Zusammenfassung
1. Der Mythos lebt (Scylla und Charybdis)!
2. Bei instabiler AP ohne Koronarintervention: ASS, Clopidogrel für 12 Monate.
3. Bei instabiler AP mit Koronarintervention: ASS, Clopidogrel (600 mg/ 150 mg/ 75 mg) für 12 Monate (Prasugrel).
4. Bei NSTEMI: ASS, Clopidogrel können vorgegeben werden. Dann invasive Diagnostik. Prasugrel (und GPIIb/IIIa- Antagonisten) während PCI und bei Hochrisiko (Ticagrelor in Zukunft?).
5. Wenn keine orale Einnahme möglich: GPIIb/IIIa- Antagonisten (nur bei Intervention) (Cangrelor in Zukunft?).
6. Bei STEMI mit Intervention: ASS, Clopidogrel/ Prasugrel.
7. Prasugrel: Patient im HK-Labor ohne Vorbehandlung mit Clopidogrel oder mit hohem Risiko für Stent- Thrombose (bekannte Clopidogrel-Resistenz).
8. GPIIb/IIIa-RA: i.v. Gabe, unmittelbar vor Intervention, Bail-Out-Therapie.
