DEAR FRIENDS,
I take this opportunity to welcome each of you to the LeadingAge Michigan 2019 Annual Conference and Solutions Expo. We are gathering in Kalamazoo to share ideas, learn promising practices, network with peers, and take a deserved break from the intensity of our daily work.
Mission-based long term services and supports (LTSS) lie at the heart of senior care here in Michigan. Our members demonstrate that truth every day through their performance, dedication, diligence, and endurance as health care and especially long term and post-acute care slowly emerge from the political and regulatory chaos that surrounds us.
We have yet to see the end of the transformational change that is engulfing the industry. For over ten years, we have worked to inform and prepare members to face unprecedented challenges in our work. We continue that mission.
For years, the LeadingAge Michigan Annual Conference and Solutions Expo has been the forum for LTSS innovation and training. We believe that what makes this event so special is the fellowship, shared values, and experiences we hold together. It is our collective spirit that supports our strength and holds us to our mission.

BRIAN ELLSWORTHVice President, Public Policy and Payment TransformationHealth Dimensions Group
Brian leads efforts to develop value-based payment approaches for post-acute and home-and community-based care providers, including value-based strategic positioning, bundled payment opportunities analysis and continuing care network development. Brian has more than 30 years of experience in health care financing, delivery and policy from both payor and provider perspectives.
TIM RICHARDSONNational Speaker & Author
Tim has been a full time professional speaker since 1988. Tim earned the designation of Certified Speaking Professional (CSP) given by the National Speakers Association in 1997. He has also been the host of a radio talk show interviewing authors, business owners, and community leaders. Tim is author of Jump Starts: Wit and Wisdom to Super Charge Your Day, co-author of Transformation Thinking: Tools and Techniques That Open the Door to Powerful New Thinking and contributing author to Meditations for the Road. His latest book he is writing is titled How to Live Rich in a World Consumed with Cash, Career, and Conquest. He has been interviewed on regional and national TV and radio programs and writes articles for national publications.

Jack did not originally envision iN2L as a business; the impetus for what became the company was a philanthropic idea—to donate computers to assisted living communities and nursing homes in Southern California. In 1999, Jack retired as vice president of strategic sales for Vishay Intertechnology and started what has become a successful gerontechnology company. As of 2015, the company has a customer base of over 2000 communities across all 50 states. He is a sought after national and international speaker on technology as a means to create personalized experiences that engage and connect residents to their loved ones and the world at large, specifically individuals with dementia. iN2L’s work has been recognized by the Wall Street Journal, NPR, and dozens of senior living publications.

FULL CONFERENCEIncludes admission to all conference events and access to online handouts. Continental breakfasts and lunch are provided on Monday and Tuesday. Admission to the Solutions Expo on Tuesday. One ticket to Tuesday’s Member Awards Gala is also included.
MONDAY OR TUESDAY ONLYIncludes admission to education sessions on Monday or Tuesday, admission to the Solutions Expo on Tuesday and online access to handouts. A continental breakfast and lunch are provided each day. Tuesday-only registration does not include the Member Awards Gala. Tickets for the Member Awards Gala can be purchased separately.
WEDNESDAY ONLYIncludes admission to education sessions on Wednesday and access to online handouts. A buffet breakfast and morning refreshment break will be provided.
SINGLE-SITE SAVER IThis rate allows facilities and CCRCs from a SINGLE physical address/location to send up to 10 individuals each day. Continental breakfast and lunch each day are included for each participant.

NOTE: If you choose a Single-Site or Multi-Site Saver registration option, a form will be sent to you to provide names for your staff members. All individuals registered through Single or Multi Site options will receive access to the online handouts. Tickets to Tuesday’s Member Awards Gala are not included. Tickets for the gala can be included on your Single or Multi Site Saver form.

REGISTRATION DETAILS
REGISTRATION CONFIRMATIONSConfirmations will automatically be sent to the email provided during the online registration process. Additionally, a final confirmation email will be sent to all registrants 3-5 days before the conference and will include a link to access online handouts.
SPECIAL DIETARY/ACCESSIBILITY NEEDSParticipants with special dietary or accessibility needs should notify the Association by email at [email protected] or by calling us at (517) 323-3687. Please let us know of any special needs in advance so that proper arrangements can be made.
NO SOLICITINGExcept for registered exhibitors, all participants are asked to refrain from marketing products or services during the conference.
PAYMENTAll registration fees should be paid at the time of the online registration. Members receive the benefit of selecting “bill me” during our online registration checkout.
UNIQUE EMAILSBe sure to provide a unique email address for each person attending. Registrants will receive important conference information and updates via email only. If a registrant’s email address is not provided, they will not receive their certificate of attendance and verification of their continuing education contact hours.
SUBSTITUTIONS, CANCELLATIONS & NO-SHOWSSubstitutions and cancellations should be submitted, prior to the conference, in writing to [email protected] and must include the reason for cancellation..Cancellation & Grievance Policy
PARKINGA reduced self-parking rate of $10 per day in the attached, covered, parking garage. Valet parking is available for $25 per day. Both parking options are offered to overnight guests and day-only attendees. Parking fees are not included in any of the conference registration fees.
PHOTOGRAPHY AND VIDEO RELEASE STATEMENTSessions or activities at the conference may be photographed or video-recorded. Attendance at the conference constitutes the consent of all participants to the future broadcast, publication or other use of photographs or videos at the sole discretion of LeadingAge Michigan.

Evaluation & Certificate of AttendanceAn electronic evaluation will be emailed to all conference attendees approximately two weeks post event. Once the evaluation has been completed, attendees will receive their Certificate of Attendance via their unique email provided during the conference registration process. Certificates will also be available by accessing your profile in the LeadingAge Michigan website.

MODERATORS
Are you interested in taking on an active insider’s role at the conference but not sure what you might be able to help with? Now you can make the most of your conference experience by volunteering to moderate one or more of the education sessions. As a moderator, you will be the liaison between the association and a session to ensure the program runs smoothly and the presenters have everything they need.
Specifically, session moderators will:• Greet the speaker(s)
• Scan name badges
• Distribute handouts (if speaker has hardcopy materials for the session)
• Give opening announcements
• Introduce speakers
• Complete a quick session evaluation
No experience is required! We will provide a Moderator Packet with easy instructions. Moderator information will be available for you to pick up at the onsite registration area.
