DESCRIPTION
A breast implant rupture is a recognized complication of breast implants and can be either intra or extra capsular. An intra-capsular rupture occurs when the shell of the implant ruptures but the fibrous capsule formed by the breast remains intact. Silicone gel does not freely extravasate and this makes it difficult to detect on clinical exam or mammography. Intracapsular rupture is best seen on an MRI. An extra-capsular rupture can lead to a change in the implant contour and may be detected on clinical examination , mammography or ultrasound. An extracapsular rupture implies intracapsular rupture as well. MAMMOGRAPHY OF THE BREAST The detection of implant ruptures (particularly silicone implant ruptures) are very difficult on mammography and detection of intra-capsular silicone ruptures are almost impossible on mammography. ULTRASOUND OF BREAST Ultrasound may demonstrate a snowstorm appearance of an extra-capsular rupture, or the stepladder sign of an intra-capsular rupture. The overall sensitivity rates on ultrasound are thought to range between 59-85%. The cost for this is $198 and the Medicare rebate is approximately half of this amount. MRI OF BREAST MRI is the most accurate imaging examination for the evaluation of silicone gel breast implant rupture. Often MRI does not require contrast if the indication is solely for this purpose. MRI has a reported result of 76%-95% in the detection of rupture. There is no Medicare rebate for MRI of the breast available at the present time and the cost for this is $490.
