Johnny appeared to be a happy child, and no one ever reported him as having any behavioral problems.

He enjoyed spending time with his older sister, and they would often play outside together.

Johnny’s relationship with his parents was good, but they had to work many hours to make ends meet.

Age Two to Six

Age Six to Eleven

At the age of six, Johnny began to be defiant and would not listen to authority figures.

Johnny’s kindergarten teacher reported that he had difficulty following directions.

As Johnny grew, school officials noted that his defiant behaviors were increasing, and that he was not attending school on a regular basis.

Many attempts were made by the school to assist the family with his ongoing behaviors without success.

Johnny was failing all his classes, and becoming aggressive with other children.

Age Twelve

Johnny had been identified a child in need of services at the age of twelve, due to truancy and continued behavioral issues.

The Court ordered that a social history be completed, and provide recommendations for services.

It was found that Johnny’s father was an abusive alcoholic, and his mother was addicted to anxiety medication.

Neighbors stated that Johnny and his sister were often left alone late at night, and played in the yard well after midnight.

The recommendations were for Johnny’s parents attend AA and/or NA, and that Johnny be monitored by the court.

Age Fifteen

Johnny is charged with assault at age fifteen.

Johnny became angry at a peer during class after overhearing talk about his “drunken” family. Johnny assaulted the peer, and he was sent to the Detention facility.

Johnny was tested for substances while in Detention, and tested positive for Marijuana.

Court Order

Johnny appeared before the court, and it was recommended in the amended social history that he receive services to address issues in the home, community, and school.

These services included Mental Health/Substance Abuse Assessment, and to comply with any recommended services from the assessment.

The Judge ordered that Johnny and his family begin services to address problems identified.

Johnny Meets Debbie

Johnny and his family meet with Debbie for initial intake.

Debbie explains the therapeutic process, and has Johnny’s parents complete require forms.

Debbie administers a complete mental health and substance abuse assessment.

Debbie makes recommendations for treatment based on findings of the assessment.

Debbie answers any questions Johnny and his family may have.

Johnny and his family are contacted by intake worker.

Johnny and family meet with therapist for mental health/substance abuse assessment. Treatment plan is developed.

Johnny meets therapeutic mentor.

Johnny meets therapeutic vocational mentor.

Team meets and reviews treatment plan. Mentors are provided goals and objectives based on therapist treatment plan. Services are implemented.

Therapeutic Vocational Mentoring – Job Readiness; GED preparation; Remediation; Life Skills Training; Transportation; Crisis Intervention; 24/7 contact.

Also Included:

Each participant is eligible to receive a monthly stipend up to $200 if they participate in all services provided in the program.

Services are reviewed every 30 days to ensure the needs of the client are being met.

A monthly report will be provided to referral source, and treatment team meetings will be scheduled as needed.

Program 1 - $65.00 per hourIncludes mental health/substance abuse assessment; therapeutic mentoring; vocational services; anger management/substance abuse group 1X weekly; parent support group 1X month, 24/7 access to therapist.

Program 2 - $65.00 per hour + $200.00 per month (therapy)Includes mental health/substance abuse assessment; individual/family counseling; therapeutic mentoring; vocational services; anger management/substance abuse group 1X weekly; parent support group 1X month, 24/7 access to therapist.

** vocational services include a stipend up to $200.00 monthly to each child if actively participating in ALL aspects of the program.
