RIGOROUS PHYSICAL & OCCUPATIONAL THERAPY





Rehab Select is more than a typical rehabilitation inpatient facility.

We feature a physician-led, adaptive program that keeps patients with us only as long as they need (between 14-16 days on average), and rounds up to 7 days a week, and has interdisciplinary meeting each week to help work towards appropriate discharge for each short term patient.

Three of Rehab Select’s facilities are among the 23 Alabama nursing homes chosen to partner with Alabama Quality Assurance Foundation’s (AQAF) CMS Initiative to Reduce Avoidable Hospitalizations Among Nursing Home Residents.

•  We’re hands-on (and quick). Rehab Select patients are seen by a physician within 48-72 hours of initial complaint to assess course of action—much faster than the typical 10-day recommendation.

•  Our specialists are truly special. Rehab Select offers a

wider variety of specialists than most facilities of our type. From wound care to vascular surgery to psychiatry, our patients are seen more quickly and more often by experts, reducing the need for hospital transfer to simply get an assessment.

•  Our professionals are on-call and on point. Rehab Select facilities offer physician and nurse coverage either six or seven days a week (depending on location)—that’s significantly more time on the floor than most facilities.

•  We assess thoroughly. Our facilities utilize the INTERACT program implemented by the AQAF RN Coach. This walks nurses through a thorough checklist of assessment actions required before calling a physician. With proper procedure in place, fewer snap decisions are made, and fewer hospital trips are initiated.

•  We don’t stop at discharge. Quality care shouldn’t end when a person leaves our facility. Rehab Select partners with home health providers to ensure that once patients leave us, the continuity of care continues and a return trip to the hospital won’t be necessary.
