Hazardous Materials team conducting air monitoring for xylene and the building was ventilated. No patients were affected. The "all-clear" was given shortly after 5:00 p.m. on June 22, according to Newport News Fire spokesperson

Initiated follow up contact with TRHD EH Manager and provided an updated to the Acting Director. After contacting EH Manager providedassistance to Westmoreland EOC regarding food safety information due to power outage via press release. No request was made by Westmoreland for any additional assistance. TRHD EPC remains on standby to provide additional information if requested.

Introduction The Eastern Virginia Healthcare Coalition (EVHC) is a collaboration of health care organizations and providers, public health departments, and community partners working together to care for the community before, during and after an emergency.

The EVHC provides benefits, resources and funding awards to all health care organizations in all the various phases of Emergency Management.

Preparedness                                                                                                                                                       The EVHC region covers 26 different jurisdictions in Eastern Virginia. Our Core Membership consists of Hospitals, Public Health, Emergency Medical Services and Emergency Management in each of these jurisdictions. Our Partner Membership includes federal and private health care facilities including but not limited to Long Term Care, Skilled Nursing Care, Ambulatory Care Centers, Community Health Centers, Hospice, Home Health and various volunteer organizations.

The EVHC is governed by a Charter, Bylaws, and Memorandum of Understanding’s (MOU’s) that are overseen by an Executive Council consisting of one representative from each of Core Membership groups. The daily operations of the EVHC are handled by three full time staff members and one part-time staff member. This includes a Regional Health Care Coordinator, Regional Healthcare Coordination Center Manager, Long Term Care/Skilled Nursing Coordinator and an Inventory Management Specialist.

The EHVC holds monthly business meetings and has workgroups that focus on Training and Exercises and Preparedness and Response. All meetings are open to anyone from the Core and Partner Member organizations that have a direct duty or interest in Emergency or Disaster Management.

Additional preparedness activities of the EVHC include conducting healthcare facility site visits, assisting with planning Hazardous Vulnerability Analysis’s (HVA’s) and evaluating health care exercises. Based on information found during these site visits, from the HVA’s, and in exercise After Action Reports/Improvement Plans the EVHC offers quarterly training courses. Basic and Advanced Disaster Life Support, the National Incident Command System, Certified Hospital Emergency Coordinator and the American Burn Life Support are some of the courses that have been offered in the past and will continue to be offered in the future.

The EVHC host’s exercises through-out the year based on threats and risk identified that can or have occurred in the region. The majority of these Exercises involve coalition communication systems and Mobile Medical Assets. All exercises, trainings, site visits and monthly meetings are open to all Core and Partner Members.

Response                                                                                                                                                                               The EVHC maintains a Regional Healthcare Coordination Center (RHCC) overseen by a full-time RHCC Manager. The RHCC and RHCC Manager are the health care equivalent to a Regional Emergency Operations Center (EOC) and Regional Emergency Manager. During an emergency or disaster, the RHCC supports health care facilities with decision making and resource coordination. The regional RHCC bridges the on-site health care incident command team with the local EOC, specifically with the Emergency Support Function – 8 Medical and Public Health. During a large disaster or a “no-notice” emergency, the RHCC can also request a

specific resource immediately from other the five regional Healthcare Coalitions or the State EOC.

Through MOU’s every hospital in the region has agreed to share specific medical equipment with not only all other hospitals but with any other healthcare facility that has a specific need. The EVHC refers to this equipment as Mobile Medical Assets (MMA). The RHCC maintains an MMA Deployment Plan that is used for sharing between facilities during a pre-planned event or during an emergency. A list of MMA that is available through the EVHC RHCC and is listed in the final section of this document. It is important to note that list of MMA is not inclusive of all the MMA that may be shared.

For Non Clinical resource requests, a health care facility will need to contact their local EOC. Examples of Non-Clinical resources include

The EVHC maintains two fixed facility RHCC’s, one in Chesapeake and one in Newport News and two mobile RHCC’s that are available to travel any healthcare facility in the region. The RHCC Manager is assisted by EVHC Primary On-Call Staff during non-business hours and is available 365 days a year. Each RHCC relies on a minimum of 3 redundant communications systems for both daily and emergency communication.

The RHCC is responsible for consolidating and exchanging situational awareness for the region. The RHCC utilizing several communications systems for this including phones, radios, social media and web-based systems. One of the main communications systems the RHCC relies on is the Virginia Healthcare Alerting and Status System or VHASS.

VHASS is a web-based system that is used primarily to track the status of available health care beds as well as patients that are transferred from the field or residents that are transferred from one health care facility to another during an evacuation. VHASS also tracks healthcare resources on a daily basis specifically available Emergency Department or “MCI Beds” beds and licensed or “Clinical Beds” Other health care resources that can be reported using VHASS include:

Facility and Utility Operational Status

The RHCC Manager utilizes VHASS on a daily basis for non-emergency health care situational awareness, communication and resources coordination. All Core and Partner members designate at least one of their staff members to have an account and participate in VHASS during an emergency and at a minimum on a quarterly basis during exercises and bi-monthly communication tests.

Recovery and Business Continuity The EHVC support’s effected health care entities in the post-emergency recovery process by assisting the health care delivery system to restore operations and repatriate patients. The EVHC, along with its government partners (local, state, and federal), can assist its members with the local, state and/or federal process for reimbursement, reconstitution, and resupply.

Additionally, the EVHC may also assist with re-stocking and replacement of any Mobile Medical Assets in the local health care caches that are damaged or used during disaster operations.

The EVHC coordinates resources that can be used for Continuity of Operations such as medical tents, or portable air conditioners. Over the last several years some healthcare facilities in the region have taken advantage of coalition funding to install “quick connects” to their building for portable generators and many hospitals have had water wells installed on their property for non-potable water. These efforts will assist our region after a large disaster. The RHCC ensures that all healthcare facilities are a priority for other utility restoration and continuity and can work with healthcare organizations to locate other vendors or suppliers if their manager supply chain

 CHARTER 

 MISSION/PURPOSE 

The  Eastern  Virginia  Healthcare  Coalition  (“Coalition”)  is  a  collaboration  of  healthcare organizations  and  providers  and  suppliers,  public  health  departments,  Emergency  Medical  Services (including  transport  agencies),  Emergency  Management  Agencies  and  community  partners  working together  to  care  for  the  region before, during and after  an emergency.    The EVHC will  provide direct support  and  liaison  to  the  Virginia  Department  of  Health  (VDH)  and  Virginia  Hospital  and Healthcare Association (VHHA) in their administration of the U.S. Department of Health and Human Services (HHS) Office of the Assistant Secretary for Preparedness and Response (ASPR) Hospital Preparedness Program (HPP) Cooperative Agreement.    The  Eastern  Virginia  Healthcare  Coalition  endeavors  to  develop  and  promote  the  emergency preparedness, mitigation, response and recovery capabilities of local healthcare entities by:  Strengthening community medical resiliency, surge capacity and capabilities  Building relationships and partnerships  Developing emergency preparedness, mitigation, response and recovery capability guidelines  Facilitating communication, information and resource sharing  Maximizing utilization of existing resources  Develop and Coordinate training, drills, and exercises  Guiding and supporting the function of the Regional Healthcare Coordination Center   GEOGRAPHIC BOUNDRIES The  Coalition  is  an  inclusive  body  open  to  all  organizations/entities  that  provide  or  support  health services  within  the  following  26  jurisdictions  of  Eastern  Virginia  who  will  work  collaboratively  on emergency preparedness, mitigation, response and recovery activities: Accomack County, Chesapeake, Essex County, Franklin, Gloucester, Hampton, Isle of Wight County, James City County, King and Queen County, King William County,  Lancaster County, Matthews County, Middlesex County, Newport News, Norfolk,  Northampton  County,  Northumberland  County,  Poquoson,  Portsmouth,  Richmond  County, Southampton County, Suffolk, Virginia Beach, Westmoreland County, Williamsburg, and York County.  ORGANIZATION/STRUCTURE The Coalition has:  

1. A Chair and Vice Chair whose duties are outlined in Section III. 2. An Executive Council to conduct Coalition business as directed by the membership. 3. A Regional Healthcare Coordinator and staff for day to day operations. 4. Membership that includes core and partner organizations and entities. 5. Subcommittees  and  workgroups  as  requested  and  organized  by  the  membership  that  will 

function temporarily or long‐term, as needed.  FUNDING The Coalition is a non‐profit organization and receives directed funding for activities, operations and staff through grants from the U.S. Department of Health and Human Services (HHS) Office of the Assistant Secretary for Preparedness and Response (ASPR) Hospital Preparedness Program (HPP) Cooperative Agreement.   The Coalition recognizes the strong need to identify sustainment funding sources.

BYLAWS 

 I. MEMBERSHIP  A. Core Organizations 

1. Core organization members of the Eastern Virginia Healthcare Coalition are listed in Appendix A. 2. Core organizations sign an Organizational Memorandum of Understanding with the Eastern 

Virginia Healthcare Coalition. 3. Core organizations will create and maintain accuracy of an organizational account in the Virginia 

Healthcare Alerting and Status System (VHASS). 4. Core organizations appoint one primary and one alternate representative to the Coalition. The 

representative will have the authority to represent and speak on behalf of the core organization. 5. Core organization representatives serve term lengths as determined by the sponsoring 

organization. 6. Core organization representatives are eligible to fill Executive Council positions. 7. If an individual representing a core organization withdraws from participation, the core 

organization will appoint a new representative within 60 days. 8. Individuals may represent more than one core organization, but must clearly be acting in the 

interests of each represented core organization independently. 9. A Coalition member’s time during Coalition planning and activities is compensated by their 

organization without reimbursement from the Coalition.  B. Partner Organizations 

1. Partner organization membership to the Eastern Virginia Healthcare Coalition is open to the organization types listed in Appendix B that are located within the Coalition’s geographical boundaries. 

2. Partner organizations appoint one primary and one alternate representative to the Coalition. The representative will have the authority to represent and speak on behalf of the partner organization. 

3. Partner organization representatives serve term lengths as determined by the sponsoring organization.  

C. Invited Guests Collaborating organizations and subject matter experts may attend Coalition meetings and activities and may fully engage in Coalition discussions and other activities, but have no vote.  D. Member Responsibilities: 

1. Provide representation at Coalition meetings and activities. 2. Participate in collaborative regional preparedness planning. 3. Participate in the development of surge capacity plans, inter‐organizational agreements, and 

collaborative emergency response plans. 4. Contribute to meeting Coalition priorities, goals, and contractual deliverables. 5. Respond to regional emergencies and disasters in collaboration with other members. 6. Support the activities and operations of the Regional Healthcare Coordinating Center. 7. Work to implement emergency preparedness and response capability guidelines within the 

organization’s activities.

8. Vote on the members EVHC Leadership  E. Membership Roster A current roster of member organizations, including core or partner designation and contact information, will be maintained by the Regional Healthcare Coordinator. A meeting attendance roster of member organizations will also be maintained.  F. Changes in Member Representation All changes in member representation must be submitted in writing to the Coalition on a new Organizational Memorandum of Understanding, endorsed by the representative’s organizational senior leadership.  

II. MEETINGS and VOTING  A. Scheduling  Coalition meetings will be scheduled on the first Thursday of each month at 9:00 a.m. Electronic notice and agendas for all meetings shall be transmitted at least 5 working days in advance of the meetings.  B. Venue Meetings will be held at locations convenient for members with Web conferencing as an alternative to in‐person attendance at meetings.  C. Attendance Meetings may be attended in person, by conference call or by other electronic means.  D. Emergency meetings Emergency meetings may be convened at the request of the Coalition Chair provided that electronic notice is given to each member at least 48 hours prior to the proposed meeting stipulating the time, place and objective of the meeting. No business may be transacted at an emergency meeting except that specified in the notice.  E. Quorum and Voting No quorum at a Coalition meeting is required for a vote.  Simple majority will rule though compromise and collaboration will always be sought.  III. LEADERSHIP  A. Coalition Chair 

1. Election a. The Chair will be elected for a one year term for each calendar year by the core and 

partner organization members. b. To be eligible to stand for election the individual will be a representative of a core 

organization and have attended 50% of that calendar year’s Coalition meetings. c. The Vice Chair will be placed into nominations for Chair on the agenda of the November 

meeting agenda for action and election during the December meeting.

i. A motion from the floor for additional candidate nominations may be brought up for a vote before the Coalition. 

ii. With a passed motion to call for additional nominations, the floor will be opened for additional nominations to the position of Chair. 

iii. Only individuals accepting a nomination will be considered for election. iv. A motion to close nominations must be made and passed to officially identify 

the individuals nominated for Chair at the December meeting. d. In the event of the unexpected departure, resignation, or removal from office, the Vice 

Chair replaces the Chair, subject to a ratification of the Executive Council at their next meeting. 

e. Nominations shall be made in a fashion to maintain the multi‐disciplinary, multi‐regional composition of the Coalition.

function of the Coalition.  B. Coalition Vice Chair 

1. The Vice Chair will be elected for a one year term for each calendar year by the core and partner organization members. 

a. Nominations for Vice Chair will be solicited at the November meeting for action and election during the December meeting. 

i. A motion from the floor for candidate nominations will be brought up for a vote before the Coalition. 

ii. With a passed motion to call for nominations, the floor will be opened for nominations to the position of Vice Chair. 

iii. Only individuals accepting a nomination will be considered for election. iv. A motion to close nominations must be made and passed to officially identify 

the individuals nominated for Chair at the December meeting. 2. To be eligible to stand for election the individual will be a representative of a core organization 

and have attended 50% of that calendar year’s Coalition meetings. 3. The names of individuals nominated for Vice‐Chair will be listed on the December meeting 

agenda for elections taking place during the December meeting. 4. Duties are the same as Chair except Vice Chair will not chair Coalition meetings unless Chair is 

absent. 5. Acts for the Chair in his/her absence. 6. Nominations shall be made in a fashion to maintain the multi‐disciplinary, multi‐regional 

composition of the Coalition.

C. Regional Healthcare Preparedness Coordinator 1. Hired or contracted by the Virginia Hospital and Healthcare Association. 2. Responsible for management, day to day operations, and administrative support of the 

Coalition. 3. Supervise the Regional Healthcare Coordinating Centers (RHCC). 

 D. Executive Council The Executive Council is comprised of the Chair, Vice‐Chair and one (1) representative from the following membership groups: Southside Hospitals, Peninsula Hospitals, Long Term Care, End Stage Renal Disease, Emergency Management Agency, Emergency Medical Services, and Public Health. 

1. Election a. To be eligible to stand for election to the Executive Council a member representative must 

have attended two (2) of the last four (4) Coalition meetings. b. Nominations for membership to the Executive Council will occur during the November 

meeting. c. The names of individuals nominated for the Executive Council will be listed on the 

December meeting agenda for elections taking place during the December meeting. d. In the event of the unexpected departure, resignation, removal from office or any new 

vacancies on the Executive Council will be filled as soon as possible by vote of the coalition members. 

e. Nominations shall be made in a fashion to maintain the multi‐disciplinary, multi‐regional composition of the Executive Council. 

f. Executive Council members will serve for two years, except Chair and Vice chair shall be one year.  

g. There is no limit to the number of successive terms an Executive Council member may serve.  

2. Duties and Scope of Responsibilities a. Lead the strategic planning process and continued development of the Coalition. b. Provide budgetary oversight and approve budget requests. c. Provide oversight on development of regional and health sector emergency preparedness 

plans. d. Provide oversight and approve recommended work‐plans and budget requests of Coalition 

subcommittees, workgroups and projects.  

F. Conducting Business and Voting 1. A quorum is necessary to conduct official Coalition business at an Executive Council meeting  2. A quorum is 75% of Executive Council members participating in the meeting or vote. 

a. If a quorum is not present at a meeting, business will take place under the condition that any motions put forth to a vote will be presented to absent Executive Council members via email, conference call or other electronic means in order to receive a quorum vote. A reasonable amount of time will be allowed for receipt of absentee votes. Votes will be received no more than 5 business days from the date of the meeting.  Notice will be sent no later than the next business day following the meeting. Such special votes will only be held if discussion has occurred at a previous meeting. If a quorum is not obtained the motion fails.

b. Proxy voting is allowed. Attendance requirements apply to proxy votes. Proxy instructions must be sent to the Coalition Chair in writing prior to the meeting. 

  IV. SUBCOMMITTEES and OTHER GROUPS  The Coalition may establish subcommittees and workgroups to perform such tasks and duties as deemed appropriate by the Coalition. Each subcommittee and workgroup will have a charter to define duties and responsibilities and its Chair will provide progress reports at Coalition meetings. Members are appointed to subcommittees and workgroups as approved by the Coalition Chair and subject to approval of the membership. The following standing workgroups have been established to ensure the operations and activities of the Coalition are maintained: 

1. Budget Workgroup 2. Regional Healthcare Coordination Center Workgroup 3. Training and Exercise Workgroup 

 Outcomes of subcommittees and other groups are ratified by the Executive Council before action.  V. AMENDING THE BYLAWS  Amendment of these bylaws may be proposed at any meeting of the Coalition.  The amendment shall be acted on at the following Executive Council meeting provided a copy of such proposed amendment(s) are distributed at least thirty (30) days in advance of such meeting or fully stated at the first meeting, and attached to the electronic notice for that meeting. A two‐thirds majority vote by the Executive Council is required for the amendment to carry.  VI. LEGAL DISCLAIMER  Indemnification and Limits of Liability  This Charter and Bylaws shall not be interpreted or construed to create an association, joint venture separate legal entity or partnership among the Coalition members or to impose any partnership obligation or liability upon any Coalition member. Further, no Coalition member shall have any undertaking for or on behalf of, or to act as or be an agent or representative of, or to otherwise bind any other Coalition member.  No Coalition member shall be required under this Charter to indemnify, hold harmless and defend any other Coalition member from any claim, loss, harm, liability, damage, cost or expense caused by or resulting from the activities of any Coalition officers, employees, or agents acting in bad faith or performing activities beyond the scope of their duties. In the event of any liability, claim, demand, action or proceeding, of whatever kind or nature arising out of participation in a unified health and medical response to an emergency, the Coalition member agrees to indemnify, hold harmless, and defend, to the fullest extent of the law, each Coalition member, whose only involvement in the transaction or occurrence which is the subject of such claim, action, demand, or other proceeding, is the approval of this Charter.
