In 2012, our Care Consultants performed 2,336 home visits.

What is a Care Consultant? We

call our social workers “Care Consultants.”

Care Consultants are compassionate, state-

licensed professionals who perform the Council’s

primary work: to empower older people and their

families by offering customized care choices, so they can live safely in their own homes and stay healthy as they

age. In addition to regular telephone contact with our clients, our Care Consultants made 2,336 personal in-

home visits to older adults in Delaware County during 2012. Each Care Consultant provides personalized care to

approximately 110 clients at any given time.

* Clients participate in paying for their services based on a sliding fee scale. This helps ensure that local levy dollars are used based on our clients’ ability to pay and level of need.

Since 1995, we have served a cumulative total of 5,380 clients through our in-home care program.

2012 Service FactsIn 2012, we served our 5,000th client in our in-home program. And also:

Delivered 61,240 cans of nutritional supplements (Ensure and Glucerna)

Served 188,000 hot, nutritious meals through Meals On Wheels

Distributed 83,000 tons of free shelf-stable groceries and produce to low-income seniors

Answered 28,375 phone calls at our reception desk

Coordinated home chores for 787 older adults - chores like cleaning, painting, and mowing

Drove 191 seniors to medical appointments via local volunteers

Counseled and supported the needs of 250 family members caring for older adults

Gave farmers’ market vouchers for $44,118 in fresh fruit and vegetables

Provided opportunities for more than 250 seniors to receive free health screenings

Key 2012 Accomplishments

We were proud to be one of only 20 programs nationwide, and the only one in Ohio, to be awarded the highest level of accreditation available by the Meals On Wheels Association of America.

This year we expanded our meal offerings to include therapeutic selections for people on low-sodium or restricted-carbohydrate diets. We also added menu choices to further suit individual needs and tastes.

Our FIRST program began in 2012, a cooperative partnership with local fire departments which is designed to serve those seniors who frequently rely on rescue personnel for assistance with non-emergent issues.
