Relevant history:

Blood tests revealed **** Deletion syndrome at 2 months. Suffered from frequent bronchiolitus and chest infection He had a Full Team Assessment at York Hospital Child Development Centre at one year old. Referred to Speech & Language Therapist, occupational therapist, physiotherapist, orthoptist,

orthotics, audiology, Portage. Recent referral to respiratory department at Leeds General Infirmary. Oliver has been discharged from the orthoptist. He has Lycra shorts to support his muscles. He started Puddleduck Day Nursery and attends Mondays 8am – 1pm and Thursdays 8am –

6pm. Oliver had a broncoscopy at LGI (no scar tissue found). Under review with the dietician due to slow weight gain.

Additional information:

Oliver is prone to having bronchiolitus and chest infections which will probably result in hospital admission. Antibiotics given as soon as he has a cold.

He has large numbers of hospital appointments for health checks and therapies.

It would be helpful if these appointments could be co-ordinated.

physical needsOliver bum shuffles to reach objects and travel about his home. He can tolerate being in a 4 point crawl position and move his arms forward in this position. He can stand at furniture to play with minimal support and has a standing frame to help him achieve this milestone. He has taken a few cruising steps along the sofa. He enjoys spending time in his walking frame and manoeuvring himself around the house and outside.Oliver can hold objects that are easy to hold but can struggle with smaller items that require a more refined grasp such as crayons. Oliver has small hands so large objects are tricky.Oliver needs:Opportunities to explore his environment Activities to help him develop his fine motor skills as advised by his Occupational Therapist. Support for his physical development and help to follow his treatment plan as advised by his Physiotherapist which includes spending time in his standing frame and walking frame

N2 Communication and Interaction

Oliver’s understanding of language is ahead of his expressive language. He can understand and follow simple instructions e.g. give me a kiss etc. Oliver uses picture cards to help him make a more independent choice. He can make a choice from 2 picture cards. He is able to correctly identify familiar pictures from a choice of 2 and post the correct card in a post box.Makaton has been introduced and we are currently working on auditory bombardment of sounds.Oliver needs:Clear instructions using simple language and to be given visual clues through pictures and Makaton to support his understanding of instructions.Face to face modelling of sounds to help him imitate sounds.Support to follow his treatment plan as advised by his SALT

Oliver has a great explorative nature and loves to explore new toys and try and work out how they work. N4 Social and Emotional Oliver is wary of new people, both adults and children. He can become upset in new situations and when he is unsure what you are asking him to do. Oliver has settled well into nursery and has built up relationships with the staff. Since I started nursery only three months ago, my highlights have been making/forming close bonds with staff that take care of me. Especially **** my key worker. I have adapted to the nursery routine well with the support of my chair and standing frame. Both these sets of equipment have enabled me to sit with my friends for the full duration of a mealtime and also to take part in activities with my peers and on a one to one basis.

What have been the highlights in the last year?

Oliver’s highlights in the last year are that he has learned to bottom shuffle giving him more independence to explore his surroundings. He can also stand against a table and sofa and play unaided. Also his understanding has come on a lot.

He recently started nursery – which he loves.

What have been the highlights in the last year?

Oliver has made some lovely progress with his gross and fine motor skills. He can bum shuffle with speed and has taken a few independent steps to cruise the furniture. He can now hold a crayon and mark make effectively.What’s important in the next 3–5 years? Are the long term goals/outcomes still appropriate?

It is important that Oliver continues to receive 1:1 support to help him reach the outcomes set. When looking at school placement it would be beneficial for his parents to be supported on visits so that they can make an informed choice as to where would be the best place for Oliver.

Medical support should also continue.

What support is working well?

Oliver likes the Portage visits and although during the winter when he is unwell it can be tricky to get him to focus he normally gives everything a go.

Oliver has settled well at nursery and separates from his Mum really well. The EYIF provides opportunities for 1:1 time to work towards his outcomes.

Oliver needs time to further develop his gross and fine motor skills.
