WHAT?

A one-year project 2016 – 2017:1. to develop a first prototype (Proof of Concept, PoC)

of the mICF for person-centred assessment of functioning to record a person's functional status

2. to test and validate the user experience & acceptance using user-centered and agile methods

3. to evaluate the information quality and how this solution assists the person and care providers in shared decision-making of the patient goal setting and rehabilitation planning.

WHY?Expected benefits of ICF-based mobile solution• Service user: easier self-assessment, clearer

communication with health professionals, possibility for networking

• Professionals: easy and quick summary of the service user’s life situation, possibility to transfer information to service plan and other systems

• Organisations: less work time in documentationThe solution is based on international ICF standard, that allows structured information collection
