• NORMALE dans les 1ères h en cas d’IC mais : • AH = hyperdensité spontanée !• Signes d’IC précoces :
• Hypodensité systématisée au territoire artériel
• Effacement noyau lenticulaire • ’’ ruban insulaire • Dédifférenciation SB/SG • Effacement sillons corticaux • "Trop belle" artère = thrombus
IRM = examen de référence
• IC :
• FLAIR = hypersignal
• Diffusion = de manière précoce
• Angio-IRM du Willis = artère occluse
• Perfusion = pénombre
• AH :
• T2 * = hématome
Bilan étiologiqueAVC hémorragique
• AUCUN si profonde chez sujet hypertendu !• IRM si normotendu
• Malf, tumeur… !• ARM/IRM si lobaire :
• Malf, thrombose veineuse, amylose…
AVC ischémique
• Glycémie : diag diff !!! • ECG : troubles du rythme, SCA • ETT : cardiopathie emboligène • Echo TSA/trans-crâniennes • NFS, hémostase • IRM cérébrale à 48h • Bilan FDR CV !• +/- Holter-ECG, bilan thrombophilie,
marqueurs tumoraux, ETO

• Paracétamol si T > 37,5°C • HTIC :
• Tête à 30° • +/- mannitol
• Équilibre HE !• RESPECT POUSSÉE TENSIONNELLE
réactionnelle, sauf chiffres très élevés : • AIC : HTA < 220/120 mmHg • AH : HTA < 185/110 mmHg
Ttt antithrombotique si AIC
• THROMBOLYSE IV : • Indications :
• Délai < 4h30 • Absence d'hémorragie • Score NIHSS 5 - 22 • Non étendu (sylvien superficiel) • TA < 185/110 • Pas d'anticoagulants • Pas de convulsions • Pas d'AVC ou trauma récent • Glycémie normale • < 80 ans
• En USINV : Rt-PA (Actilyse) !!• ANTIAGRÉGANT PLAQUETTAIRE :
• ASPIRINE IVL • 300mg puis 75mg/j • Après 24h SI THROMBOLYSE
• Plavix si CI ou polyathéromateux !!• ANTICOAGULATION efficace :
• En URGENCE : dissection cervicale, TVC • Si embolie : entre J5 et J30 • CI si infarctus massif (sauf TVC !!)
URGENCE • APPEL 15/ Transfert SAMU • ALERTE THROMBOLYSE si < 4h30 • HOSPITALISATION en USINV • MEC :
• Repos au lit strict allongé, à jeun • VAS, VVP, scope, SNG
PEC à distanceAutres !
• Kiné • Orthophonie, ergothérapie • Soutien psy !• 100% ALD + AAH +/- APA • Reclassement professionnel, adaptation poste,
taux d’invalidité !• Mesure de protection :
• Sauvegarde de justice • Curatelle/tutelle !
• SURVEILLANCE tous les 6 mois puis tous les ans
Prévention 2ndR !• FDR CV :
• AntiHTA pour TA < 130/80 • Statine pour LDL < 0,75 g/L • Equilibre diabète • Arrêt tabac • RHD
• Antiagrégant plaquettaire À VIE : • Aspirine 75 mg • +/- plavix !
• Ttt chir : endartériectomie carotidienne • Si sténose symptomatique
homolatérale > 50%
Surveillance • Cstes vitales • Glycémies cap • Échelle NIHSS • Troubles déglutition • Conscience • Complications décubitus • Imagerie de contrôle à 24h !!

THROMBOPHLÉBITE CÉRÉBRALE3 signes
• CÉPHALÉES aiguës • HTIC : seule dans 25% des cas • CRISES CONVULSIVES :
• Hémicorporelles à bascule !• DÉFICITS NEURO FOCAUX :
• À BASCULE = sinus longitudinal sup • Ophtalmoplégie = sinus caverneux
IRM en URGENCE +++
• HYPERSIGNAL T1 et T2 du sinus = thrombus ! • + angio-IRM veineuse => absence de flux
Diagnostic positifTDM
• SANS inj : hyperdensité du sinus
• AVEC inj : PDC paroi => signe du delta
PEC • HOSPIT en URGENCE en USINV • MEC : repos au lit strict, à jeun, VVP, scope !• Étiologique :
• Eradication FOYER INFECTIEUX • Arrêt CONTRACEPTION ORALE !!! !
• Symptomatique : • ANTIÉPILEPTIQUE si crise • Anti-oedémateux • Kiné !
• Anticoagulant en URGENCE (même si transformation hémorragique ++++) :
• HÉPARINOTHÉRAPIE curative : • HBPM : Lovenox 100UI/kg en 2 SC/j
• Relais AVK 6 mois au moins

• Favorable après ttt AC • Souvent défavorable si profonde
FDR
• Généraux : FDR de MTEV • ATCD de MTEV • K • Grossesse, POP • Thrombophilie • Chir récente
• Locaux : • Inf cérébrale • Tumeur cérébrale

• Plaquettes 2x/sem !• IRM + angio-IRM veineuse :
• De contrôle • À 6 mois
DISSECTION CERVICALE3 signes
• SIGNES LOCAUX : dus à hématome • Carotide :
• CBH homolatéral • Vertébrale :
• Cervicalgies ou céphalées post !• Signes cliniques d’AVC ischémique
FDR
• Trauma cervical !• Hyper-extension prolongée !• Elastopathie congénitale sous-jacente :
• Marfan • Ehler-Danlos
IRM cervicale en URGENCE +++ • ANGIO-IRM des TSA en T1 + FAT-SAT !• Dissection artère :
• Calibre externe augmenté • Hypersignal en croissant paroi • Sténose artérielle !!
• IRM cérébrale
