SUBMISSIONS: • Nominations are completed online at www.pahowhofcu.org through a link to Survey Monkey • Nominee must be over 18 years old and work, volunteer or have retired from a Global Health Organization • Anyone can nominate a fellow employee or themselves through a form on the website • Entries must be submitted May 2019 – November 2019 2. JUDGING: Management of PAHO/WHO FCU will select one winner each month. The winner will be notified by email or by phone. The recipient will receive a$700 cash prize and trophy in exchange for a photo release. A $1,000 donation to the Global Health charity of the awardee’s choice will also be made 3. AWARDS: Winners agree to the use of their names, voices, photos and / or likeness for the purpose of advertising, social media posting, trade or promotion without further compensation Winners will be notified by email or phone, whichever is applicable. Should the email be returned or phone number is invalid two more attempts will be made via email and one by regular mail if applicable. Winners must contact PAHO/WHO FCU within 5 calendar days from the date the notification is sent to claim their prize. Failure to contact PAHO/WHO FCU within that 5 day period will result in immediate disqualification of the selected entrant and a new winner will be selected. PAHO/WHO FCU is not responsible for and shall not be liable for late, lost, misdirected, or unsuccessful efforts to notify winners. 4. OTHER CRITERIA: • Employees, officers, or directors of PAHO WHO Federal Credit Union, its affiliates, subsidiaries, and their children, parents, spouse, and members of their household are ineligible to participate. • PAHO/WHO FCU reserves the right in its sole discretion, to cancel the contest in its entirety. • The charity must be a non-profit, 501(c)(3) or the international equivalent and have a mission that serves Global Health. PAHO/WHO FCU management must approve the charity. The check will be made payable directly to the organization. • All federal, state and local laws and regulations apply. • PAHO/WHO FCU, is not responsible for any taxes that winners may be subject to as a result of cash prize • PAHO/WHO FCU, its agents and representatives, its affiliates, subsidiaries, advertising, promotion and fulfillment agencies and legal advisors are not responsible for and will not be liable for (I) late, lost, damaged, misdirected, incomplete, OR unintelligible entries; (II) telephone, electronic, hardware or software program, network, Internet or computer malfunctions, failures or difficulties of any kind; (III) failed, incomplete, garbled or delayed computer transmissions; (IV) any condition caused by events beyond the control of PAHO/WHO FCU that may cause entry to be disrupted or corrupted; (V) any injuries losses or damages of any kind arising in connection with or as a result of, or from participation in the Globies; or (VI) any printing or typographical error in any material associated with the Globies. ¬

1.
