Objectives : The aim of the initiative is to act as community health workers collaborating with the social resources of the community. The session topics were chosen by people of the community and their participation was conducted by three health care workers of our center. R esults: - Participation of 178 people, members of the Home of Retired. - 6 sessions from February to June and October to November 2013. - Health topics chosen: First Aid, anxiety and depression, memory loss, joint pain and colds and flu. Conclusions: - Coordination between community services. - Active participation by users. - Proximity of professionals that insert into the community. - Use of resources (professionals, human and material infrastructure).
