TEACHING SESSIONS(FOR STATE USE)

Helps evaluate county performance

Calculated from Activity Reports Enter number of hours for month

on each activity May change in future

ISSUES WITH COMPLETING FORMS

Same people are reported each time under Unduplicated – People. People should only be

reported here the first time you teach them.

ISSUES WITH COMPLETING FORMS

People are reported in #1, but nothing is reported in #2. If you taught a person, you had

a contact with them. If you report in #1, you must report in #2.

ISSUES WITH COMPLETING FORMS

Numbers don’t match for 1a, 1b, 1c. These three numbers must be

the same!

ISSUES WITH COMPLETING FORMS

Numbers don’t match for 2a, and 2b. These two numbers must be the

same!

ISSUES WITH COMPLETING FORMS

Number of People is larger than Number of Contacts. Number of Contacts must be equal

to or greater than Number of People, or you are teaching pieces of people.

#Contacts/#People = Average #Contacts per Person Can not be less than 1.
