• CHIP is an intensive lifestyle program with proven results based on years of scientific research.

• Participants commit to make lifestyle changes for a number of weeks (determined by each program).

• Change happens in the way you eat, move, and think- it will challenge you for the better!

• After the program, participants decide if they want to continue or revert to their old lifestyle.

management and forgiveness, as well as eating more plants and whole foods, not eating less.

• CHIP combines evidenced based lifestyle medicine research with behaviour change principles to evoke positive change in participants.

• CHIP graduates often experience weight loss, lower blood pressure, lower cholesterol, more energy, and more enthusiasm for life.

What is Virtual CHIP?•Virtual CHIP is a way to bring an evidence-based intensive lifestyle intervention to more people. It involves the same program components of a traditional CHIP class such as a trained facilitator, CHIP kit, and blood draws, but is done via conference calls with participants completing some of the course work on their own time.

•Virtual CHIP is designed to provide group support, a key component of traditional CHIP classes, through two weekly “community engagement” group conference calls.

•Accountability is built into Virtual CHIP through retention-check quizzes.

• Available to employees working in multiple locations.

• Ability to share program content with family or other support networks by watching videos at home.

• Less workday time commitment, which makes CHIP more attractive to busy professionals who would like to improve their health.

Getting Started with Virtual CHIP • Find a CHIP facilitator (either internal or external).• Arrange blood draws and Health Risk Assessment. • Promote Virtual CHIP as a wellness program to your

employees.• Set up a virtual information session and enrollment

process.• Create a detailed syllabus for participants.• Order CHIP kits.• Start the class.

Community Engagement Ideas

•Stress to the group the importance of building a community to enhance peer support.

•Collect photographs and short bio of all participants and share with group.

•Use a social network tool to allow the group to share ideas throughout the class.

•Call on people by name during the calls to share their experiences.

•Share CHIP-friendly recipes, and encourage people to try them and report to the group.

•Develop a walking competition within the class.

Sample Eligibility Requirements• Full-time, part-time employee or PRN.• Completed application for participation in the pilot.• Access to an Internet-enabled computer for virtual learning and a telephone for virtual

class/support calls. • Targeted Participants would have:

• BMI of greater than 29 OR• BMI greater than 25 plus 2 chronic health conditions such as:

• Hypertension• Diabetes• Pre-diabetes• High Cholesterol• Coronary Artery Disease• Arthritis• COPD• Asthma

• Others may be considered after interview with program facilitator• Complete biometric screening (tests performed no longer than 90 days before the

class start date can be accepted) • Testing will be offered for those who do not have recent laboratory data

Participants must attend (telephonically or in person) weekly classes to remain eligible and cannot miss more than one class per month.

Examples of Accountability

• Have participants sign a moral contract outlining expectations and commitment.

• If the company pays for participation and employee quits the program, they will have to pay back 50% of the fees through a payroll deduction.
