TO: Asst. Schools Division Superintendent Chief Education Supervisors, CID & SGOD Education Program Supervisors Elementary and Secondary Public Schools District Supervisors Heads of Functional Units/Divisions in the Division Office All Others Concerned

•1. Please be advised that the Conduct of Second Quarter Division Monitoring, Evaluation and Adjustment (DMEA) on June 28, 2019 is re-scheduled on July 10, 2019, 8:00 a.m. to 5:00 p.m. at Villa Rosita Hotel, Concepcion Pequena, Naga City.

2. Chiefs of SGOD and CID and head of offices under OSDS are directed to accomplish the attached DMEA Forms (DMEA Form 1, 2, & 3). Accomplished forms will be presented per functional division on the same day.

3. All Elementary and Secondary Public Schools District Supervisors are directed to accomplish Enclosure 4 for submission online at smmecamsur@gmailcom on or before July 5, 2019.

4. Widest dissemination of this Advisory is earnestly desired.

5. Please be guided accordingly.
