On October 27, 1994 the insulation was removed from the control center heating!ventilation and air conditioning [(CCHVAC)(VI)] return air fan discharge duct{as part of the implementation of Engineering Design Package [EDP] 26868 forifire damper modifications. On October 31, 1994 engineering personneli

determined that a nine foot length of C channel, which had been wedged againsta piece of unistrut and partially wrapped inside the duct insulation, had not I

been seismically supported. The channel is located above the control roompanel H11-P808 containing controls for both safety related and non safetyrelated plant equipment. The cause of the problem was an engineering errorwhich occurred in 1983 when the structural channels were unbolted from thecontrol room ceiling. To correct the situation, the affected (south) channelhas been welded to the adjacent channels, and the south channel was re-boltedto the control room ceiling. Engineering has revised (i.e., posted) thedrawing which specifically references the weld configuration of this structuralchannel and has conducted a review of similar configurations.

Description of the Event:

On October 27, 1994 the insulation was removed from the control center heatingventilation and air conditioning [(CCHVAC)(VI)] return air fan discharge ductas part of the implementation of Engineering Design Package [EDP] 26868 forfire damper modifications. On October 31, 1994 engineering personneldetermined that a nine foot length of C channel, which had been partiallywrapped inside the insulation had not been seismically supported. The Cchannel was being held in place by the insulation and a close physical fitbetween the duct and a nearby unistrut tubing support. Upon discovery thecaterial was secured in place and an investigation of the design was initiated.

A review of the design history revealed that in 1977 this C channel was part ofa duct support which had been bolted to the control room ceiling. The north,cast, and west supports were welded to steel plates which were each welded tothe duct. However, the south support was bolted to the ceiling, but was notwelded to the duct, probably because of its location at the inside radius of anolbow in the duct. Original as-built design drawings had been updated toreflect this configuration.

In 1983 a design change was implemented to allow for CCHVAC duct thermal i

expansion during a fire. The bolts attaching the structural channels to thec3111ng were removed. Insulation covered the mated surface between the flangeof the C channels and the duct, obscuring the absence of welds from the channelto the duct for the south channel during the 1983 modification.

The channel is part of a hanger that is located above the control room panelH11-P808 which contains controls for both safety related y d non safety relatedequipment including standby gas treatment [VL], CCHVAC, engineered safetyfcature room coolers [VA), emergency diesel generator ventilation [VJ), torushardened vent [VLR], and remote manual containment isolation valves [ISV). The '

duct hanger, which is within the control room envelope, is required to ber.cismic category I or II/I and therefore, the condition described above is ;

considered a condition outside the design basis.i

Cause of the Event:

The cause of the problem was an engineering design change process error whichoccurred in 1983 during the initial construction of the plant when designchange request "H-0980", revision "A" unbolted the channel from the controlroom ceiling. The error involved utility non-licensed, and contractnon-licensed personnel who prepared and approved the design change request.This error is attributed to two possible causes.

1) The field design engineers may have incorrectly interpreted the hangerdrawing in 1983 when the ceiling bolts were removed. The hanger drawingrefers to two inch welds every five inches along this support as " typical"to hold the duct to the structural channels. However, it was not obviousfrom the drawing that this entire section of C channel was not welded inplace.

2) The field and design engineers did not perform an adequate field walkdownto identify the weld configuration for the structural channels.

Analysis of the Event:

. Structures important to safety are required to be designed to withstand theioffects of natural phenomena such as earthquakes. The specific criteria for 1

the control room is seismic category I or II/I for structures and components. |

The south structural C channel for the CCHVAC return air fan discharge ducthinger did not meet this seismic criteria. Therefore, the condition describedla reportable in accordance with 10CFR50.73(a)(2)(ii)(B) for a conditionoutside the design basis. The actual probability of the south channel fallingon the H11-P808 control panel during a seismic event appears small whenconsidering: 1) The tight geometry of structures and components above thecontrol room's suspended ceiling. 2) And those structures and components ared signed to meet seismic category I or II/I criteria.

Corrective Actions:

EDP 26868 was revised to have the south structural channel welded to theedjacent structural channels, and to have the structural channels rebolted tothe control room ceiling. A new fire damper was also installed that does notrequire space for duct thermal expansion at this location. EDP 26868 postedchanges against vendor drawing 2849 17 for the CCHVAC return air fan dischargeduct hanger to specifically reference the weld conff guration of the structuralchannels.

Engineering personnel have inspected all accessible fire damper modifications,performed in the 1983 time frame which unbolted hangers or other members from I

w211s, floors and ceilings to assure the hangers and members are appropriatelyattached to the duct. No cther seismic problems were found. One previouslyidentified discrepancy was observed that does not, impact the seismicqualifications of the equipment and is being dispositioned within theCorrective Action Program.
