Gold Coast Primary Health NetworkAt the time of publication, Gold Coast Medicare Local (GCML) was still awaiting a decision from the Australian Government Department of Health, about who will be administering the Gold Coast Primary Health Network, which will replace Gold Coast Medicare Local from July 1 2015. GCML believes it has offered a strong tender, with general practice at the cornerstone. A decision is expected at the beginning of April. Details will be provided once more is known.

1. How to use the service directory to find professional services, support groups and information to assist patients.

2. How to list and edit an organisation/group or program on the HealthyGC website so other professionals and Gold Coast community members can find it.

3. How to access resources, publications, news, and professional events specifically for healthcare professionals.

The HealthyGC website receives more than 5000 visitors a month and has information and resources for both community members and healthcare service providers and professionals.

The comprehensive online service directory contains over 2000 listings of healthcare services, local support and self-help groups, online and telephone support and local programs. This is a free service for all local healthcare service providers and professionals.

GPs invited to offer clinical placementsGold Coast Medicare Local and the Griffith University School of Medicine would like to invite GPs to offer a one day a week clinical placement for 2nd year medical students, during June to August this year.

The placements aim to introduce students to the range of health problems and opportunities for disease prevention encountered in general practice, the social context of disease and health and to practice basic clinical skills that they have learnt at the School of Medicine.

The Board of Directors is looking to the future role of General Practice Gold Coast (GPGC) as we all await the news around which organisation was successful in its bid for the Gold Coast Primary Health Network. Directors have spent some time in a strategic planning exercise and will share results over the coming month. To add value to the plans for the future, GPGC will undertake a brief survey to ensure all members views and ideas around GPGC’s role into the future are taken into account. Planning for the delivery of CPR training is in the final stages of development and more news on how members can access CPR training will be available next month. As we wait for the PHN news we wish Primary Care Gold Coast all the best with its bid.

A significant campaign has been undertaken to visit all general practices on the Gold Coast who don’t currently refer to the Access to Allied Pyschological Services (ATAPS) program. Ninety five practices have been visited and as a result we have noted a significant increase in referrals in the General and Child Programs during January and February, which is particularly pleasing. Total referrals are at their highest levels since the 2012/13 financial year. Sessions provided are also increasing in relation to the number of referrals being received. Gold Coast Medicare Local (GCML) has been successful in re-establishing ATAPS service provision through a contract arrangement at headspace Southport, and expect to see an increase in referrals from this source over the coming months. ATAPS provides a significant benefit to GPs by allowing them to refer for an additional six sessions, once patients have used their initial 12, if the GP determines that there are exceptional circumstances (e.g. change in diagnosis or care circumstances) and the patient would benefit from the additional sessions. GPs are also reminded that they can access an online presentation of the Gold Coast HealthyGC website (www.healthygc.com.au). The HealthyGC website provides valuable information, events and resources for General Practices, and also provides the opportunity for practices to promote their services through the comprehensive online service directory, to the broader community. This presentation shows practices how to navigate the site, find GP resources, list their practice in the service directory and sign up for the GP Bulletin newsletter and alerts. The Gold Coast Integrated Care (GCIC) Program led by clinicians and supported by Gold Coast Health and Gold Coast Medicare Local, aims to reduce unplanned hospitalisations by sharing knowledge and linking services to close the gaps in the health care system for people with chronic and complex health issues.

Fourteen practices are involved in GCIC, with 13 having signed agreements with Gold Coast Medicare Local, for practice support.

The practice support component

includes data extraction and data cleaning, training and support for RecordPoint, assistance with quality improvement processes and systems and compensation for staff time spent in non-billable work such as risk stratification, consenting of patients, practice management backfill and GP remuneration for non-MBS input into patient risk stratification.

The first wave practices (3 practices) have completed the on-boarding document and continue to complete risk stratification processes to

finalise a disease register of patients categorised as 1, 2 or 3. Two service navigators have commenced and the practice support team nominated.

Allied health providers identified from the first practices have attended a program overview and information session about Recordpoint. Planning continues to identify further allied health professionals for the next wave of practices to go live.

Oncology Education EventRadiology Oncology Queenland is holding an education event for general practitioners. The evening will focus on common cancer management issues, including the role of radiation therapy in every day general practice. It will include topics on multidisciplinary cancer management, the role of radiation therapy in general practice, common general practice cases including breast cancer, prostate cancer and palliative medicine and Stereotactic Radiation Therapy. This will be followed by a tour of the radiology department.

Gold Coast Lead Clinician Group

Following the Voicebox project, 17 digital stories have been beautifully produced allowing participants to share their personal stories about mental illness. These will be captioned to assist those with hearing impairments, and also for situations where the videos may be broadcast without sound, such as practice waiting rooms. PROJECT SHOWCASEThe Gold Coast Lead Clinician Group (GCLCG) was established by Gold Coast Health and Gold Coast Medicare Local (GCML) to review processes between community and acute services to enable greater integration of care. To contact this group email: [email protected]. The latest actions are below.

• The GCLCG recommends that as a standard of care, outpatient visits should lead to a letter being sent to the patient’s GP, with a very high priority to those discharged from a service or having change in their management.

• The GCLCG discussed the concept of Clinical Prioritisation Criteria and felt caution was needed when implementing such a big change to the system to reduce the potential impact of possible negative effects.

• The GCLCG will review the current General Practice Liaison Officer work plan/partnership agreement at the next meeting and will identify some strategies for the next financial year should GCML be successful for the PHN tender.

• The CGLCG were updated on the on the Gold Coast Integrated Care Program, and has approved of the progress to date.

Aged care referral changes(Extract from Pulse + IT Magazine, by Kate McDonald)All new referrals for federally funded community and residential aged care services will be conducted electronically from July 1 under the new My Aged Care system, although exceptions have been made for healthcare professionals sending inbound referrals who still prefer to use fax machines. Department of Social Services (DSS) representatives are currently conducting a series of information briefings to the sector around the country to explain the changes, including a live demonstration of how the new My Aged Care web portals will work for clients, assessors and service providers.

The changes will see the Home and Community Care (HACC) program aged care system screened initially by a contact centre run by Healthdirect and using a national screening and assessment form (NSAF). The contact centre will establish a central client record that can be shared by the client and their carers or family, independent assessors and aged care service providers.

Following the initial screening process, a referral notice will be sent to an independent Regional Assessment Service (RAS) or Aged Care Assessment Team (ACAT) by email. The email does not contain personal details, but will alert assessors that there is a new referral, which can be accessed through a web portal. Assessors will then conduct a face-to-face assessment with the client using the pre-populated NSAF and develop a care plan in association with the client. Depending on the client’s needs, they can then be referred electronically to community or residential service providers, who will also be able to access the central client record and the referral through a provider portal.

School ImmunisationGold Coast Medicare Local is asking GPs to help promote the School Immunisation Program which commenced last month, by providing immunisation advice, to encourage the return of consent forms.

Students who missed the first dose of HPV vaccine at school will be directed to catch up at their GP or council clinic. If they catch up before April 18 they can resume the normal cycle through the School Program at four months.

Whooping cough reminderPregnant women in their 3rd trimester can still receive free whooping cough vaccine. This is a Queensland Health strategy to further protect newborn babies against whooping cough.

The Gold Coast sees 300 cases of this highly infectious disease per year which can be fatal for young babies. Practices just need to identify who they are ordering vaccine for and place their usual monthly order via QHIP.

Decision AssistGP education Decision Assist is offering a range of programs in 2015/16 to assist GPs and practice nurses to enhance their skills and knowledge about advance care planning and palliative care.

The program, funded by the Australian Government, is hosting a series of workshops nationally and delivering online education opportunities, many of which are accredited with the RACGP and

ACRRM to contribute to CPD points. GPs are also encouraged to phone the national telephone advisory line – 1300 668 908 – for advice on palliative care 24 hours a day, every day of the week, and advance care planning from 8am until 8pm daily.

The service is free and available to GPs and aged care staff around Australia who need timely advice on subjects ranging from palliative

care medication and symptom management, to psycho-social support and bereavement advice; how best to initiate conversations about advance care planning and palliative care, as well as the development of care plans and understanding of the legal framework.

Library blues bustingFrom now until the end of June, 13 Gold Coast libraries, through funding from Partners in Recovery Gold Coast, is presenting Upbeat, a program supporting positive mental health and wellbeing. This program aims to bring health and wellbeing benefits to the community through mood lifting books, ebooks, CDs and DVDs in support of many common mental health issues. Art therapy, music therapy, mindfulness workshops and information sessions are also available.

Acting Mayor Donna Gates said the City’s libraries provided a valuable contribution to social wellbeing. “Our libraries are for everyone; and they are well placed to make a positive impact on people’s lives in many ways, including providing social interaction and

community involvement. They’re true community hubs – inviting and inclusive.

PIR Consortia member, Gold Coast Medicare Local, said PIR is supporting the library project because of its benefits to the community. CEO Matt Carrodus said mental health has been identified as a significant issue on the Gold Coast, and increasing access to community information, activities and support was important.

“This is an innovative wide-reaching program that aims to

improve education, awareness and understanding, and to reduce the stigma that can exist for people with a mental illness,” Mr Carrodus said.

Expressions of interest sought from local GPsA small group of GPs is being sought to work with Dr Paul Van Buynder, Public Health Medical Officer at the Gold Coast Public Health Unit, on a range of public health issues and suggested solutions that could be identified at a general practice level. This will be a short term group and GPs will be remunerated at the Gold Coast Medicare Local (GCML) GP rate and supported by General Practice Gold Coast. RACGP Standards for General PracticeThe RACGP has commenced the process of developing the next edition of the RACGP Standards for general practices. General practices can become involved in this process – sending in a submission, responding to a questionnaire, providing verbal feedback, using social media and/or attending a workshop. In brief....April No Falls aims to raise awareness about reducing the falls risk, particularly among senior Australians – one of the largest causes of hospitalisation. GPs can provide advice to patients about ways to reduce their falls risk. This can include:

• Talking to a podiatrist about having healthy feet and appropriate footwear

• Regular medication reviews by a GP or pharmacist• Regular eye checks by an optometrist• Bone strengthening and balance exercises through an exercise

physiologist or physiotherapist• Seeing a dietitian about eating a healthy and balanced diet• Hearing checks through an audiologist, to maintain balance• Removing hazards around the home

Gold Coast Pharmacist Jason Keily said that multiple classes of medications, and polypharmacy can significantly increase the falls risk in a patient. Regular medication reviews performed by a pharmacist, may help reduce the risk of medication misadventure.

“Patients who take four or more medications, or have had recent hospital admissions with medication changes, may benefit from a Home Medication Review (HMR),” Mr Keily said.

“Home Medication Reviews help promote coordinated care, especially when the patient has a need for chronic disease management. A fully completed HMR referral can be claimed under a MBS item 900, which currently stands at $151.75 per fully completed referral.”

Exercise Physiologist Andrew Simpson said a home exercise program with home hazard management can be an effective way to reduce falls.
