PreCode ReportsCreated to give you advance notice of the students who are precoded for the various assessments. These reports are an opportunity to identify students who need to be excused from a test, moved to another test or have a hand-gridded form prepared. The reports can also be used for Routing School students and PreGED/Skills students to determine the school where there results should be sent back to for accountability.

Coding on Answer DocumentsThe information your schools provide on the answer documents can have significant affects on their SPS. Especially important is the coding of accountability data, both accountability codes and routing codes (for students at alternative schools). It is also important to properly code student data (id, name, DOB and gender) when hand gridding an answer document. These data are used in matching the student’s test to SIS enrollment.

Accountability CodesACCOUNTABILITY DATA CODES: Codes for Students Who Are Not Enrolled (Have Exited) before or during Testing If the student exits (withdraws) from school before or during testing, use one of the following SIS Exit Reason Codes (01–

03, 05, 07–16, 21-26, and 97, applicable for testing) in the first two columns of Section V (LEAP 21/GEE 21) or Section I (ITBS/ITED), or Section J (LAA), starting at the left: 01 – Expelled (out of school) * 02 – Dropped Out * 03 – Illness: Student intends to return to school (long-term illness; student withdraws from school). Use Code 03 if the

student has been exited from school due to extended illness and plans to return. This illness is NOT a short-term illness that requires a doctor’s letter. *

05 – GED Only * 07 – Death (of student) * 08 – Transferred to another public, charter, or laboratory school within district * 09 – Transferred to another public, charter, or laboratory school within Louisiana * 10 – Transferred out of state or country * 11 – Transferred to Adult Education * 12 – Transferred to correctional institution * 13 – Transferred to Vocational Technical School * 14 – Transferred to nonpublic school * 15 – Exit from grade for reassignment to another grade (changed grade of student). Use Code 15 if the school changed

the grade of the student after October 1, 2002. * 16 – Transferred to home study/in-home private schooling * 21 – Transferred to State School * 22 – Options Program Completer: GED & Industry Based Certificate * 23 - Options Program Completer: GED & Locally Designed Skills Certificate * 24 - Options Program Completer: Industry Based Certification * 25 - Options Program Completer: Local Skills Certificate Only * 26 - Options Program Completer: Certificate of Completion * 97 – Unknown * * To use these codes, the student must be exited from the DOE Student Information System (SIS) with the same exit code. Do not use any of these codes if a student has started taking any part of the test.

Codes for Students Who Are Enrolled (Have Not Exited) but Do Not Complete Testing on the Precoded Answer Document If the student is enrolled during testing but did not complete testing for one of the following reasons, the principal must use

one of the following codes (80–96):

Accountability Codes 80 – If the student takes some, but not all of the subtests due to short-term illness or if the student is

absent for the entire testing cycle due to illness, the student will not receive a score of zero for the School Performance Score as long as he/she submits a letter from his or her doctor that :

is written on the doctor’s office letterhead stationery has the doctor’s original signature, and has the student’s dates of absence noted as being inclusive of all testing and makeup

dates. The School Test Coordinator must collect the doctor’s original letter from any students who were absent and provide these to the District Test Coordinator, who will maintain these letters on file for one year.

81 – if the student is LEP and has enrolled in and completed less than one full academic year in an English-speaking school, the School Test Coordinator must have sent a copy of the LEP deferment form to the District Test Coordinator before testing began.

82 – if the student receives precoded answer document(s) but the IEP indicates that he or she meets the eligibility requirements and participates in alternate assessment.

83 – if the student receives precoded answer document(s) for regular testing but is tested in the LAA-B program. Use of code 83 is allowed only in districts that have chosen to participate in LAA-B testing. Use of code 83 in a district not participating in LAA-B testing may result in an assigned zero for your School Performance Score.

84 – if the student receives LEAP 21 or GEE 21 precoded label(s) but is an Options (PreGED/Skills) Program student and should take The Iowa Tests instead.

86 – if the student’s precoded answer folder is not scannable, and a hand-coded answer folder was used instead. Use code 86 on the damaged answer folder with precoded label.

87 – if the student received a precoded label and answer document for LAA-B but was tested in the regular testing program. Use code 87 on the LAA-B answer document to which the precoded label has been affixed.

88 – if the student’s family member(s) dies. 89 – if the student is in protective custody. 90 – if the student’s scores are located during the Accountability Codes cleanup process. 91 – if the student used a hand-coded answer document because the answer document with precoded

label could not be located during testing. Use code 91 on the unused answer document(s) with precoded label(s) that was(were) found.

92 – if the student is in the old GEE program, not GEE 21. 93 – if the student is Option 2, 8th grade, and the precoded label was applied to a Science/Social Studies

answer document in error. Option 2 students take only English Language Arts or Mathematics. 94 – if the student receives a precoded LAA Response Document but is tested with LEAP 21, GEE 21,

The Iowa Tests or LAA-B. 95 – if the high school student receives a precoded LAA Response Document and is not in the 1st, 3rd, or

exit year of high school. 96 – if the student receives answer document(s) with precoded label(s) for test(s) that the student

previously passed.

Accountability Codes

ADDITIONAL NOTES: LEAP 21/GEE 21 only: If a test booklet and/or precoded answer document is torn,

soiled or not able to be scanned for some reason, the test administrator will contact the School Test Coordinator, who will notify the District Test Coordinator. Upon authorization from the DTC, the School Test Coordinator will affix a “Unscorable Document Label” to the answer document. The DTC will keep on file for one year documentation about why the answer document was not scannable.

All students in accountability option schools must be coded to their home-based school using the school site code. For accountability purposes, a home-based school is defined as the school where the student would have been enrolled if he/she had never enrolled in the accountability option school.

If any other unusual situations arise, the District Test Coordinator should contact the LDE

for additional assistance.

Accountability CleanupThere will be three cleanup reports that each school might have to work. Unlike prior years, this year all cleanup will be done through web-based applications, not paper reports. The Absence/Void Report has been replaced by the Impending Zero Report. The Invalid State Id report has been enhanced to include invalid dates of birth. The Routing reports are pretty much the same as they were in the past.

This report contains the names of students for which the precode document was not returned, a document with an unexcused absence was returned or a document with an invalid accountability code was returned. Any of these situations that are not cleaned up will result in a zero being assigned to the school for its SPS.

Accountability CleanupThe Invalid State Id or DOB Report

This report contains the names of students for which a document with an invalid State Id or Date of Birth (DOB) was returned. These 2 fields (along with the student’s first and last name and gender) are critical for the various matches done by the test vendors and the State. Matching records is very important both for your SPS and for subgroup reporting requirements of NCLB.

These reports contain the names of students whose scores are or should be routed back to a home-based school. Primarily this includes the students at Option 1 (or Routing) Schools, but may also include students in the Options program (PreGED or Skills curriculum). Use this report to verify or correct the site code of the student’s home-based school.
