YOUTH REGISTRATION FORM FOR HOPE YOUTH

Return to Kim or Beth by June 6*

Adult volunteers will be given a t-shirt and be needed for the full day of 8:15 am until 3:30 pm. We willplace you with a small group of youth to complete a service project. We do require that all adults workingwith minors have a background check prior to the event. If you have any questions please contact Kristy.We are only able to have as many youth as we have adult helpers to transport them and are grateful forany time given! We could not do this without you and are very thankful to all the adults who take time tohelp our youth serve others!

June 23, Christ of the Bay Lutheran Church, 450 N. Laverne Dr. / Manna for Life/ Movie(Beth and Kim are not available so if Hope youth are interested we need an adult as well. Talk to Kim orBeth if interested).

June 30, Faith Lutheran Church, 2335 S. Webster Ave. / Heritage Hill / Timber Rattlers(In order to particpate June 30, you must register by June 9. There will be NO additions for this eventafter this date due to purchase of group tickets).

July 28, Pilgrim Lutheran Church, 1731 St. Agnes Dr. / Botanical Gardens / Pool 9

August 4, Our Saviour Lutheran Church, 120 S. Henry / Cerebral Palsy Center / Bay Beach

Ways to Volunteer:

I can drive and my car holds besides myself with seatbelts.

I will plan/work the lunch my church sponsors.
