Information for 4 th grade students and parents: As we begin to prepare for the end of the 2015-16 school year, we also begin preparing for the beginning of the 2016-17 school year! With the transition of fifth grade, your students will experience a new structure that will help them prepare for middle school. Each fifth grader will have a primary homeroom teacher next year, but will work with a team of 2 or 3 teachers for core content areas. This will allow teachers to better meet individual student needs, and will help students make connections with peers beyond their homeroom class. Teams of two will share students for reading and math. Teams of three will share students for reading, math, and science/social studies. Students will be with their homeroom teach- ers for interactive read aloud, writing instruction, word study, specials, and science/social studies (for teams of 2). We are excited about the community-building opportunities we will have. We are excited about having more adults know your child really well and are looking forward to better meeting students’ academic needs. We will continue to utilize our Parent Input Form to gather information pertaining to your child. This form is important to us as we create our homeroom classes. This form is at- tached to this newsletter (for current 4th graders only) and can also be found on our school website or in our school office. The form is the only communication channel that will be accepted. This form focuses on the strengths and needs of your child. You may describe the characteristics you desire in a teacher as well. If you choose to write the name of a specific teacher please list the reasons for your request. There are never any guarantees that teacher requests will be met due to the many variables we take into account when placing students into classrooms. We ask that you trust our professional judgment on what is best for ALL children and want you to know that we take this process very seriously. We do encourage you to fill out this form and return it to the HIS office no later than Fri- day, May 27, 2016. If you have specific questions, please call our office. Questions relat- ed to sixth grade placements should be directed to Northridge Middle School at 825-9531. The mission of Heritage Intermediate School is to equip students to achieve

their potential and positively impact their community.

ISTEP+ (Round 2) During the time period of April 18 – May 6, 2016 all of our 4th and 5th graders will be tak-

ing the multiple choice section of the ISTEP+ test. As you may recall, we took the Ap-

Notes from our School Nurse….

Just a few reminders for everyone about the Middlebury Community Schools medication policy: 1. Medication must come to school in the original container with a note from the parent that specifies the time the medica-tion is to be given and the dose of the medication. If your child is to receive prescription medication for more than 5 days, a note to the school from the doctor is required. 2. All prescription and non-prescription medications must be kept in the nurses' office (includes tylenol, tums, throat sprays). The student must bring the medicine to the clinic right away in the morning. 3. In some cases, emergency medications (i.e. inhalers) may be carried on the student's person IF the parent provides a note from the doctor stating that the student may carry and self-administer the inhaler. It is recommended, however, that all inhalers are kept in the nurse's office. 4. Any medication that is sent to school may be released only to the parent or a designated adult over the age of 18 with the parent's written permission to pick up the medicine. Please do not send medicine to school with your child and ex-pect the school to send it back home with your child. For more information about the medication policy or to download a medication form, you may refer to: http://www.mcsin-k12.org/health/prescription.pdf

Sixth Grade Immunizations

The state of Indiana (IC 4-22-2) REQUIRES the following immunizations for students in grades 6 for the 2016-2017 school year. Students not in compliance will be subject to exclusion from school.

Two doses of varicella vaccine, or a history of chicken pox disease. One dose of tetanus-diptheria-acellular pertussis vaccine (Tdap) given on or after the 10

th birthday.

One dose of meningococcal conjugate vaccine (MCV4).

Fifth Grade Parents: Please remember to make your child’s appointment for his or her sixth grade immunizations ASAP, if you haven’t done so already! If you’d like to schedule them with the Elkhart County Health Department, the phone number is (574)523-2127.

Hoops For Heart Update

Recently at HIS, all students took part in Hoops

for Heart during PE. It was a fundraiser for the

American Heart Association. The students collected dona-

tions from the community which were sent to the American

Heart Association. In all, the students raised $5,014.45. The

students also learned about the importance of taking care of

their bodies, especially the heart, and learned some ways to

help themselves and others stay healthy. The students also

participated in healthy fitness activities during the Hoop for

Heart event. Some of the events they took part in were bas-

ketball shooting contests and spinning the hula hoop.

3rd Gr. Parent Orientation Night (for parents of 3rd graders going into 4th grade)

Place: Her itage Inter School~ Main Gym

Date: Tuesday May 3, 2016

Time: 6:30 pm

We respectfully ask that students not attend

during this session. The program will last

approximately 45 minutes and there will be infor-

mation handed out to assist in the transition for your

child to the Intermediate school.

There will also be a 3rd grade transition trip to Her-

itage later in May for your student. This will in-

clude an orientation trip to Heritage and activities

that will blend students together from all four ele-

mentary schools. More information will be coming

home with your child regarding this trip.

From our Student Handbook...

One Thing Constant in the World...is Change

As the end of the school year approaches, we begin to mentally prepare children for the changes coming up in their lives. In 5th grade, we start introducing the students to what life will be like in the middle school. More inde-pendence, more responsibility, and of course, more op-portunities. To help children with change, be sure to keep some schedules constant. Maybe the same times of getting up and going to bed, maybe eating together as a family, are ideas for keeping consistency for children. Children thrive on knowing what to expect from adults. You are one thing that they can depend upon!

Good Bystander Swim 3/11/2016

Good bystanders are students who have shown good friendship qualities. They have either stood up for an-

other student who was being bullied, included a student who was being left out, or told an adult if a bullying

situation did not stop. The following students participated in a fun swim time with Coach Keller at the middle

school pool to celebrate their good choices. More students will be given this opportunity throughout the

school year. Way to go good bystanders!
