Warm greetings from PUP-CSSD!
The Bachelor in Cooperatives second year students (BC 2-1) are required to undertake the Practicum Program this semester. We have selected your cooperative as one of our partners in this program.
As an enhancement course, the Coop Practicum aims to:
upgrade the students’ competencies in the various facets of cooperative management and leadership through exposure and hands-on experience;
Enable students to apply the cooperative principles, values and management theories and skills learned;
Impress the importance of human relations in the working environment; and Instill positive attitude, develop self-confidence and heighten motivation—the basic qualities
required of committed and socially responsible professional cooperator.
The deployment starts November 24, 2013, in time for the cooperatives’ year-end activities like annual inventory taking, preparation of financial statements, activity reports, and fulfillment of other tasks related to the holding of the annual membership meeting and other activities of the coop.
As partners, we shall share the responsibility of providing guidance, coaching and supervision during the practicum period. The College shall provide the Practicum Program Guidelines, hand-delivered by the student-trainee.
Thank you very much for accepting our students and for your untiring help in making this young future coop professionals amply prepared to be managers and leaders of a truly viable, sustainable and progressive cooperatives.

Course Description

This course is an in-service training program for students in partnership with the institutional and community based cooperatives. Students are fielded to the cooperative organizations and spend at least 200 hours of observation and cooperative practice. This course intends to acquaint the students on the different facets of cooperative development and organization and provide them initial hands-on experience on cooperative operations. This course shall also validate the theoretical learning in the university with practical applications in the real cooperative world. The trainee/s aside from the practicum shall conduct a scrutiny and assessment of the cooperative’s operations. This strategy shall allow the students to rationalize their learning during the practicum program.
Course Objectives:
After the course, the students should have been able to:
a. Upgrade the students competencies in the various facets of cooperative management and leadership through exposure and hands-on-experience and/or observation;
b. Enable the students to apply the cooperative principles, values, and management theories and skills learned;
c. Impress the importance of human relations in the working environment; and
d. Instill positive attitude, develop self-confidence, and heighten motivation – the basic qualities required of a committed and socially responsible professional and cooperator.
Course Orientation
This part of the syllabus shall acquaint the students with extensive overview on COOP 3076. Briefing the students on the course, its objectives, the subject matters/topics under discussion, and the philosophy under which the course finds relation to the vision, mission, goals, and objectives of the College and the University shall be the primary focus of this section. The use of varied learning activities for students and teaching methodologies by the instructor shall be included to advance the learning processes involved in this course. In the same manner, values, the appreciation of Filipino heritage, culture, customs, and traditions shall be integrated to make possible student’s consciousness of their role as responsible Filipinos and cooperators.

1. Monitoring of students/trainees in the coop training station (ocular or via phone).

Film showing, seminars, etc. for the students to be able to upgrade the students competencies in the various facets of cooperative management and leadership through exposure and hands-on-experience and/or observation, enable the students to possibly apply the cooperative principles, values, and management theories and skills learned, and impress the importance of human relations in the working environment; and
Corrective Learning Activities
Film showing on successful cooperatives followed by lecture and discussion. In the same manner, learning games will be utilized to highlight students understanding and learning abilities. Small group discussions on simple cooperative cases will be introduced.
Enhancement Learning Activities
A one-week enhancement seminar with simulation will be conducted to provide broader knowledge on cooperative operations and management before students will be deployed to coop training stations. Tests and examinations will be administered to check students learning on the different facets of cooperative operations.
Enrichment Learning Activities
Case problem analysis shall be used to facilitate and enrich students understanding of cooperative organization and practical applications. Term End Paper will be required plus the submission of coop practicum portfolio with a written report on the student’s sharing their learning and experiences on the coop practice.

Class Standing (Participation, and Interaction to include seatwork, reporting, recitation – oral and graded, assignments etc.)
Formative tests (quizzes and unit tests) Summative Tests (mid-term and final examination) Practicum & Performance Test: On-the-Job Training in a Primary Cooperative Have the student discuss before the class problems they have encountered in
their training station, let students orally present their coop organizational and operational scrutiny and have it presented before the class so that others can contribute in the deliberation and formulation of the general coop learning and experiences.

Introduction
This document contains the general policy and guidelines for the Polytechnic
University of the Philippines – College of Cooperatives & Social Development
(PUP-CCSD) for COOP 3076 – Cooperative Organization & Practical Applications.
The term “practicum” or “program” as used in this document shall both or
interchangeably refer to the Exposure Program for COOP 3076 unless otherwise
specified.
The contents are as follow:

I. General Objectives of the Program
In line with the College’s thrust to meet the cooperative sector’s demand for well-
rounded and technically-competent Cooperative Specialists and graduates, the Coop
Practicum Program aims to:
a. give Coop students exposure to the actual leadership, managerial and
operational aspects of the various types of cooperatives;
b. enable the students to gain experience, relate and apply the principles,
values and theories on cooperative leadership, management and
development;
c. give the students insights of the various operations, processes, he/she
learns in the College to real coop situations and problems
d. be acquainted with the techniques, processes, and controls presently used
in coop development and management;
e. develop and instill a positive attitude, self-confidence and self-motivation
required of a committed and socially responsible professional cooperator in
handling tasks; and
f. impress in the students the importance of human relations in the coop
working environment.

II. General Requirements of the Program
a. All students shall fulfill common requisites and requirements for course COOP
3076 – Cooperative Organization & Practical Applications.
b. A student must meet the required minimum field exposure of 200 hours or
approximately 8 weeks.
c. All Bachelor in Cooperatives sophomore students must undergo the Program
by enrolling in the corresponding course at second semester of the Academic
Year.
d. The faculty assigned to the subject shall serve as the Practicum Adviser.
He/she shall also coordinates with the host organizations regarding the
requirements of the course as well as their responsibilities as host.
e. A student is required to submit a periodic and final report at the end of the
practicum period.
III. Procedures & Guidelines
a. At the start of the term, the student/s shall be provided a list of host cooperatives
where they can choose from and have their practicum. The listing shall be
secured from the faculty adviser. Other directories can be downloaded from
www.cda.gov.ph.
b. If the student wishes to take practicum in a cooperative or cooperatives-related
institution not listed, he/she should approach the Adviser to obtain approval and
for proper coordination.
c. The student secures from the Adviser the official LETTER OF REQUEST (LR)
addressed to the cooperative concerned and endorsed by the adviser. A
student must not approach the Dean, Vice Dean, Chairs or Vice Chairs for
endorsement.
d. The student hand-delivers this letter to the cooperative and waits for
acceptance. The student is responsible for following-up the Letter of Request
and securing written approval from the cooperative.
e. When an ACCEPTANCE LETTER (AL) coming from the cooperative is received
by the student, he/she provides the adviser a copy of the letter.
f. The adviser must in turn confirm with the said cooperative regarding the
student’s acceptance to undergo practicum with them. The adviser and the
cooperative’s representative should agree on the period, schedule and other
details of the student’s practicum.

g. The student can then proceed with the practicum following the mutually
approved schedule.
h. The student is required to make a weekly report. The adviser checks this
weekly or monthly during their classroom meeting.
i. The student should at all times keep all documents pertaining to his/her
practicum. However, the adviser must be informed of any new documents or
forms that were accomplished in relation to his/her training. He/she complies all
these documents for inclusion in his/her report.
j. The student must comply with the general guidelines of the cooperative
regarding its own practicum program policy.
k. Upon completion, the student is required to obtain from the cooperative the
following documents:
Certificate of Completion
Final Evaluation Rating (The Evaluation Instrument shall be provided
by the PUPCSSD – Department of Cooperative & Social Development
or the cooperative if the coop has its own evaluation instrument for
trainee/s).
l. The student is then required to prepare a written report according to the
Reporting Format prescribed by the Adviser and submits this to the adviser on or
before deadline set.
m. The student shall hand-delivered to the host cooperative the “PRACTICUM
POLICIES & GUIDELINES”.
IV. Duties & Responsibilities of the Host Cooperative
a. Through the immediate supervisor or whoever shall be designated by the
Cooperative to take charge of the student’s practicum, the Cooperative ensures
that the practicum a student undergoes is aligned with the student’s
specialization and/or related to the student’s course.
b. Ensures a safe working environment for students undergoing practicum.
c. Assigns an immediate supervisor who will monitor the attendance and
performance of the student for the duration of the practicum, in coordination with
the Practicum Adviser.
d. Upon completion of the required number of hours/weeks, require the student to
submit to the Adviser a Certificate of Completion stating the student’s general

performance and that the student has satisfactorily completed all the
requirements.
e. Through the assigned supervisor, fills-out the Evaluation Form then sign, seal,
and submit this to the adviser through the students.
f. Inform the College through the adviser and/or the student other matters related
to the student’s practicum program, assistance and/or extension services that
the cooperative and the College may undertake.
g. Allow and entrust students with tasks like annual inventory taking, bookkeeping
and posting, clerical tasks, assisting in the coop’s education and training
activities, participating in the coop’s community programs and services,
observing regular coop officer’s meetings, helping out in the holding of the
annual membership meeting, etc.
h. Assist the student in the preparation of the scrutiny of the cooperative’s internal
and external environment through the use of the SWOT Analysis instrument.
V. Grading System
The adviser gives the student a grade based from the following criteria:

Grading Scale: The University grading system equievalency shall be applied when
giving final grade to the student trainee. Please refer to the illustration presented below:

VI. General Practicum Written Report Format
All students undergoing practicum are required to submit a Written Report at
the end of the term. Please refer to the prescribed report format and other details in
the succeeding pages.

adviser as soon as they are needed in the process of completing the program
requirements.

Reminders:
Use assorted colored fly leaf/cover for each sub sections/divisions of the project.
Use font arial 12 for the entire report/project except for the front and loose cover pages.
Font size for leaf/loose cover pages should be 36 for title headings.
Final Compilation should be Book-Binded. The Color should be Dark Red. Lettering/Text for the cover should be in Gold.
Use 8.5” by 11” bond paper for the project. Special paper may be used for the practicum

Cooperative Practicum Evaluation Instrument
This cooperative practicum evaluation cum research instrument is intended for the sophomore cooperative students. The objective of this instrument is to be able to monitor and assess student’s performance during their on-the-job training at the coops. It is of best interest of the College of Cooperatives & Social Development to determine how students carry out their theoretical learning in the university vis a vis the actual praxis of their learning in the cooperatives. This instrument consists of two (2) parts. Part I includes the coop profile etc. Part two indicates the areas for evaluation that uses the following rating scale: 4 = Outstanding, 3 = Very Satisfactory, 2 = Satisfactory, 1 = Fair and N/A – Not Applicable. The instrument should be filled out by the trainee’s immediate supervisor and/or coop officer. Please answer this instrument as objectively and completely as possible. We assure you that the response you will provide will be treated with strict confidentiality.
