About Hospices• There are more than 200 hospices across the UK
• Hospices improve the quality of life and wellbeing of adults and children with a life-limiting or terminal illness
• As well as taking care of people's physical needs, they also look after their emotional, spiritual and social needs.
• Hospices support carers, family members and close friends, both during a person's illness and during bereavement.
About Hospices contd…
• Accessed by patients with a range of illnesses, including cancer, heart and lung disease, motor neurone disease and dementia.
• All services are provided free of charge.
• There are 3 hospices which cover all or parts of the Triborough: Royal Trinity Hospice, St John’s, and the Pembridge.
About Trinity Hospice
• Founded in 1891, we are the oldest hospice in the UK
• Based on Clapham Common• Covers 750,000 people across Central
and Southwest London• We receive less than a third of our
funding from the NHS.

TYPES OF HOSPICE CARE
Patient and Family Support
Community Outpatients Inpatient Unit
1. Community
For patients who wish to be looked after at home.• Community Clinical
Nurse Specialists• Consultants• Occupational therapists• Befriending volunteers*• Hospice at Home**
2. Outpatients (Day Hospice)
Support at the hospice for patients living at home.• Physiotherapy• Group classes e.g. art,
exercise, relaxation• Complementary therapy• Nurse and Consultant
Clinics• Transport available
3. Inpatient
Provides symptom control, rehabilitation and end of life care at the hospice.• 28 beds• Individual ensuite rooms
and bay wards• Visiting times open• Families welcome to
stay
4. Patient and Family SupportEmotional and practical support to patients and their families.• Counselling• Welfare benefits advice• Practical planning• Spiritual care• Bereavement support
ACCESSING OUR SERVICES
Where Trinity worksWe provide services in the following CCG areas:• Hammersmith and Fulham• Kensington and Chelsea• Westminster• Lambeth• Wandsworth• Richmond• Merton
We sometimes care for people from other areas e.g. if friends or family live nearby.
Our dementia service is delivered across West London CCG.
How to refer
Referrals are accepted from:• Healthcare professionals (GP, Hospital Doctors and
Nurses, District Nurse)• Other providers of support such as social workers*• Self referrals from patients and those close to them*
Please call 020 7787 1065 to speak to our Referral and Admission Nurse.
