The discharge-planning process should start with screening as soon as the patient is admitted. Screen every type of inpatient,
from observation to one-day surgery patients.

DischargeCentral integrates with most health information systems, which means that the patient episode is automatically created in the application at admission. This automation will: • Kick-off a standardized
discharge process • Provide immediate access to
admitted patient status • Eliminate manual data entry

Efficient discharge-planning processes depend upon knowing the patient’s history and evaluating the patient to determine if the patient: • Is a readmit • Is a frequent flyer • Needs post-discharge care • Can return to a previous level of care

Collaboration across the care continuum is required to develop an effective post-discharge care plan. This includes
communicating with multiple providers at the next care setting to determine bed availability based on patient needs and patient
choice.

Hospitals are responsible for the initial implementation of the patient's discharge plan. This requires making sure that the next
level of care has all relevant clinical information before discharge.

Securely share patient clinical data electronically with selected post-acute providers, ensuring that the providers are informed and ready to receive patients before discharge. Specialty reports can also help: • Track transition and readmission practices • Identify issues and bottlenecks that increase patient LOS and
other key performance indicators • Develop interventions that lead to more efficient,
cost-effective care
