SCOPE OF WORK 1.1 The purpose of this Solicitation is to establish pricing for the maintenance and repair of Rack & Pinion elevators at JEA’s Northside Generating Station (NGS). 1. SCOPE OF WORK

1.1 The purpose of this Solicitation is to establish pricing for the maintenance and repair of Rack & Pinion

elevators at JEA’s Northside Generating Station (NGS). The work shall include scheduled Preventive

Maintenance (PM), maintenance repairs and adjustments (including the purchase of parts/materials and

subcontractor services), and the scheduling of and participation in the State Safety Inspections and tests

for eight (8) rack & pinion elevators. In addition, the Contractor shall be capable of providing rack &

pinion elevator capital improvement upgrades, if requested, during the term of the contract.

2. GENERAL REQUIREMENTS

2.1 These specifications are a statement of the level of work and services that are to be provided under this

contract. They are not intended to be, nor shall they be construed as limiting specifications or

requirements. At a minimum, the Contractor shall be required to take all steps and measures which

would be taken by a prudent owner to maximize the life expectancy of the elevators and related systems

and to ensure safe and reliable rack & pinion elevator operation.

2.2 The Contractor shall provide full elevator maintenance service, in compliance with the ASME Safety

Code requirements, the elevator manufacturer’s recommendations, the Elevator Industry Field

Employees’ Safety Handbook, National Electrical Code, and all other applicable laws, regulations, rules,

ordinances, codes, etc. Full maintenance service is defined as all services, repairs, inspections and

testing necessary to maintain all rack & pinion elevators, appurtenances and accessories in a fully

operational mode at all times except for pre-scheduled downtime including all labor, parts and materials.

Reference is made to Section 8.6 of the Code which establishes requirements that must be followed.

2.3 The Contractor shall hire qualified personnel to perform monthly elevator inspections and/or

maintenance in order to monitor the operational status of the elevator systems.

2.4 The Contractor shall provide all labor, materials, small hand tools, and equipment, including all Personal

Protective Equipment (PPE) (at a minimum, safety glasses, hardhats, hearing protection, steel-toed

boots, and standard work gloves shall be worn) as necessary to perform Rack & Pinion Elevator

Maintenance at NGS. Small hand tools shall include the kind and type commonly used in the industry

and shall include portable radios and/or cell phones.

2.5 The Contractor shall provide, as needed, rack & pinion elevator maintenance service TWENTY-FOUR

hours/day; SEVEN days/week. This shall include scheduled preventive maintenance activities,

Operations & Maintenance (O&M) repair (scheduled and call out) as well as capital upgrades,

parts/material purchases, subcontractor labor, and subcontractor annual inspections (i.e. Contractor

participation in the annual State Safety Inspection).

2.6 The Contractor shall provide the JEA Representative with one copy of the ASME A17.1 Safety Code for

Elevators and Escalators, and, the Elevator Industry Field Employees’ Safety Handbook. In the event a

2.9 Estimates for maintenance and repairs shall be submitted in writing to the JEA Representative and must

include, at a minimum, the estimated labor hours and parts/material costs associated with the work.

2.10 All equipment shall be maintained in accordance with the elevator manufacturer’s recommendations, the

best practices of the industry, and applicable codes, standards, and regulations. A laminated copy of the

maintenance program must be maintained and provided to the JEA Representative (or their designee).

2.11 All scheduled work that requires a rack & pinion elevator to be taken out of service shall be coordinated

with the JEA Representative (or plant Control Room) and the Contractor shall report to JEA

Representative (or plant Control Room) the status of elevator equipment or systems not operating by the

close of each work day. Any equipment or system that is non-operational by the official start time of the

occupants shall be reported to JEA Representative (or plant Control Room) that morning and the

Contractor shall be responsible for the installation of barricades for non-operational equipment and/or

systems, as deemed necessary by JEA.

2.12 In order to provide for uninterrupted JEA business, unnecessary out-of-service periods for elevators must

be minimized. Therefore, it is essential that rack & pinion elevator repairs be performed in a timely

manner.

2.13 The Contractor shall not change or alter the existing elevator equipment or any electrical circuits, wiring,

controls, or sequencing without written authorization from JEA. If changes are authorized, the

Contractor shall make appropriate revisions to the elevator drawings and/or specifications. All parts or

components installed, or improvements made, by the Contractor during the term of this contract shall

become and remain the property of JEA. Contractor shall not initiate any out of scope repairs without

obtaining prior written authorization from the JEA Representative. Repair authorization will be per JEA

normal process.

3. SPECIFIC REQUIREMENTS

3.1. Contractor shall maintain a branch office, fully staffed with maintenance personnel, located within a 50

mile radius of JEA Northside Generating Station; 4377 Heckscher Drive, Jacksonville, Florida, 32226

for the life of contract.

3.2. The Contractor shall have in its employ, at all times, a sufficient number of capable, licensed and

qualified employees to enable it to properly, adequately, and safely manage, operate, maintain and

account for the elevator system. The Contractor’s certified elevator technician(s) shall possess a State of

Florida Elevator Technician Certification(s) and have a minimum of three (3) years of experience in the

maintenance of Rack & Pinion elevators. Any changes in Contractor personnel related to or working

with this contract shall be provided to the JEA Representative in writing within 5 days of such a change.

3.3. The Contractor shall require all employees to wear distinctive clothing (e.g. shirt with company logo)

with name tag for ready identification.

3.4. Elevator Technician Helpers may perform work and/or repairs for which they are qualified. Elevator

Technician Helper qualifications are determined by state, local and/or city requirements.

3.5. Contractor will maintain an on-site weekly Maintenance and Task Log for each Rack & Pinion elevator

outlining regularly scheduled preventive maintenance and periodic requirements for the equipment. The

Log shall state the daily hours, equipment serviced, type of service (scheduled PM or Repair) and parts

replaced. These Logs shall also identify the exact frequency of such requirements and contain a record

of when and by whom the specific task was performed. These Logs will be made available to JEA upon

request.

3.6. Contractor shall maintain a web-based reporting and monitoring service capable of documenting all

maintenance tasks performed for each JEA rack & pinion elevator.

3.7. The Contractor shall provide approved metal waste containers for flammable and combustible wastes

associated with the performance of this contract. Flammable and combustible wastes shall be removed

from the premises daily. The Contractor shall provide approved metal storage cabinets for any

flammable materials associated with the performance of this contract. Storage of flammable and

combustibles liquids shall be limited as much as possible and shall conform to the latest edition of NFPA

30, Flammable and Combustible Liquids Code.

3.8. All parts installed on rack & pinion elevators covered by this specification shall be new and genuine parts

supplied by, or certified by the Contractor as equivalent to, the original equipment manufacturer, or its

successor prior to the installation of the part. When replacing existing parts or installing new parts, the

Contractor shall install energy efficient parts to the maximum extent possible

3.8.1 Contractor shall be responsible for obtaining all OEM manuals from the original

manufacturers of all elevator equipment covered under this specification.

3.8.2 Contractor shall provide the JEA Representative copies of all applicable OEM manuals.

3.8.3 Contractor shall notify the JEA Representative when parts are utilized from JEA inventory.

3.8.4 The Contractor shall coordinate with JEA to obtain and maintain a stock of applicable

replacement parts to minimize the interruption of elevator service. Particular attention shall

be given to parts which are “proprietary” and/or “long lead time” items.

3.8.5 JEA reserves the right to purchase parts directly from the manufacturer.

3.9 If painting is required, the Contractor must comply with the ANSI color coding system outlined in the

ANSI A13.1, Scheme for the Identification of Piping Systems, and maintain the identity (identification

information) of the equipment. The Contractor must obtain the approval of the JEA Representative

before storing anything in machinery spaces. Operating supplies such as lubricants, rags, cleaners, etc.,

shall be properly secured in containers approved by JEA. Storage shall not negatively impact the means

of egress, fire protection systems, and emergency lighting; nor, shall it significantly increase the amount

of combustible material in the machinery space, this only applies if acceptable by ASME A17.1 and 2

safety code. Clearances from electrical equipment shall be maintained as required by NFPA 70, the

National Electrical Code.

3.10 Asbestos Containing Material (ACM) may be present within the rack & pinion elevators. The Contractor

is responsible to maintain the existing asbestos containing material, if present, in an intact and/or non-

friable condition. Locations of asbestos containing material may include the following:

3.10.1 Floor tiles or resilient flooring material inside of the car.

3.10.2 Adhesive flooring mastic inside of the car.

3.10.3 Soundproofing and waterproofing application on the exterior of the car.

3.10.4 Brakes and cable stops.

3.10.5 Circuit boards

JEA will inform the Contractor, to the best of their knowledge, which areas in the plant facilities have

asbestos containing materials.

4.1 IT IS EXTREMELY IMPORTANT THAT THE CONTRACTOR AND JEA WORK

TOGETHER TO ADDRESS ANY SAFETY CONCERNS SUCH THAT POTENTIAL

ACCIDENTS ARE AVOIDED.

4.2 All employees of the Contractor (including all Subcontractors), who perform work on JEA P property,

shall be JEA Safety Qualified. Contractor Supervisors/Foremen will be required to attend the Safety

Leadership Development program offered through the Northeast Florida Safety Council (NEFSC) or an

equivalent program as required by the JEA Contractor Safety Program.

4.3 Site specific safety training will be required to work at the NGS plant site. The JEA Safety Department

or JEA Representatives will provide a PowerPoint Training module for the NGS facility. Contractor is

responsible for ensuring ALL personnel have received the appropriate safety training, as required by JEA

Contractor Safety Program, and shall submit a roster of the employees who received the training.

4.4 Contractors are required to wear proper Personal Protective Equipment (PPE). PPE minimums include

steel-toed boots, hard hat, safety glasses, and standard work gloves. Hearing protection is required while

working in electric plant power block areas and when operating machinery or equipment (including

saws). Ripped jeans, shorts, tennis shoes, sleeveless shirts, and shirts with offensive logos or messages

are not permitted.

4.5 Contractor shall maintain a safe work environment at all times. Contractor shall keep their work areas

free of trip hazards daily and shall maintain excellent housekeeping through the completion date of the

project. Contractor shall provide any and all dust curtains, temporary partitions, walk-off mats or any

other barricade or process necessary to keep the job site clean.

4.6 Contractor shall abide by the JEA Hot Work Permit Program, Lock Out/Tag Out Procedure and the

Confined Space Entry Procedure.

4.7 Contractor shall abide by the JEA Contractor’s Safe Work Practices Manual.

5. ENVIRONMENTAL

5.1 JEA is under strict environmental standards with respect to all construction activities, including

purchasing, delivery, erection, and operation / maintenance of equipment.

5.2 Violations of standards may result in fines against and/or imprisonment of the guilty parties. The

Contractor’s work shall be in compliance with all applicable environmental standards. The Contractor is

liable for breeches of permit conditions instigated by its personnel.

5.3 JEA will assist the Contractor in environmental compliance by providing information upon request and

monitoring the work. Environmental standards are contained in permits, permit application materials,

Conditions of Certification, stipulations, and compliance documents. Copies of these documents are

available for inspection at the JEA Environmental Compliance office. The Contractor shall cooperate

fully with JEA in insuring compliance, including participating in meetings, implementing the JEA

Representative’s instructions, and performing other actions as requested.

6. SECURITY

6.1 Contractor shall supply a list of names of the personnel that may be assigned to enter JEA plant facilities

for rack & pinion elevator repairs during and after normal working hours. Photo ID’s are required for

all personnel that will be working on JEA property.

6.2 Only authorized Contractor personnel will have drive-on plant access. Contractor is responsible for

transporting their personnel from the JEA designated parking area to their work area.

6.3 A JEA issued security badge shall be visible at all times while on JEA property.

6.4 Background checks and mandatory training may be required for entry to NERC regulated spaces.

6.5 Parking on JEA property shall be approved through the JEA Representative. Parking for Company

vehicles and Company personnel vehicles is limited to four (4) vehicles within the NGS facility. All

Company vehicles driven onto plant sites must be properly identified with Company placards.

Additional parking for personnel is available off-site in the designated Contractor parking area.

6.6 Certain work for JEA may entail entering maritime facilities, such as the fuel loading docks and/or the

adjoining JAXPORT properties, all of which are governed by the Transportation Security Administration

(TSA). In order to gain access these facilities, the Contractor must obtain in advance, at their own cost, a

Transportation Worker Identification Credential (TWIC). The estimated cost is $125.25 and is valid for

five (5) years. Eligibility for a TWIC is subject to certain immigration and criminal background check

requirements. Additional details may be found at www.TSA.gov.

7. RESPONSE TIME

7.1. Contractor must provide a monitored 24 hours a day, 7 days a week telephone number and be able to

provide email notification to JEA for dispatch response.

7.2. The Contractor agrees to a maximum of a twenty (20) minute call back.

7.3. The Contractor agrees to a maximum of a two (2) hour on-site response time for non-emergency service

calls, unless otherwise agreed upon.

7.4. The Contractor agrees to a maximum of a one (1) hour on-site response time for emergency calls.

8. WORK HOURS

8.1. Hours for Straight Time Work are 7:00 AM to 4:00 PM or as scheduled by JEA.

8.2. Hours for Call Out Overtime (OT) are 4:01 P.M. to 6:59 A.M. Monday through Friday, weekends and

emergency only (with pre-arrangement for flextime) by JEA.

8.3. JEA Holidays are as specified in Section 23 of this Technical Specification.

9. NON-EMERGENCY SERVICE CALL

9.1. All service calls performed during Straight Time Work Hours will be defined as a “Non-Emergency

Service Call” and shall be invoiced at the Straight Time Hourly Rates indicated in the Respondent Rates

Workbook. Upon notice of a Non-Emergency Service Call, the Contractor shall be on-site within two (2)

hours of the request, unless otherwise agreed upon. The service request by JEA will typically be made

by phone call to the Contractor.

9.2. In some instances, a Non-Emergency Service Call may be mutually scheduled between the JEA

Representative and the Contractor. In the event the Contractor is not able to fulfill the scheduled

appointment time, the Contractor shall notify the JEA Representative as soon as they are aware of the

change.

9.3. Upon arrival at the plant site, the Contractor shall check-in with the JEA Representative (or NGS Control

Room) who will provide a description and location of the problem, to the extent possible. Upon

troubleshooting the problem, the Contractor will either place the system back into normal operation or

provide the JEA Representative (or NGS Control Room) with a detailed explanation of the problem and

verbal estimate to complete to the repair. The Contractor shall follow up the verbal estimate with a

written estimate to the JEA Representative within 24 hours. Upon check-out from the NGS plant site,

the Contractor shall provide a Service Report to the JEA Representative (or NGS Control Room) for

approval and signature.

9.4. Service Reports: An electronic or hard copy of the Service Report shall be submitted to the JEA

Representative (or NGS Control Room) upon completion of a maintenance service call.

9.4.1. Upon check-out, the Contractor shall submit a legible Service Report describing, at a minimum,

the work performed, parts replaced, hours worked, date worked, time-in, time-out, and

Technician’s name. Upon review and approval, the JEA Representative will sign and date the

acceptance of the Service Report.

9.4.2. The Service Report shall serve as part of the backup documentation for payment invoicing.

9.4.3. If the Service Call uncovers a problem that has the potential to be a major problem, or if it may

be the cause for delays during a plant shutdown, the problem must be brought directly to the

attention of the JEA Representative (or NGS Control Room) so that a plan of action can be

formulated for the timeliest repair to the equipment or system.

9.5. Inspection Reports: Inspection Reports shall be submitted to the JEA Representative after the

completion of each inspection.

9.5.1. The Contractor shall indicate on the first page of the report, the frequency period (i.e., Monthly,

Semi-Annual, or Annual) the date of the inspection, the Inspector’s name and facility elevator

and location.

9.5.2. A verbal account of the inspection shall be provided to the JEA Representative at check-out for

the day(s) prior to submission of the final written Inspection Report. The turnaround time for

the Inspection Report shall not exceed two (2) weeks. The Inspection Report shall be submitted

electronically to the JEA Representative.

9.5.3. If a problem is found that has the potential to be a major problem, or if it may be the cause for

delays during a plant shutdown, the problem must be brought directly to the attention of the JEA

Representative (or plant Control Room) so that a plan of action can be formulated for the

timeliest repair to the equipment or system.

10. EMERGENCY SERVICE CALL

10.1. The Contractor shall demonstrate that they have capable personnel and established procedures in place to

respond to an “Emergency Service Call” - 24 hours per day, 7 days per week, inclusive of all holidays. An

Emergency Service Call shall be invoiced at the Overtime Hourly Rates indicated in the Respondent

Rates Workbook under the requirements stated below.

10.2. When emergency repair services are requested, the JEA employee making the request must be able to obtain

voice contact with a person capable of assuring that the request has been received and that an appropriate

response will be initiated. The Contractor shall provide a telephone response to the JEA employee making

the request within twenty (20) minutes. Contractor personnel must be on-site ready to perform required

services within one (1) hour, after the 20 minute call back of the initial request. JEA retains the right to

assess the Contractor for any damages caused by the Contractor’s failure to respond within the specified time

limitations. Company may be excused for not meeting a Response Time if the delay was due to Force

Majeure events as defined in the Contract Terms & Conditions. Conditions such as employee illness, vehicle

problems or similar situations are not acceptable reasons for an untimely response.

10.3. If the Contractor receives a request for an Emergency Service Call due to an issue that could have

reasonably been detected during the periodic inspections, but was not reported by the Contractor, the

Contractor is required to bear the full cost to resolve the emergency repair services.

10.4 The Contractor must follow these requirements when responding to an Emergency Service Call:

10.4.1 Determine the facility location and elevator number.

10.4.2 Record the date, time, and person receiving the call.

10.4.3 Determine the immediate condition of the passengers.

10.4.4 If passengers complain of injuries, determine the number of passengers and brief description

of their condition, and whether medical attention is required.

10.4.5 Contact elevator maintenance personnel to immediately release passengers.

10.4.6 Contact emergency personnel if medical attention is necessary.

10.4.7 Contact JEA using the submitted Emergency Service Call Numbers List. Notify JEA

personnel of the situation and what immediate actions have been taken to date.

10.4.8 If possible, maintain communications with the passengers while making these contacts.

10.4.9 If not possible, reestablish communications with the passengers once emergency contacts

have been notified.

10.4.10 Record all actions taken as a result of the emergency service call, including total time elapsed

from initial call to passenger release, time elevator was returned to service, and the cause and

resolution of the shutdown.

11. SUBCONTRACTING

11.1 JEA reserves the right to approve or disapprove any subcontract and any subcontractor selected. NOTE:

The Contractor must subcontract for safety and/or certification inspections with individuals/firms

independent of the maintenance Contractor’s organization.

11.2 The Contractor is responsible for certifying to JEA in writing, that all of its subcontractors involved in

the performance of this contract have been made aware of the provision to report to JEA any

recommended maintenance and/or repair items that are necessary to maintain the physical integrity of the

interior or exterior of the facility and to ensure that it is free of missing components or defects.

11.3 The requirements of the JEA Contractor Safety Program apply to ALL Subcontractors.

12. ITEMS TO BE SUPPLIED BY THE CONTRACTOR

12.1 Parts/Materials and equipment should be selected and used in a manner that reduces exposure to JEA

personnel, and that reduces the impact on the environment as compared to other products and services

used for the same purpose, in accordance with Executive Order 13423 and RCRA Section 6002,

Comprehensive Procurement Guidelines (http://www.epa.gov/cpg/products/tissue.htm).

12.2 The Contractor will furnish all parts/materials, equipment, and employee training necessary for the

performance of the work of this contract unless otherwise specified herein.

12.3 The Contractor will furnish to JEA all Safety Data Sheets (SDS) for any materials used in the

performance of this contract five (5) days after award. All new products used during the life of the

contract must have SDS provided to the JEA prior to use. All chemicals used will be those commercially

available which meet Federal, State and local codes. Contractor shall maintain the SDS in a location

accessible to all employees and will advise the JEA Representative of their location. The SDS will be

available for inspection by JEA on request. The Contractor will take every precaution to ensure that only

safe products are used. Information can be obtained from Federal, State, and local agencies concerning

the safe chemical materials. An inventory list of products to be used under this contract shall be provided

to JEA in a timely manner. This list shall be updated throughout the term of the contract. JEA will

contact the Contractor immediately if any item is deemed inappropriate for use under this contract.

12.4 Supplies and/or products to be used under this contract must contain recycled materials,

“environmentally preferable” products, and bio-based products to the maximum extent feasible. The list

of Comprehensive Procurement Guideline (CPG) items and their associated Recovered Materials

Advisory Notices (RMANs) are available at http://www.epa.gov/cpg. Information on CPG items can be

found at: http://www.epa.gov/cpg/products.htm. Products designated as bio-based must be included on

the United States Department of Agriculture (USDA) Bio-based Products List

(www.biobased.oce.usda.gov). JEA will accept supplies and/or products conforming to these

requirements.

12.5 The Contractor shall take all reasonable precautions, as directed by the JEA, or in the absence of such

direction, in accordance with sound industrial practices, to safeguard and protect JEA property.

13. ITEMS TO BE SUPPLIED BY JEA

13.1 JEA property (e.g. spare parts, etc.) shall remain the property of the JEA in all respects. JEA property

shall not be removed for use at other locations without the approval of the JEA Representative.

13.2 JEA property shall be used only in direct operations of providing contract services and shall not be used

in any manner for any personal advantage, business gain, or other personal endeavor by the Contractor or

the Contractor’s employees.

13.3 Space will be assigned to the Contractor for the storage of an inventory of his/her bulk supplies,

replacement parts, and the equipment which he will use in the performance of work under the contract.

The Contractor shall maintain this space in a neat and orderly condition. JEA will not be responsible in

any way for damage or loss to the Contractor’s stored supplies, materials, replacement parts, or

equipment.

13.4 JEA shall provide adequate telephone lines for the elevators to be monitored. Several elevators may

share the same telephone line. JEA shall also provide the automatic dialer apparatus and will be held

responsible for its working order.

14. REFERENCE MATERIALS DRAWINGS, WIRING DIAGRAMS, MANUALS

14.1 All drawings and wiring diagrams used by the Contractor for work under this contract are the property of

JEA and shall be accessible to and/or turned over to the JEA upon request. The Contractor will annotate

the wiring diagrams of all wiring changes performed or discovered.

15. STATE INSPECTION AND TESTING SERVICES

15.1 The Contractor shall be responsible for having required State Elevator inspections accomplished by

individuals certified and qualified in accordance with the requirements of the latest edition of ASME

QEI-1, Standard for the Qualification of Elevator Inspectors. The name of the inspector(s) and evidence

of certification will be submitted to the JEA Representative for approval prior to annual inspection. The

Contractor shall provide personnel who are familiar with the equipment to perform tests and assist the

State Certified Elevator Inspector. JEA will pay for the cost for the State inspection, certificate and

permit.

15.2 The Contractor shall schedule and have performed the required annual inspections and tests of all

elevators at the recommended intervals completed by January 31st of each year, unless otherwise agreed

upon. The Contractor must subcontract for the performance of the required State safety inspections with

individuals/firms independent of the Contractor’s organization. This cost for the independent firm will

be reimbursed by JEA (See Respondent Rates Workbook Bid Item #12). The Contractor shall furnish a

copy of the inspection schedule which shall clearly indicate the edition of ASME A17.1 & 2 being used

on this contract. JEA will be advised of any changes to the schedule and notified ten (10) working days

in advance of the inspection date to afford the JEA an opportunity to observe the inspection. The

required State annual inspection of the emergency signaling device (alarm) and the means of two-way

communications must be documented.

15.3 The Contractor shall provide a qualified employee to accompany the State Certified Elevator Inspector

during the Sate annual inspection.

15.4 The Contractor shall furnish a written report of each State annual inspection to JEA. The report shall be

submitted on the following checklists from the American Society of Mechanical Engineers (ASME):

Checklist for Inspection of Electric Elevators and shall include a written list of deficiencies. The

Contractor shall be responsible for the correction of all listed deficiencies within the scope of the contract

by the date specified by JEA and submitted on the appropriate QEI form and/or its supplements. Any

deficiencies marked “emergency” must be corrected immediately. When all listed deficiencies have been

corrected the Contractor shall sign and date the annual inspection report and return it to the JEA

Representative.

16. ADJUSTMENTS AND TESTS

16.1 The Contractor is responsible for the accomplishment of all tests required, at the intervals specified and

in accordance with, the ASME A17.1–2016 Safety Code for Elevators and Escalators manual. Copies of

all test results shall be furnished to JEA.

16.2 All rack & pinion elevators provided with firefighters’ emergency operation shall be subjected monthly

to Phase I recall by use of the key switch, and a minimum of one-floor operation on Phase II.

Deficiencies shall be corrected. The Contractor shall maintain a record of findings and make it available

to JEA, elevator inspection personnel, and the Authority Having Jurisdiction (AHJ). Such tests shall be

conducted such that they do not interfere with the normal operation of the building. A written inspection

test log shall be kept in each elevator machinery room.

16.3 Elevator Emergency Telephones must be maintained and tested. This testing shall be conducted monthly.

A written inspection test log shall be kept in each elevator machinery room.

16.4 Elevator Phones must be tested for the following functions:

16.4.1 Dialing function

16.4.2 Proper outgoing message identifying the elevator and building location

16.4.3 Volume and sound quality of the phone speaker

16.4.4 Quality of connection on both ends

16.4.5 Indicator light operation – flashes when connection is made

16.4.6 Ensure that hitting button a second time will not hang up the phone

16.4.7 Where provided, verify that line sharing equipment works properly with no crosstalk between

elevators, and that each elevator can be addressed individually.

NOTE: For all Alimak Rack & Pinion elevators, paragraphs 17 thru 19 below, contain the minimum, but not

necessarily the complete Preventive Maintenance (PM) Service requirements to be performed in the time

periods specified. In addition to the items listed below, the Contractor must complete all service

recommended by Alimak (i.e. if it is recommended by Alimak, the OEM, it shall be considered to be required

by JEA.). No additional charges for those services will be approved, as they are considered to be a

mandatory part of the monthly, semi-annual and annual requirements.

17. MONTHLY ALIMAK Rack & Pinion ELEVATOR PM SERVICE and INSPECTION ITEMS

17.1 Inspect all instruction and load signs to determine if they are installed and legible. Replace as needed.

17.2 Inspect and repair the roller assemblies and the counter rollers of the machinery plate, and car

mounting dampers and bolts.

17.3 Inspect the self-adjusting brake for proper spring loaded operation, note the air gap.

17.4 Check worm gears for oil leakage and refill with oil as needed. Report oil leakage on check-off sheet.

17.5 Clean the cooling flanges of the electric motor, machinery and the controller.

17.6 Check battery and charger and tone frequency control system, check transmitters and receivers. Note

battery reading on the check-off sheet.

17.7 Inspect safety hooks, check bolts.

17.8 Inspect the pinions and the rack for wear.

17.9 Check for wear on the worm wheel.

17.10 Check control voltage. Note meter reading on the check-off sheet.

17.11 Inspect auxiliary and main control panel. Perform visual inspection of components and inspect for

loose wires; repair as needed. Check main voltage; note meter reading on the check-off sheet.

17.12 Inspect the cable trolley, rollers and cable pulley. Make sure roller moves freely. Make adjustments as

needed.

17.13 Inspect car enclosure, vision panels, floor and top of car. Clean vision panels.

17.14 Check limit switch cams in mast, adjust as needed.

17.15 Check light, push buttons, emergency stop and alarm signal. Repair all defects found.

17.16 Inspect centrifugal brake and check for proper operation. Adjust if needed.

17.17 Inspect the hoistway.

17.18 Check alignment of mast.

17.19 Check car run-by clearance. Inspect hoistway enclosures, landing doors, mechanical/ electrical

interlocks, rap door interlocks and sills. Adjust as needed.

17.20 Check landing proximity switches or landing control system as equipped (car leveling device), vane

switches and adjust as needed.

17.21 Inspect power cable.

17.22 Inspect control cable.

17.23 Inspect cable guides. Make sure that the rubber blades are in place. Adjust if needed.

17.24 Check the position of the cable support in relation to the cable guides and adjust if necessary.

17.25 Inspect the anchors and/or the wall tie-ins.

17.26 Check the mast bolts.

17.27 Check car/counterweight buffers.

17.28 Check attachment of machinery plate.

17.29 Inspect the final limit switches. Check the three-phase switch, the normal and final limit cams and car

overload device, adjust as needed.

17.31 Inspect pit switch and pit, make sure that the pit is clean, clean or adjust as needed

17.32 Inspect safety device. Note safety expiration.

17.33 Lubricate the equipment according to manufacturer’s recommendations.

17.34 Re-torque bolts as required on car, landing enclosures, staircase and platform mast, tie-ins and cable

guides. Contractor shall map monthly re-torqueing of equipment.

17.35 Submit completed checklist to JEA Representative showing the completed items checked, what work

was repaired and completed, and how many hours were spent by the Contractor to perform the

monthly inspection.

18. SEMI-ANNUAL ALIMAK Rack & Pinion ELEVATOR PM SERVICE and INSPECTION ITEMS

18.1 Test heating units in motors, gears and electrical panels for proper operation. Record date and next due

date.

18.2 Clean or replace air filter on worm gear (Industry conditions, perform quarterly). Record date and next

due date.

18.3 Test electrical components with meter. Note date last tested. Record date and next due date. Note

reading points and readings.

18.4 Perform drop test. Record date and next due date.

18.5 Submit completed checklist to JEA Representative showing completed items checked and what work

was repaired and completed, and how many hours were spent by the Contractor to perform the semi-

annual inspection.

19. ANNUAL ALIMAK Rack & Pinion ELEVATOR PM SERVICE and INSPECTION ITEMS

19.1. Disassemble and inspect the coupling between the electric motor and the gear.

19.2. Check structure for possible corrosion damage.

19.3. Check motor overload protection.

19.4. Perform full load drop test.

19.5. Conduct a yearly no-load, low-speed test of car and counterweight safeties and a test of buffers and,

every fifth year, a rated load, rated speed safety test of governors and buffers, as required by ANSI

code 17.1 and 17.2, and Chapter 399 of the Florida Statutes.

19.6. Replace or rebuild all other components such as safety devices as required by the elevator

manufacturer.

19.7. Perform annual Safety Inspection with State of Florida Certified Elevator Inspector.

19.8. Submit completed checklist to JEA Representative showing completed items checked, what work was

repaired and completed, and how many hours were spent by the Contractor to perform the annual

inspection.

20. PAYMENT METHODS

20.1 Lump Sum – Fixed Rates for Scheduled Preventive Maintenance – Bid Items 1 thru 8

20.1.1 JEA will pay the Contractor a Lump Sum amount for scheduled preventive maintenance as set

forth below and the compensation provided shall constitute full payment for the work.

20.1.2 Scheduled rack & pinion elevator inspections shall be performed at a fixed rate for monthly,

semi-annual, and annual maintenance intervals. Compensation will be made to the Contractor

for each inspection that is performed per the bid costs indicated in the Respondent Rates

Workbook. In addition, the Contractor shall input, in the Respondent Rates Workbook, the

estimated # of man-hours required to perform each routine elevator inspection. Fixed rates

for Preventive Maintenance are applicable to Bid Items 1 through 8.

20.1.3 The rates of all elevator inspections shall remain fixed for the first three (3) years of the

contract. Thereafter, the Contractor may request a Consumer Price Index (CPI) adjustment

annually per Contract Terms & Conditions.

20.1.4 Maintenance and repairs determined to be over and above that for routine inspections will be

invoiced utilizing the T&M rates established in the Respondent Rates Workbook. Estimates

for ‘out of scope’ elevator maintenance and repairs shall be submitted in writing to the JEA

Representative and must include, at a minimum, the estimated labor hours and parts/material

costs associated with the Work.

20.1.5 Upon completion of the scheduled preventive maintenance, the Company shall submit an

invoice per the JEA Purchase Order instructions. At a minimum, all Lump Sum invoices shall

reference the JEA Purchase Order number, Plant Work Order (PWO) number, Bid Item #,

elevator description, Date of Service as well as Invoice #, Invoice Date, and copy of signed

Service Report.

20.1.6 Invoicing shall be submitted within sixty (60) days of task completion.

20.2 Time & Material (T&M) Payment Method

20.2.1 Payment

20.2.1.1 JEA will pay the Contractor for service calls using the T&M payment method as

set forth below and the compensation provided shall constitute full payment for the

work.

20.2.1.2 Pricing of all services calls shall be based on Hourly Labor, Parts/Materials, and

Subcontractor unit prices and markups submitted in the Respondent Rates

Workbook. T&M rates for maintenance and repairs are applicable to Bid Items 9

through 12.

20.2.1.3 The Hourly Labor Rates (Bid Item #9) shall remain fixed for the first three (3)

years of the contract. Thereafter, the Contractor may request a Consumer Price

Index (CPI) adjustment annually per Contract Terms & Conditions.

20.2.1.4 For Parts/Material purchases, the Company shall submit a quote from a third party

supplier for parts/material purchases to include part(s) cost, applicable sales tax,

freight and delivery charges as shown by original quotation. A markup amount

(Bid Item #10) shall be added to these costs, but shall not be added to applicable

sales tax, expedite charges, delivery or freight charges.

20.2.1.5 Budgetary estimates for rack & pinion elevator maintenance and repairs shall be

submitted in writing to the JEA Representative and must include, at a minimum,

the estimated labor hours and parts/material costs associated with the Work.

20.2.2 Invoicing

20.2.2.1 Within two (2) weeks of completion of the Work, the Contractor should submit a

preliminary invoice to the JEA Representative for approval. A preliminary invoice

can help to alleviate potential delays in final invoice approval and payment

delivery. The preliminary invoice shall contain, at a minimum, the following

backup documentation:

20.2.2.7 Invoice summary for each service call or scheduled inspection to include line item

expenses for labor, parts/materials, and subcontracts with totals for each. The

invoice summary should also include the name of the Contractor employee(s)

performing the Work, job classification, hours worked, dates worked, and hourly

labor rates. For an example of an invoice template, see Attachment A.

20.2.2.8 Receipts for Parts/Material purchases.

20.2.2.9 Receipts for Subcontractor costs.

20.2.2.10 Other backup documentation, as deemed necessary to verify accuracy of billing.

20.2.2.11 Upon approval by the JEA Representative, a final invoice shall be submitted per

the JEA Purchase Order instructions, using the invoice template provided in

Attachment A, or an approved equal.

20.2.2.12 Final invoicing shall be submitted within sixty (60) days of task completion.

20.2.3 Labor Classifications

20.2.3.1 Elevator Technician: must possess at least 4 years or more work experience in

elevator maintenance and repair services. The Elevator Technician shall possess a

State of Florida Elevator Technician Certification and have a minimum of three (3)

years of experience in the maintenance of Rack & Pinion elevators. Must be

familiar with all aspects of vertical transportation equipment, be able to diagnose

and correct problems, and follow appropriate industry standards concerning

maintenance and service procedures. Must have good working knowledge and

skills in the use of hand and electrical tools such as grinders, drill motors,

electrical testing equipment, hoisting and rigging equipment, chain tackles, etc.

Responsible for professional and courteous customer service, safe work practices,

and ensuring that maintenance and repair services are performed on a timely basis.

20.2.3.2 Elevator Technician Helper: 0-3 years of work experience in elevator maintenance

and repair services. Assists the Elevator Technician to service and repair freight or

passenger elevators, performing any combination of following duties: Assists in

moving and raising elevator components, using hoists, dollies, and rollers. Assists

in disassembly and assembly of elevator components, using technician’s hand

tools. Cleans equipment, using scrapers and cleaning compound, and lubricates

equipment with grease guns. Performs other duties as required under the direction

of the Elevator Technician.

20.2.4 Hourly Labor Rates – Bid Item #9

20.2.4.1 Elevator maintenance and repair services utilizing Time & Material payment

methods will be performed on a per-hour basis with a minimum charge of one (1)

hour. Hourly Labor Rates shall be provided in the Respondent Rates Workbook

and will begin when the worker arrives at the job site. There shall be no

compensation permitted for elevator technician travel to and from the NGS plant

site.

20.2.4.2 Hourly Labor Rates shall be all-inclusive such that each job classification shall

include wages, taxes, benefits, workers compensation, general & administrative

costs, all O&M expenses (including fuel) associated with an elevator technician

company vehicle as well as profit and overhead.

20.2.4.3 Hourly Labor Rates shall be quoted inclusive of all applicable training,

certifications, and approvals required to operate the subject vehicles and/or

equipment and to safely perform the Work at the NGS plant site in an

environmentally responsible manner.

20.2.4.4 Hourly Labor Rates shall be quoted inclusive of all basic Personal Protective

Equipment (PPE) and small tools & equipment. Basic PPE shall include, at a

minimum, safety glasses, hardhats, hearing protection, steel-toed boots, and

standard work gloves. Small hand tools & equipment shall include the kind and

type commonly used in the industry and shall include portable radios and cell

phones.

20.2.4.5 Hourly Labor Rates shall remain fixed during the first three (3) years of the

contract. Thereafter, the Contractor may request a Consumer Price Index (CPI)

adjustment annually per the Contract Terms & Conditions.

20.2.4.6 Contractor shall make arrangements to allow all work as defined in this

specification to be completed during Straight Time work hours.

20.2.4.7 Contractor will be paid at the “Hourly Labor Rate” indicated in the Respondent

Rates Workbook for all classifications of labor that are engaged in the Work.

20.2.4.8 The Overtime Hourly Labor Rate shall not exceed 1.5 times the Straight Time

Hourly Rate for work performed after normal work hours, including weekends and

holidays. Double Time will not be paid..

20.2.5 Parts/Materials Mark Up – Bid Item 10

20.2.5.1 For parts & materials purchased by the Contractor and used in the execution of the

Work, the Contractor shall be paid the actual cost of such parts & materials,

including sales taxes, if required, and freight and delivery charges as shown by

original receipted bills. A markup amount shall be added to these costs, but shall

not be added to applicable sales tax, expedite charges, delivery or freight charges.

The mark up amount shall equal the "Parts/Materials Mark Up" as stated in the

Respondent Rates Workbook. The Mark Up amount shall not exceed 20%.

20.2.5.2 The calculation for “Parts/Materials Mark Up” shall be expressed as follows:

Example: Cost of Parts/Materials = $2,000.00

“Parts/Materials Mark Up” = 10%

Total Parts/Materials Cost plus Mark Up = $2,000.00 x 1.10 = $2,200.00

20.2.5.3 JEA reserves the right to select and approve, or to reject the parts & materials to be

used and the sources of supply of any parts/materials furnished by the Contractor.

20.2.6 Subcontractor Services Mark Up – Bid Items 11 & 12

20.2.6.1 The Contractor will be permitted to utilize approved Subcontractors to assist with

the execution of the Work. JEA will pay the actual Subcontractor’s cost as shown

by copies of original receipted invoices. A markup amount shall be added to the

Subcontractor cost. The mark up amount shall equal the “Subcontractor Services

Mark Up” as stated in the Respondent Rates Workbook. The “Subcontractor

Services Mark Up” shall not exceed 25%.

20.2.6.2 The calculation for “Subcontractor Services Mark Up” shall be expressed as

follows:

Example: Cost of Subcontractor = $500.00

“Subcontractor Services Mark Up” = 20%

Total Subcontractor plus Mark Up = $500.00 x 1.20 = $600.00

20.2.6.3 In no instance shall the value of the Subcontractor’s work exceed that of the

Contractor, unless prior approval is obtained from the JEA Representative.

20.2.6.4 JEA reserves the right to select and approve, or to reject Subcontractors to be

utilized by the Contractor. Subcontractors must meet all of the same requirements

of the prime Contractor as stated in these Technical Specifications.

20.2.7 Administrative Costs

20.2.7.1 Administrative costs will not be permitted as a separate billable cost. These costs

must be included in the Hourly Labor Rates bid.

20.2.7.2 Time spent by the Contractor developing an estimate for a job will not be

permitted as a separate billable cost. These costs must be included in the Hourly

Labor Rates bid.

21. UNACCEPTABLE SERVICE, FINES, PENALTIES

21.1 In the event that any service performed under this specification is deemed unacceptable by either JEA

or any regulatory agency to which JEA is subject, for reasons which are reasonably under the control

of the Contractor, all fines and/or penalties assessed against the JEA by any regulatory agency which

are due to actions/inactions fully attributable to the Contractor shall be paid in full by the Contractor.

22.1 Contractor shall perform all work in accordance with established federal standards and regulations,

local codes and regulations, and the current issues of the following codes and regulations.

services. The evidence will consist of at least three (3) similar service contracts in

the United States in the last five (5) years date ending Response Due Date. A

similar service contract is defined as maintenance, repair, and testing of

commercial or industrial elevator services in the total amount of $100,000.00 per

year or greater. Two (2) of three contracts must include demonstrated experience

in the maintenance, repair, and testing of Alimak Rack and Pinion Elevators. No

two (2) service contracts submitted shall be from the same customer.
