กรอกแบบค าขอใหถกตองและครบถวน Fill in the Application Form correctly and completely.

ขดขอความทไมใชออกและกาเครองหมาย ในชอง ทเกยวของ Cross out inapplicable wording and mark in the relevant box .

หากชองวางส าหรบกรอกไมพอ ใหจดท าเปนเอกสารแนบกบค าขอ If the space provided is not sufficient, describe in attachment.
