BUSINESS PERMITS

OFFICE OF THE CITY MAYOR

• Applicant fills up the UNIFIED APPLICATION FORM which can be secured at the Business Permits & Licensing Office (Business Bureau) located at the ground floor of Sangguniang Panlungsod Building.

• APPLICATION FORM can also be downloaded at www.davaocity.gov.ph

• Barangay Clearance from the barangay

where the business establishment is

located (ORIGINAL)

• Current Year Community Tax Certificate

(Cedula)

• Sketch of business location in

separate sheet

• Certified List of Employees/Certificate of

NO Employee (ORIGINAL)

• Certificate of any Tax Exemption, if

applicable (Certified True Copy)

• Lease Contract duly notarized (if rented)

• Tax Declaration or Building Permit (if not

rented)

• Certificate of Registration of Business

Name from DTI (PHOTOCOPY)

• If represented, Special Power of Attorney

with CLEAR photocopy of ID of the person

authorized to transact & sign and a CLEAR

photocopy of ID of the taxpayer/applicant

(ORIGINAL SPA)

• Certificate of Registration from SEC(Corporation/

Partnership) or CDA (Cooperative) (CTC)

• Articles of Incorporation / Partnership /

Cooperation and By-Laws (CTC)

• Board Resolution/Secretary Certificate

authorizing person to transact & sign and

CLEAR photocopy of ID of signatory and

authorized person (ORIGINAL Board

Resolution/Secretary Certificate)

LICENSING OFFICER • reviews and verifies the application form

and the information provided by the applicant

• determines the correct line of business the taxpayer is engaged in

• assigns a corresponding application number (Business ID)

• enumerates all the Regulatory Offices (ROs) which must first approve the application made prior to the issuance of the permit

• releases the application form to the taxpayer for the approval of all the Regulatory Offices at the back of the form

After the necessary approval and assessment of all ROs, taxpayer must return and submit the application form to the Business Bureau:

• Licensing Officer receives and verifies the approval and assessment made by the ROs and all other supporting documents

• BB Chief approves the application form

• EDP encodes the application data, scans documents and releases application form to the applicant for the necessary payment at the City Treasurer’s Office

• Assesses fire safety fee on TOP, accepts payment, issues OR and Fire Safety Inspection Certificate (FSIC)

• Receives the application form, ORs issued and other necessary issuances/documents

• Generates Business Permit

• Licensing Officer reviews Unified Application Form & other documents and affixes initial

• BB Chief approves Business Permit

• Releases Business Permit with the corresponding Business Plate and other necessary documents
