The ChallengeThe costs of treating a patient with a chronic disease are often dramatically increased when that patient also has a behavioral health issue. Health organizations are recognizing the benefits of integrated care, where primary care and behavioral health issues are treated by the same team, in the same office, holistically – providing better overall care for the patient at reduced cost. Payment reform and new delivery models consider behavioral health at the primary care setting essential to controlling costs. Medicare reimbursements of $68 annually to physicians who perform behavioral health assessments help promote the integration of behavioral health within the primary care process.
How SmartStream Helps Integrate Behavioral Health and Primary CareSmartStream, as an EMR extender, searches for whether the patient has completed a behavioral health assessment whenever he schedules an appointment or his insurance is verified. If no current assessment exists, SmartStream will send a notification to the patient to complete a behavioral health assessment, M3, or ‘Monitor My Mood’. The notification will include an automated hyper-link to M3 and is sent using secure email or secure mobile messaging. The objective is for the patient to complete the behavioral health assessment BEFORE their scheduled office visit. SmartStream detects when the patient completes the online assessment and can send reminders if an assessment is not completed up until the appointment date. Once an assessment is completed, SmartStream notifies the physician by secure message or secure email. An automated hyper-link included in the message takes the physician directly to the patient’s results. The physician will then know before the office visit whether the patient is experiencing any behavioral health issues.
