Why Incident Reports?
• Incident Reports are required by licensing, DCFS, JJS as well as other consumers to document incidences such as serious youth behaviors, injuries to youth, reports or allegations of abuse and other items specified on the Incident Report form.
Why Incident Reports Cont.
• Incident reports allow the Village to keep a record of the youth’s problem behaviors. This can be beneficial with treatment planning for each youth.
• Incident reports allow the Village to keep its staff safe from liability in the case that there is a concern in the safety of the child

Incident Reports
• Timeliness: All incident reports need to be turned in within 24 hours of the incident. This is a contract requirement.
• Complete and Accurate: All information needs to be filled out and accurate.
• Quality of Work: The incident report should be written behaviorally specific, and should not include judgment calls, slang or derogatory terms other than items directly quoted.
