When I went to Haiti with a medical team

two months after the earth-quake in 2010, I expected to treat patients never to see them again. A burn patient came to our clinic to have her dressings changed, and I thought it was amazing that I could provide continuity of care even during my short stay.

She was in her 40s and happened to be cook-ing over a large pot of hot oil when the earthquake struck. She fell in and suffered extensive third-degree burns to her chest and breasts. She was hos-pitalized for a short period and then received contin-ued outpatient care. A few days before I saw her, she had received care at anoth-er clinic. They used gauze to dress her burns, which is not the preferred dress-ing because it sticks to the wounds and pulls off the new tissue that is being formed. I am sure it was the only supply the other clinic had available.

Our cl inic was in a church, and we had a lim-ited amount of donated supplies. I was able to give her some pain medication

before starting her treat-ment. My heart ached for her as I removed her gauze. I couldn’t believe she didn’t scream from the pain; she just firmly held my hand for support and took deep breaths. I applied a generous layer of burn cream and some non-stick dressings and an Ace bandage. I saw her every two days during my 10 days there.

There was story after story of heartache and devastation. I went back in

December, and I do plan to go again. The Haitian people are so genuine and thankful — very, very endearing. Anytime you see different cultures and values, it changes you as a person and as a pro-fessional. It completely changed my perspective on medic ine , hav ing gone into a place where they have nothing. It not only makes me thankful for what we have here, it makes me want to give more of myself.

Uterine fibroids are a quite common condi-

tion, but seldom do they weigh 5 pounds.

A few years ago, a 46-year-old patient was referred to me with mul-tiple large fibroids, which enlarged her uterus to the size of a seven months’ pregnancy. The patient was feeling pressure and some pain, and by the time she saw a doctor, her right ureter was compressed causing her kidney to enlarge.

An ultrasound showed that her uterus measured 15 inches. Normally, a woman’s uterus mea-sures about 3-by-2-by-2 inches. Many fibroids of various sizes were pres-ent that contributed to the overall size of her uterus. I offered the patient lap-aroscopic surgery with the use of four incisions of 1/2 inch each. If traditional open surgery had been performed, it would have required an incision of at least 12-to-15 inches into the abdomen. This tradi-tional open surgery would have also meant a hospi-tal stay of about a week, and four to six weeks of recovery.

Due to her age, a lap-aroscopic hysterectomy was performed, which included the removal of all the fibroids. The small incisions were made and various specialized instru-ments were used to per-form the operation. After the hysterectomy, remov-ing the large fibroids was a challenge, requiring a spe-cial surgical instrument that extracts the fibroid tissue in smaller pieces. This part of

the procedure was tedious and prolonged because of the need to reduce the tis-sue size before it could be removed from the abdo-men. The surgery lasted more than four hours. As a result of the laparoscop-ic surgery vs. traditional open surgery, the patient was able to recover more quickly and go home from the hospital in just two days.

The fibroids weighed a lmos t 5 pound s —

probably among the larg-est to be removed laparo-scopically in the United States. Fibroids rarely are cancerous, and fortunately hers were not.

At her six-month check-up the patient said she was feeling great, was able to exercise regularly, had no more pressure symptoms, and did not experience any pain from sex compared to before the operation. Her right kidney had also fully recovered.

When someone suf-fers a cardiac arrest,

prompt response to emer-gency treatment is criti-cal to their survival and recovery. On one occa-sion, I was called in by the emergency department regarding a 55-year-old gentleman who had come in with chest pain. He was initially alert and able to provide some medical his-tory. He had a very weak heart — only about 10 per-cent heart muscle function as a result of severe block-ages and was felt not a can-didate for heart surgery. Suddenly, his heart slowed down and he stopped breathing. The monitor showed the heart’s electri-cal activity was still there, but he had no pulse. CPR

and resuscitative measures were promptly started.

The longer you work on a patient, the probability of a successful resuscitation decreases dramatically. In this instance, the emergen-cy staff and I persisted for almost an hour and when we were about to give up hope, we spoke to his fam-ily. I said, “Once we stop, he will pass very quickly because we are artificially keeping him alive with CPR.”

We had almost stopped when the patient started showing signs of recov-ery, opened his eyes and started waking up. It kind of stunned everybody. We were very thrilled to see him return to conscious-ness, and by the next

morning he made full neu-rological recovery.

Within the next two weeks, we treated him with what we call a “hybrid approach.” One of our sur-geons performed a bypass on the front wall of the heart, and two days later I went in and did an angio-plasty on another major blockage on the back of the heart. A year later, he celebrated the anniversa-ry and invited all of us to his party.

This was a unique suc-cess story. Now, I feel that most patients in this situation deserve a longer resuscitative effort. You never know if and when someone will turn around.

In my specialty, pioneer-ing new operations is

particularly challenging because there is no physi-cal room for error in a baby. This was exempli-fied when I performed a complex opera t ion through tiny “minimally invasive” incisions on a boy who was born four weeks prematurely. Parker was born with esopha-geal atresia, a condition in which the esophagus does not reach the stomach. The standard method for repairing this condition is to make a large incision. I have done similar surgery in adult patients using the minimally invasively technique. However,

it’s totally different in a 4-pound child; the size and space and visualization are so much trickier.

Only a handful of peo-ple in the world had done this operation minimally invasively and never on a child this small. I explained this to his parents and they trusted me to do this for the first time on their son. For me, as a parent and phy-sician, this is always very humbling.

From birth, Parker had been unable to eat or even tolerate his own saliva. When he was 2 days old, we put a feeding tube in his stomach. The surgery was done when he was 56 days old. We moved his

stomach up into his chest to restore normal anatomy. Three weeks later, Parker was home, and another five weeks after that he was getting all of his nutri-tion by mouth. It was a great outcome and he con-tinues to do well.

There are 56,000 prac-ticing general surgeons in the U.S., but only 1,100 pediatr ic surgeons — roughly 13 in Wisconsin. We are trained in adult general surgery with addi-tional training in pediat-rics. Most of us choose this field because we love kids. Having a son only a year older than Parker, I often think, “This could be my child, my family.”

Everyone who has open heart surgery is a little

fearful. For one patient, it was her third one in less than a year.

This 73-year old wom-an came to me from up north. She had suffered a heart attack eight months earlier and needed emer-gency surgery at that time to close a rupture of the heart wall resulting from the heart attack. The repair didn’t hold though, and six weeks later she needed more surgery to repair a contained rupture (pseudo-aneurysm) that developed at the same site.

Five months later, pain in her left rib cage sent her back to her primary care physician. A recur-rent pseudo-aneurysm, with a cavity nearly the size of her entire heart, had formed at the previ-ous site and blood from the heart was going in and out of it. Over time, the pseudo-aneurysm expanded and was forc-ing its way between her eighth and ninth ribs. You could see the pulsation with the naked eye, and you certainly could feel it. It was going to require yet

another operation.We opened her chest

down the middle again, put her on a heart-lung machine, removed the giant pseudo-aneurysm, and closed the hole in the heart’s left ventricle that had been communicating with the pseudo-aneu-rysm. The patch used to close the hole was about the size of a quarter, made from a commercially avail-able product that comes from the sac that holds a

cow’s heart. It’s about the consistency of a yellow rubber glove — nice and pliable. She was in the hospital about 10 days. She’s done fine since.

Before you go into any case like this, you’re thinking, “What are the potential trouble points?” Luckily, we didn’t have any. I imagine I’ll see another case like this someday, and after this one, I’m confident I’ll be prepared.

I haven’t personally talk-ed to many soldiers who

have been in hot spots, but I had one patient who had been in Iraq twice and at Guantanamo Bay. He’s a Catholic priest, a Navy “lif-er” who got progressively sicker for about four years with multiple myeloma without knowing he even had it. Here’s a guy who stuck with his unit and did pretty much exactly what he was supposed to do.

Finally, med evac had to get him out of Fallujah in 2008. If he’d gone to an Iraqi hospital and it became known he was an American, a Christian and a priest, he’d have been a target for kidnapping. He told me, “I figured I’d take my chances with my illness

rather than being abducted and beheaded.” That kind of stuff really ups the ante when you’re talking about what might happen with treatment.

His diagnosis was a long, long process at several hospitals in Germany and Maryland, and by the time he came to me, he was a 56-year-old whose kidneys were shot. We had to time his therapy carefully so that his treatments didn’t con-flict. Chemotherapy drugs destroy the normal bone marrow, for instance, so first we harvested enough stem cells from his bone marrow to be re-intro-duced later into his blood stream. All the while, he needed dialysis three times a week — and it had to be

from 8 p.m. to midnight at one point so the dialysis wouldn’t remove the stem cells from his system. He did very, very well.

He is living in a monas-tery in Wisconsin and that’s where he intends to spend the rest of his life. He’s remained in remission and just went on a list for a kid-ney transplant.

It was a challenge to get him to open up and tell us how he felt, because he had steeled himself to deal with adversity. Our rela-tionship was very focused on getting him better. I think he felt he was OK in the God department, so we could both concentrate on his medical treatment. It made for a very successful partnership.
