INSTRUCTIONS TO FILL THE REGISTRATION FORM
1. Send your Registration Form along with Original deposit
slip.2. Attach your 2 recent photos of size (Passport Sizeonly)at
the front of this Registration Form.
Al l Candidates are required to prov ide their recent
photographs with fu ll face exposure.3. Deposit Rs: 650/- (Non
Refundable/ Non Transferable) in Designated Bank Branches of MCB,
UBL, HBL or
ABL,(Blank Deposit Slip is attached with this Registration Form)
and attach Original Deposit slip NTS Copywith your Registration
form.
4. Authent ication keyword is your desired 4 d igit code (for
example: 1234) required for correct ion (in case o f
ambiguity) in your information on the provisional candidates
list on NTS Website. Remember this keywordandkeep it with you
because it will be required to submit correction query in
provisional candidates list later.5. Tick only one test city fo r
your desired test center in para 2 of the registration form.6. Tick
your desired Test Type in Para 3 (see the chart below).

briefly against every Certificate/ Degree. Writedown percentage of
secured marks in 2 digits only, forms having any information other
than percentage willnot be processed.
Incomplete or improperly filled Registration Forms will not be
processed. Roll No. Slips andResult cardswill be sent through
CourierService. Give your valid and clear Postal Address
where delivery is certain.Fill theCorrespondence City District
for Result Card delivery.
