Applying for Financial Aid ChecklistComplete the Free Application for Federal Student Aid (FAFSA) online at www.fafsa.ed.gov using the Belmont College code 009941. You can begin applying for the 2019-20 academic year beginning October 1, 2018. Students are encouraged to complete the FAFSA by our priority deadline of April 1st for the upcoming academic year (or as early as possible) not only to allow for completion of the verifi cation process (if required), but also to ensure that you will receive all eligible fi nancial assistance available.

Review your Student Aid Report (SAR). Once you complete the FAFSA the Department of Education will send you, generally through e-mail, a summary of the information you reported on your FAFSA. This summary is called the SAR. Read your SAR carefully as it contains informational messages and possible errors which you will need to correct on www.fafsa.gov.

Complete any applicable scholarship applications by April 19, 2019 for full consideration. Applications received after this date will be reviewed in the order they are received, until all funding is awarded.

Submit to the Financial Aid Offi ce any missing documents. Check your fi nancial aid status online through your MyFAO site to see if you have outstanding requirements or important messages. Use the First Time User link to set up your account for the fi rst time. Don’t forget to use the MyFAO application on your IPad for even easier access!

Should you need a loan, be sure to complete a Direct Loan Request Form, which is available on MyFAO. In addition, complete Entrance Loan Counseling, and a Master Promissory Note at www.studentloans.gov. Students need to reapply each term for their student loan.

Make sure your bill is covered. If you receive a balance due, do not assume your fi nancial aid will cover it. There may be missing forms or may truly have a balance after all of your aid is applied to your account. Check your Financial Aid status on MyFAO or by email at fi [email protected].

Access your Account Statement through your MyBelmont web portal (also available as an application on your iPad). Also check your college email, as all offi cial college correspondence to you goes to your @belmontcollege.edu email account.

ScholarshipsBelmont College has a number of available scholarships awarded by the Financial Aid Office to incoming freshmen and/or transfer students. Additionally, some scholarships are awarded by various departments on the basis of students’ majors and/or special abilities. Any student enrolled in credit courses may apply for one or more scholarships. Please note that criteria and requirements for each scholarship may vary; apply only for the scholarships that you are eligible for. Some scholarships may require separate or additional applications. This Scholarship Guide can provide more detailed information regarding application details. Students who receive federal grants (such as the Federal Pell Grant) will have their grant money applied to tuition expenses first.

Completed applications must be returned to the Financial Aid Office no later than April 19, 2019 for priority consideration. Scholarship recipients will receive a letter confirming the scholarship awarded and the amount. Because earnings are contingent on investment performance and donations are at the discretion of the donor, Belmont College reserves the right to increase, decrease or eliminate the amounts available for individual private scholarships without notice. List of ScholarshipsClay Family Memorial Nursing ScholarshipA generous donation to the Belmont College Foundation was given by the estate of Dr. Mary Ellen Clay to establish a nursing scholarship endowment in memory of her parents Walter C. and Christena Stevens Clay. Income from this fund will be devoted to scholarships for students pursuing associate degrees or certificates in registered nursing or practical nursing. Students must complete the FAFSA as funds are awarded after available grants are applied. Preference is given to students from Harrison, Guernsey, Tuscarawas, and Belmont Counties in Ohio.

GPA: 2.75 Amount: One-Two - Up to $2,250 per semester for 4 consecutive semesters (summer optional) based on available funds to cover tuition, fees and required books after other grants are applied. (depending on funds) Major: Registered Nursing, Practical Nursing Application: General Application (see page 7 of this booklet) CONSOL Energy, Inc. ScholarshipDesigned to enhance student achievement for an associate degree in the area of engineering or industrial technology. Applicants must be enrolled in 6+ credit hours and complete the FAFSA. Rubloff ScholarshipAwarded each year to an Licensed Practical Nursing student and an Associate Degree Nursing student (nominated by nursing faculty) who exemplify the following performance criteria: • Demonstrates competent bedside nursing care • Applies theoretical concepts to clinical practice • Maintains professional attitude and behavior • Implements nurse-patient relationships This award is presented following completion of the first semester of study in the LPN program and following the first two semesters in the ADN program.

Employee ScholarshipThis scholarship is funded by Belmont College employees who donate to the Annual Employee Fund Drive. The goals of providing this institutional scholarship award are to encourage persistence within the higher education system in Belmont, Harrison, and Monroe Counties (Ohio) and to identify and support students who desire a college education and possess a proven record of academic excellence. To be eligible applicants must: • Must be enrolled at least half time (6+ hours) • Have successfully completed at least 30 Semester hours • Be a resident of Belmont, Harrison, or Monroe County • Complete the FAFSA GPA: 3.0 Amount: Two- $500 Major: All Degree Seeking Majors Application: General Application (see page 7 of this booklet)

Francis J. & Edith Jackson Family ScholarshipProvides financial assistance to students from Ohio County, West Virginia and Marshall County, West Virginia, in their quest for post-secondary education. This scholarship is designed to support the dreams of a student(s) who needs financial assistance; funds are not awarded simply based upon academic performance. Application: General Application (see page 7 of this booklet) Gust Maistros ScholarshipNursing faculty submit nominations based on the following criteria: This scholarship is presented following completion of the first two semesters of study in the Licensed Practical Nursing program, and after the first semester of the Licensed Practical Nurse to Registered Nurse program. Funds awarded must be used toward direct educational expenses. Applicants must: • Exhibit professional attitude and behavior • Submit an essay of at least 500 words (or more) on the topic of “The Importance of Bedside Nursing” GPA: 3.0 Amount: Two- $500 (1 for LPN, 1 for TAN) Major: Practical Nursing, Transitional Nursing Application: See a Nursing Faculty Member to apply

Hilda Burrows ScholarshipProvides access to educational services at Belmont College for qualified unemployed persons and/or their dependent(s). Applicants must: • Be unemployed due to layoff for at least 10 weeks (student, spouse or guardian/parent) • Provide written verification of layoff or plant closing (on company letterhead) • Provide verification of eligibility for unemployment compensation • Be a legal resident of the state of Ohio • Complete the FAFSA GPA: 2.5 Amount: Tuition, Fees and Books minus other forms of aid. Kimberly Taylor Memorial ScholarshipThis scholarship honors the memory of Kim Taylor, an ADN student in the class of 1988. Mining Electro-Mechanical Maintenance ScholarshipApplied directly to the recipients account to be used to help meet the direct educational expenses of tuition/fees and books/supplies. Extra consideration shall be given to those candidates who have expressed interest or are currently employed in mining or mining related engineering occupations, but the scholarships are not restricted to these persons.To be eligible applicants must: • Have successfully completed 30 Semester credit hours in the program major • Demonstrate financial need by completion the FAFSA GPA: 3.0 Amount: Two- $500 Major: Electronics Technology, Industrial Electronics, Civil Engineering, Welding Technology Application: General Application (see page 7 of this booklet)

Nita C. Lewis Pathway to Nursing ScholarshipThe Nita C. Lewis Pathway to Nursing Scholarship was established by Mr. J.I. Lewis in memory of his wife, Nita, who served as a paramedic instructor for Belmont College. Candidates must complete the FAFSA, reside in Belmont County, and be in pursuit of an associate degree in registered nursing or a certificate in practical nursing. Special consideration will be given to applicants who are non-traditional students and/or whose service as an EMT or as a Paramedic can be verified through the Ohio Department of Public Safety. In addition to the General Application, applicants must submit a short essay stating why they want to become a nurse. GPA: N/A Amount: The number and amount of the scholarship(s) are based on available funds. Senior Citizen Tuition WaiverSenior citizens (persons age 60 years or older) who are Ohio residents, are granted a tuition waiver when enrolled in a regularly scheduled credit course on a space available basis. Verification of age may be requested. A FAFSA is required annually from any student seeking a degree or a certificate. GPA: N/A Amount: Tuition only. Does not include fees, books or other expenses. Major: Any Major Application: See your academic advisor when registering to apply St. Clairsville American Legion Citizenship ScholarshipEstablished by the St. Clairsville American Legion Post #159, this scholarship will be awarded to a first-year student enrolled full-time in a degree or certificate program. Applicants must be able to show financial need and able to verify that they are the child or grandchild of a veteran of the United States Armed Forces, including the Army, Navy, Air Force, Marines, or Coast Guard. Veteran is further defined as an individual who has served a minimum of twenty-four (24) hours on active duty and is currently serving, retired, or otherwise holds an honorable discharge. In addition to the General Application, applicants must submit a short essay (no longer than one page) that addresses their level of need and any reasons that they should be selected. GPA: N/A Amount: $500 for the fall semester and $500 for the spring semester for a total of $1,000 for the academic year Major: All degree or certificate seeking majors Application: General Application (see page 7 of this booklet)

St. Clairsville Rotary ScholarshipProvides educational financial assistance to residents of the St. Clairsville/Richland Township school district. Applicants must: • Reside in the St. Clairsville/Richland Township school district • Be a high school graduate (or possess a GED) • Be enrolled full-time in a degree-seeking program • Be in need of financial assistance as determined by the Belmont College Financial Aid Office. • Attend one Rotary meeting to receive the scholarship monies • Complete the FAFSA

2019/2020 St. Clairsville Rotary Scholarship Application

The St. Clairsville Rotary Scholarship will provide financial assistance to a resident of the St. Clairsville/Richland Township District in pursuing their educational goals at Belmont College.

The Scholarship award valued at $500 is determined by the amount of interest available from the endowment fund and may be used to help meet the costs of tuition and/or direct educational expenses.

Trustees Scholarship GuidelinesComplete the following by the priority deadline of April 19, 2019. (Trustees Scholarship applications will be accepted on a first-come basis – should funding remain – until August 24, 2019). • Apply for Admission to Belmont College online at www.belmontcollege.edu. • File your 2019-20 FAFSA (Free Application for Federal Student Aid) online at www.fafsa.gov. Belmont College school code is 009941. • Submit this 2019 Trustees Scholarship Application to the Financial Aid Office.

Eligible candidates must also meet the following requirements: • Graduate from high school and forward final transcript to Belmont College Records Office. • Reside in and graduate from a high school in Belmont, Harrison, or Monroe County. • Enroll at Belmont College in the summer or fall term immediately following your high school graduation. • Enroll in the Student Learning and Success course (FYE 1110).

Important Information: • The Trustees Scholarship will cover tuition expenses only. Students who receive federal grants (such as the Federal Pell Grant) will have their grant money applied to tuition expenses first. If grant funding meets or exceeds the cost of tuition, student’s Trustees Scholarship will not be utilized. Any remaining federal grant funds can be used to cover fees and book expenses. If a grant award total is less than the student’s tuition expense, the Trustees Scholarship will cover the balance of tuition, but will not exceed the cost of tuition. After grants and Trustees Scholarship funds are applied to the student’s account, they may still have a bill for fees and books. • Trustees Scholarship recipients are selected by the Financial Aid Office. • The Trustees Scholarship does not cover fees, books, or other expenses. • The Trustees Scholarship is available for six consecutive terms and students must enroll in the summer or fall term immediately following their high school graduation. • Trustees Scholarship recipients must maintain a semester GPA of 2.5 and must be enrolled full-time.

Financial Aid Frequently Asked QuestionsWhat is Financial Aid?Financial Aid includes all sources of funding a student is eligible to receive that helps to pay for college expenses. These resources may include Grants, Scholarships, Loans, Federal Work Study, and outside agency support. Grants may be the Federal PELL Grant or the Supplemental Educational Opportunity Grant (SEOG). Loans include the Federal Direct Subsidized and Unsubsidized Loans. Also, parents may be eligible for the Parent PLUS Loan. Scholarships may be awarded to you from Belmont College or from other agencies or companies. You are required to notify the Financial Aid Office if you receive any funds from outside agencies. What is “MyFAO?” “MyFAO” stands for My Financial Aid Office. You can access MyFAO on the Belmont College web page or your iPad application to check the status of your financial aid award, check for missing documents, download forms, check the status of your loans and more. You will need your Student ID number and your MyFAO PIN to access the site. If you do not know your PIN, go to the link provided on the MyFAO site, put in your Student ID and select “First Time User” to create a PIN. If you forget your PIN, select “Forgot Your PIN”. Your MyFAO PIN is not the same as the FSA ID you used to file your FAFSA or your MyBELMONT password.

What is Verification?The US Department of Education may select your FAFSA for verification of the information you reported. Belmont College is required to verify all applications that have been selected. The college also may require verification of any application that may be in question. In these cases, specific documentation will be required. The US Department of Education and/or the Financial Aid Office will notify, in writing, all financial aid applicants for whom verification is required. We will require a verification worksheet and certain tax documents that must be requested directly from the IRS. Specific instructions will be provided to you if you have been selected for the verification review.

What is a Special Circumstance?If a student’s projected income for the current year is less than the income reported on the FAFSA, then the student should check with Financial Aid to see if he/she is eligible for a special circumstances review. Special Circumstances include: loss of employment due to lay-off, termination or reduction; loss of earning due to disability or natural disaster; loss of an untaxed income or benefit; legal separation or divorce; death of a wage earner; or unusual medical or dental expenses.
