Event Documentation Required

New Hire Photocopy of birth certificate for dependent children, marriage license for spouse

Marriage Photocopy of marriage license

Divorce/Legal Separation Photocopy of court order

Spouse Losing or Gaining Eligibility Certificate of Creditable Coverage or letter from current employer

Birth/Adoption/Establish Guardianship Photocopy of birth certificate or court order

Death of a Dependent or Spouse Photocopy of death certificate

Your Own Loss/Gain of Other Coverage Certificate of Creditable Coverage or letter from current carrier

The warning symbol and a yellow bar will alert you that your dependent or your event is pended and indicate theneeded documentation.

Section 5: Beneficiary Designation

1. Click the green “+” sign to add your beneficiaries.

2. To designate a dependent you have previously added to the system as your beneficiary, select the dropdown arrow under the Name cell to select your beneficiary.

You may also elect to add a beneficiary that is not a dependent under your benefit coverage, including yourself.

1. To do so, click the down arrow in the Name cell to add the new beneficiary. Select “Beneficiary not listed?Add one.”

2. Elect the percentage you would like to designate to each beneficiary. You may select both Primary andContingent designations, and each must total 100 percent. You cannot select the same person as both pri-mary and contingent beneficiaries.

PLEASE NOTE: If you have already registered, you do not need to complete these steps. If you have forgotten yourusername or password, please go to https://portal.adp.com click the appropriate link(s) and follow the prompts.

The first time you access the Benefits Online Website, you will need to register and create a user name and password. You will need your company's registration pass code for part of the registration.

We are committed to protecting your privacy and ensuring the security of your personal information. To help usmeet this commitment, each employee will be required to register on the website before using any services.Follow these steps to complete the registration process.

• Go to the self-service website at https://portal.adp.com, in First Time Users Section, click on CreateAccount.

• Select Register Now and enter your employer provided Registration Pass Code of LHM-Group, click Next.

• In order to Verify Your Identity, you will need to enter the following information:• First Name*• Last Name*• SSN, and • Birth Month and Day

*NOTE: It is important that your name be entered exactly as it appears on your pay statement.

• Click Next. If any information entered does not match the data in the system, you may be sent to a secondverification page. If this occurs, do not complete the fields on this page. Please use your Browser "Back" button and re-enter your information in the correct format.

• You will then need to Enter Your Contact Information. Please enter your email address and phone number (phone number is optional). This email address is used for notification, so enter one you have easyaccess to check. You can update this email address as needed. Click Next.

• If you forget your logon information in the future, you will be asked to answer some additional questions inorder to access the system. Here you will Enter Your Security Information by creating some uniquequestions and answers that may be used in the future to verify your identity, so be sure to choose informationyou can remember. Click Next.

• Your User ID will be displayed and you will be asked to Create Your Password. Your password must be atleast 8 characters in length and contain at least oneletter and either one number or special character - forexample: Lunch@12. Keep in mind, your password is case sensitive. Once you create your unique password,click Submit.

• Please make note of your user ID and password and store them in a safe place.

Once you have successfully completed your registration, you will see a confirmation page stating "Thank youfor registering!" You can now log on to, and start using the Benefits Online Website. An email containing youruser ID will be sent to the address you provided.

Section 1: Accessing Benefits EnrollmentNewly Hired/Newly Eligible Associate Access

If you are a new employee or recently became eligible for benefits, your event will be available from the EmployeeDashboard.

Important: You will be timed out after 20 minutes of inactivity and your elections will not be saved or recorded. Ifyou need more time, it is very important that you click “Next” until you reach the Summary page. From theSummary page you will be able to click “Save & Return Later.” If you press “Cancel” at any time during theprocess, your changes will not be saved and you will be returned to the Employee Dashboard.

Important: Life event options will not go into effect until approved. See Section 4 for additional information.

Current Associate Access

1. If you are reporting a life event, choose the “Report an Event” button from the Employee Dashboard. Thisopens a pop up window.

2. Choose from the drop-down menus to report your event.

3. Your Life event will be avaliable for 24 hours. If you re-enter the system during your 24 hour period you mayaccess the event from the Employee Dashboard. Click either “Enroll” button.

Annual Enrollment Access

1. From the Employee Dashboard, click the EnrollToday buttonanytime during the Annual Enrollmentperiod.

Section 2: Adding and ManagingDependentsDependents must be added to the system before theycan be added to coverage. You will need your depen-dent’s names, birthdates and Social Security Numbersto complete this section.

1. Click the blue Manage Dependents hyperlink fromthe Summary page.

Any dependent already in the system will be listedon the Manage Dependents page.

2. To add a new dependent to coverage, click the green “+” sign to add dependents.

3. Complete the dependent’s information in the fields as required. Click the Done button after each entry.

4. Repeat to add all dependents you wish to cover under your plan then click Submit.

Important: Adding a Dependent to the system does not add the dependent to coverage. If you want a dependent tohave coverage for Medical and/or Dental and/or Vision you must make those specific election in each benefit area.

Section 3: Electing BenefitsOnce you have completed adding your dependents, you may begin electing benefits.

1. Select the plan you wish to elect for the upcoming plan year using the radio buttons, then select the depend-ents you want to add to coverage by clicking the checkbox.

2. When you have finished making your benefit elections for all benefit areas, click the “Done” button to bereturned to the Summary page. Review all of the information and if everything is accurate, click the “ConfirmElections” button at the bottom of the page, the Certification Statement opens. Click the “I Agree” button.

3. Use the radio buttons to specify if you want an email sent to notify you when updates are complete using theradio buttons. If an e-mail address is not listed for you, you will need to type your e-mail address each timeyou access this site. After you click “Submit”, the Confirmation page opens. You will see a green bar appearacross the top of your Summary with your confirmation number.

Important: Your elections will not be processed until you have completed the certifications steps and receive a confirmation number.
