Proposed Intervention
The topic is to conduct a bi-weekly 90-minute fitness
and nutrition class in order to decrease childhood
obesity, reduce the risk of diabetes, and improve self-
esteem.
Objectives
Decrease childhood obesity and risks/comorbidities associated with it. (Goal: Lower each participants weight by 10% by the end of the one year program
Decrease risk of developing diabetes. (Goal: Lower Hgba1c in all pre-diabetic patients by 0.5-2 points by the end of the one-year program).
Decrease negative self-images, decrease risk of mental illness such as depression, and improve self-esteem by monitoring self-esteem levels. (Goal: Increased scores on Rosenberg Self Esteem Survey indicating improved Self Esteem).

Staff- APRN, Registered Dietitian, Licensed Fitness Instructor
Year long program, monthly ht & wt checks, q3mos Hgb A1c, monthly Rosenberg
Initial testing July 2015
Tues/Thur, 90 minutes
Participant Criteria
Overweight/Obese BMI 85-99%tile
Pre-diabetic Hgb a1c 5.7-6.4 (American Diabetes Association)
Pediatric patients ages 10-18
Budget
Indirect costs: APRN, RD, facility, lab work
Healthy Snacks $1/person x 50 people (48 weeks) = $2,400.00
Supplies (paper, pens, printer ink) = $ 250.00
Fitness instructor $60/hour x 48 weeks = $2,880.00
Facility = $0
Program Administrators (APRN & RD) = $0
Total $5,530.00
Evaluation
Lower weight by 10%, leading to decreased BMI
Decreased Hgb A1c level by 0.5-1 point
