Primary Care Physician in Small Group Practice
Primary Care Physician in Small Group Practice
• Could be MD, PA, NP.
• Sees 25 patients a day, < 15 min each.
• Mostly sees the same stuff, but at least 1X a day encounters something hasn't seen before, or hasn't seen in awhile, or that is on tip of tongue but just can't remember – e.g., what's the lab test for celiac disease?
• Cares a lot about quality of service for patients, and patients are asking a lot more questions and generally getting more demanding. Interested in anything that can improve quality of service without adding significantly to costs.
• Overworked, lots of pressure, always worrying about the consequences of making a mistake – will pay a reasonable price for extra piece of mind.
• Increasingly expected to use EMR – would like to get information specific to the patient from inside the EMR.
Resident or Medical Student
Resident or Medical Student
• Could be 3rd or 4th year med student just starting to encounter patients, or resident with more experience and more responsibility.
• Sees patients on rounds at a hospital or in a clinic setting. Will probably spend some time in an office-based practice.
• Sees a large variety of cases and is often on the hook for figuring out a care-plan on the spot without any prior information about a patient. Will need to answer for any medical decisions to chief resident or attending. Feels a lot of pressure to do adequate research and take appropriate measures while on call, and then be able to answer for them on rounds.
• Has computers at every corner, probably carries a smartphone, very tech-savvy, uses EMRs almost exclusively over paper charts.
• Has learning responsibilities in addition to patient care that include: attending lecture, discussing other students' cases with faculty, etc.
The 5-Minute Clinical Consult
“We want to help our patients,But we need to make our businesses work,And we have no time.”

A PATIENT VISIT
• Document visit, particularly if solo practice, so may want to look up the billing codes for the injection and the diagnosis.
• Next patient indicated reason for visit is shoulder pain.
• Physician hasn’t seen recently; would like to quickly review differential diagnosis and signs & symptoms.
5 MINUTES BEFORE CONSULTATION

CONSULTATION5 MINUTES AFTER CONSULTATION
• Physician wants quick refresher on medications and therapy; check for new recommendations; check drug interactions.
• Wants to give steroid injection for pain, but hasn’t in awhile and wants to make sure it’s indicated for patient, and quickly review step-by-step.
• Wants to send patient home with instructions on basic strengthening exercises + recommendations to avoid aggravating injury.

EARLY RESULTS
"Right now being a single family doctor in a small town, in a smallpractice, I get up, leave the exam room, go to my office and look throughthe textbooks and then come back with the information. Here I could just go hold on a second. Let me look at either my desktop or my little handheld and call up the information and tell them without actually leaving the room. It saves time. ”
"I love the procedure videos! Whenever I need to do a procedure I haven'tdone in a while, I come back to my office and refresh my memory. Thanks!”
"Very useful for quick answers and patient handouts.”
”Outstanding site! like the set up of the algorithms. Everything I need inone place.”
“More succinct than its nearest competitor, and that is precisely what is needed when researching a patient at 3am!”
It has to get easier!
