APPENDIX E
GENERAL NOTE
In this Appendix, CONTRACTOR shall provide all information and documentation as
required by the Annexures.
Submission of all specified details as sought under these Annexures is mandatory and any
Tenderer not complying with this requirement (i.e. submissions with “to be advised later”,
“to be submitted on award” and the like) may be rejected.
These Annexures shall form part of the Appendix E - CONTRACTOR Resources, as well
as those submitted with CONTRACTOR's Tender.

ANNEXURE 2 – CONTRACTOR LEGAL REGISTRATION AND COMPLIANCE CERTIFICATES
Pursuant to IT.16 certified translated proof in the English language of all required registration (or exemptions as the case may be) with and/or from the appropriate relevant Qatar State Authorities must accompany copies of the original documentation in Arabic and appended as part of this Annexure 2 to this Appendix E.

ANNEXURE 3 – CONTRACTOR PERSONNEL A) CONTRACTOR REPRESENTATIVE
None of CONTRACTOR PERSONNEL listed here shall be removed or relocated
from the positions indicated or replaced without the prior written approval of QP.
Personnel removed shall be immediately replaced with new personnel who, in the
sole opinion of QP are of, at least, an equivalent standard as the personnel
removed. CONTRACTOR REPRESENTATIVE shall be a qualified Engineer with a
minimum of ten (10) years relevant experience. CVs shall be provided for all
nominated CONTRACTOR PERSONNEL. CVs shall identify the age, qualifications,
present position/length of tenure and previous experience of each nominee.

D) CONTRACTOR Key Personnel
This Annexure also includes CVs for all those persons of key positions required to
be filled under the CONTRACT.
All CONTRACTOR PERSONNEL must have the prior approval of QP before being
engaged in the WORK.
The period the personnel have been employed by CONTRACTOR shall be given.
This Annexure should include details of qualification, experience and CV of the
proposed Project Manager.
Specific mention shall be made in this Annexure of CONTRACTOR PERSONNEL
who shall be responsible for safety related matters.
Minimum Qualification Required for CONTRACTOR PERSONNEL
The following are the minimum qualification and experience required for the
CONTRACTOR PERSONNEL to be engaged in the SERVICES.
Project Engineer/Manager
Ten (10) years relevant experience out of which seven (07) years should be in
Project Management. B.Sc. (Engineering) qualification or equivalent.
Lead Discipline Engineers
Ten (10) years relevant discipline experience, out of which at least seven (07) years
should be in the respective discipline, or related to that discipline. B.Sc. qualification
or equal in an appropriate engineering subject. Past experience on similar contracts
is required.
Senior Discipline Engineers
Eight (08) years relevant discipline experience, out of which at least five (05) years
should be in the respective discipline, or related to that discipline. B.Sc. qualification
or equal in an appropriate engineering subject. Past experience on similar contracts
is required.

ANNEXURE 3 – CONTRACTOR PERSONNEL (CONTINUED)
Planning Engineer Minimum seven (07) years of experience in project planning, scheduling, coordinating, reporting and the like of similar nature of project with engineering qualification.
Senior Designers for all disciplines
Eight (08) years experience in relevant facility design including five (05) years CAD
experience should be in HND/B.Sc. (Engineering) qualification or equivalent.
Draughtsman
Five (05) years experience including two (02) years on CAD should be in
ONC/Draughtsman qualification or equivalent.

ANNEXURE 4 – FACILITIES, PLANT AND EQUIPMENT CONTRACTOR states below the details of its resources and facilities intended to be utilised for this CONTRACT. CONTRACTOR design office, design facilities, expediting and procurement agencies, fabrication yard (location and facilities) etc. are listed. CONTRACTOR states below the details of the main items of constructional plant and equipment, which it proposes to use for this CONTRACT. Details of all equipments etc. are specified. This list is the minimum that CONTRACTOR will be required to provide on WORKSITE. CONTRACTOR states whether the tool or equipment is already in CONTRACTOR ownership or will be purchased or hired. Details of any special tools should be highlighted.

The provision of this information shall not relieve CONTRACTOR from providing such information as QP may require in accordance with the General Conditions of Contract of the special equipment and tools CONTRACTOR proposes to supply or use, neither shall it in any way relieve CONTRACTOR of any of its obligations to carry out, and, complete the WORK in accordance with the CONTRACT.

ANNEXURE 5 – SUBCONTRACTORS
CONTRACTOR states below the names and addresses of all SUBCONTRACTORs from
whom it proposes to obtain work or services.
CONTRACTOR is not permitted to change any SUBCONTRACTOR(s) named in this list
without first obtaining QP written authority.
SUBCONTRACTORs are also required to submit details under each schedule of this
Appendix E along with details of relevant experience and shall be bound separately for
each SUBCONTRACTOR.
Acceptance of the Tender as a whole will not imply approval of any SUBCONTRACTOR
listed below. Full technical submissions will be required regarding all work and services
proposed to be subcontracted along with details of relevant experience as stipulated in
the General Conditions of Contract and the Specifications.
Full technical submissions are required for all services rendered under the CONTRACT
as required by the General Conditions of Contract and the Specifications.

ANNEXURE 6 – SUPPLIERS
CONTRACTOR states below the names and addresses of all suppliers and vendors
from whom it proposes to procure goods.
CONTRACTOR is not permitted to change any supplier named in this list without first
obtaining QP written authority.
Acceptance of the Tender as a whole will not imply approval of any supplier listed below.
Full technical submissions will be required for all materials to be supplied either by
CONTRACTOR or supplier, as stipulated in the General Conditions of Contract and
Specifications.
Full technical submissions are required for all goods procured under the CONTRACT as
required by the General Conditions of Contract and the Specifications.

ANNEXURE 7 – CONTRACTOR COMPANY ORGANISATION CHART
CONTRACTOR inserts a copy of its organisation chart which clearly shows the
Company structure with respect to Subsidiaries and Joint Venture Partners, if applicable.
The Organigram indicates positions of Company Directors, Managers, Partners and the
like.

ANNEXURE 8 – CONTRACTOR PROJECT ORGANISATION CHART
CONTRACTOR provides the proposed organisation chart for this CONTRACT (including
key CONTRACTOR and SUBCONTRACTOR personnel, where applicable, and the
person responsible for QA/QCand safety matters and the like) and indicate the line of
reporting to QP personnel. Inspection activities and expediting efforts shall be
segregated from each other with separate CONTRACTOR PERSONNEL. The CVs of all
the personnel identified under this Annexure shall be submitted under Annexure 1.

ANNEXURE 9 – QA/QC PROCEDURES
CONTRACTOR’s Quality Assurance and Control System dedicated for this CONTRACT is
to be provided in this Schedule.
CONTRACTOR Quality Assurance and Control System dedicated for this CONTRACT
shall conform to QP Corporate Standard for Quality Requirements for Projects (Doc. No.
QP-STD-004 Rev 1, a copy of which is appended at Appendix A).
This document will be reviewed by QP and any comments made shall be incorporated
by CONTRACTOR. Accepted document to be issued three (03) weeks after the
EFFECTIVE DATE of the CONTRACT. The quality system shall be formalised in a
Quality Manual which documents general policies, procedures and practices of the
organisation covering all quality related elements, addressing as a minimum
requirements of QP document QP-STD-004 Rev 1.

ANNEXURE 12 – CONTRACTOR’S COMPLIANCE STATEMENT CONTRACTOR confirms that it has fully compliled with the Instructions to Tenderer and has submitted a Tender in full compliance with the Tender Documents.
APPENDIX E
ANNEXURE 13 – HSE MANAGEMENT IN CONTRACT
HSE Management in contract shall be in conformace with QP HSE Regulations for
Contractors (QP-REG-S-001), a copy of which is attached at Appendix A.
CONTRACTOR shall provide along with their technical bid submission, a Preliminary HSE Management Plan for the CONTRACT, CONTRACTOR's past HSE performances and safety statistics/records for the last five (05) years as well as CONTRACTOR's HSE resources to be utilised during the execution of CONTRACT, including CVs of HSE key personnel.
