Transitioning to adulthood may be somewhat more difficult for young adults with special health care needs. Our free Transition Clinic can assist with this process.
• Eligibility to participate is 14 years old.
• Visits with Transition Clinic’s Social Worker during your regular medical appointment.
• Educational information
• Tools to assist with working towards becoming more independent as you enter into adulthood.
• Helpful tips for your transition.
• Personal goals will move you closer towards independence.
• Ohio and National resources are provided to arm you with the knowledge and supports you may need throughout this process.

Project CARE 4 Epilepsy (Coordination Access Resources Education 4 Epilepsy) provides Care Coordination, Transition Clinic, Webinars and Facebook for Youth with Epilepsy. PROJECT CARE is designed to improve care coordination, increase social support, provide education and increase engagement for teens with epilepsy and their families. It is funded by the Health Resources Services Administration (Grant #H98MC26260)..
