifi ed instructors implement the program, using standardized protocols•One-hour physical activity program three times a week•Focus is on cardiovascular conditioning, strength training, fl exibility and balance training •Class options accommodate all levels of fi tness Level I – standing version Level II – seated/modifi ed exercises for more frail adults at risk for falls•Can be tailored for specifi c chronic diseases, e.g. arthritis and diabetes•Free classes open to the public began in Sept. 2008

Churches participating with Th e Body and Soul: Living on Th rough Love program will train 5 members to become lay health advisors who conduct presentations, answer questions and conduct social marketing campaigns on health topics. A baseline survey is administered to parishioners and at the end of a 1 year cycle participants complete a follow-up survey to measure any change in opinion on health topics.

Th e ability to establish, develop and sustain personal relationships has been instrumental in the growth and development of both Body and Soul and the EnhanceFitness programs. Some key community partners have included:

•Omnicare Health Plan, Health Alliance Plan •Henry Ford Health Systems, Detroit Medical Center•Th e University of Michigan•Gift of Life Michigan•American Cancer Society

Body & Soul: Living on Th rough Love and EnhanceFitness® are prevention/education programs that look to improve the outcomes of people with diabetes, hypertension and other chronic health challenges; while bringing awareness to organ donation.

Th ese programs are examples of NKFM’s commitment to developing and implementing culturally appropriate programs that strive to increase the number of people that learn to successfully manage their health condition on a daily basis and respond to the issue of organ donation in the African American community.

•Another aspect of training involves the social marketing campaign. Th is mode of marketing highlights the need for African American donors through the use of personal success stories, introducing participants to the Michigan Organ Donor Registry.

•After training each church participates in a year long campaign, using social marketing techniques to educate their congregation on the need for parishioners to lead healthy lifestyles and the need for everyone to register with the Michigan Organ Donor registry.
