(OOH) PALLIATIVE CARE HANDOVER

value a standardised
way of transferring
information to the OOH
service

Pre-pilot phase:
• 2013. 17 Forms completed
• Findings:
• Time consuming for GPS
• Time consuming for SouthDoc triage staff
• Increased likelihood of transcription errors
• Outcomes:
• Form streamlined 3 pages – 1 page
• Process streamlined – transcription reduced
Pilot phase:
• April-September 2014
• Packs sent to GPs within SouthDoc area
• Information leaflet, Guidance document and Forms
Evaluation:
• Qualitative and Quantitative analysis
• 60 Forms
• 22 interviews
• GPs, SouthDoc triage nurses, SPC nurses,
Community residential managers
Findings:
• The form is beneficial and easy to use
• Field completion rate very high
• 36 phone calls from 26 people
• Main symptom was pain
• GPs require constant reminding regarding use of the form
• An electronic version of the form would be useful
• Potential benefit in residential and community palliative care settings
Next Steps:
• Report of project being compiled. Aim to be finished by
end of February 2015
• Links made with GPIT group & IAGPC (Irish Assoc. of
General Practice Co-ops) - development of electronic
copy of form
• Aim to trial form in other GP OOH services across the
country.
• Disseminate findings via various fora
