Your benefits We’re committed to supporting you with a comprehensive

benefits program designed to meet your unique needs. Key

features of your Havertys benefits include:

Take action This guide describes your health plan options and other

important benefits. Use this information, along with other

helpful resources available by clicking the Benefits tile on your

home page in Employee Central (EC), then clicking on the

Benefit Service Center link to choose the coverage that’s right

for you and your family. Then be sure to enroll by the enrollment

deadline to make the most of your benefits in 2020.

Who’s eligible? All full-time Team Members who work 30+ hours per week

are eligible for health benefits. New Team Members must

enroll within 60 days of their hire date to initiate coverage.

In addition, Team Members may enroll their eligible

dependents** in health benefits. Eligible dependents include:

• Legally married spouse.

• Your dependent children, which includes your natural,

legally adopted, step, or foster children to age 26,

regardless of student or marital status.*

• Children for whom you or your spouse are required to

provide coverage under a Qualified Medical Child

Support Order (QMCSO).

• Other children who live with you in a regular parent-child

relationship, and for whom you have obtained legal custody

or guardianship.

• Mentally or physically disabled children who are totally

dependent upon you for support.

• Medical Plan: Regardless of student or marital status,

dependents to age 26 are eligible for coverage.

• Dental and Vision: Coverage is available to those unmarried

children younger than 19, and those unmarried children

younger than 26 who are enrolled as a full-time student at an

accredited secondary school, college, university, vocational

or trade school.

* Full-time student status is verified two times per year by our benefit service center. The dependent must be verified for full-time student status or dental and vision coverage will be dropped.

** Dependents added to plans must be verified with marriage certificates for spouses and birth certificates for children along with social security numbers.

Coverage for spouses To help manage health care costs for our company and our

Team Members, we require that spouses who have access to

benefits through their own employer obtain that coverage. If

your spouse is eligible for employer-sponsored coverage in

2020, you will not be able to enroll him/her in coverage through

Havertys. Only spouses who do not have access to other

coverage may enroll in coverage through Havertys.

Effective date of coverage For new Team Members, the effective date of coverage for

most plans is the first of the month following 60 days of service.

For existing Team Members enrolling during Open Enrollment,

the effective date of all plans is January 1, 2020.

Important reminders

• New Team Members: Enroll within 60 days from your

date of hire. If you don’t enroll within this time period, you

will not have benefits coverage, except for plans and

programs that are fully paid by Havertys, such as basic life

insurance.

• Open Enrollment - October 21, 2019 —

November 8, 2019: Enroll before the enrollment

deadline. If you do not make changes to your coverage

within the enrollment time period, your current coverage will

continue. However, if you want to participate in any of the

following benefits in 2020, you must actively enroll in them

during Open Enrollment:

— Health Care Flexible Spending Account (FSA).

— Dependent Care FSA.

— Legal Services.

• After your enrollment opportunity ends, you will not

be able to make changes to your benefits until the next

Open Enrollment, unless you experience a qualifying life

event, such as marriage, divorce, birth, adoption, or a

change in your or your spouse’s employment status that

affects your benefits eligibility.

Quality health coverage is one of the most valuable benefits you enjoy as a Havertys Team Member. Our benefits program offers plans to help keep you and your family healthy and also provide important protection in the event of illness or injury.

Premium changes Please review your open enrollment guide for changes to

your medical plan premiums.

We understand that any premium changes can present

challenges for Team Members and their families. Accordingly,

Havertys will absorb a portion of the new cost increase to help

offset any changes.

Medical Havertys offers a choice of three Anthem Preferred Provider

Organization (PPO) medical insurance plans so you can select

the coverage that’s best for you and your family. Each PPO

features a network of doctors, hospitals, and other health care

providers who have agreed to charge negotiated fees for their

services. Each time you need care, you decide whether to use

an in-network provider or an out-of-network provider. Each plan

also offers prescription drug benefits administered by Express

Scripts.

Unless a service is covered by an office visit copay, you will

meet an annual deductible before the plan begins to pay for

most eligible benefits. Once you meet your deductible, the plan

pays a percentage of the cost of care — also known as

coinsurance — and you pay the rest.

You can choose from the following options:

• The Premier PPO Plan

• The Plus PPO Plan

• The Basic PPO Plan

Which plan is right for you? The best medical plan for you depends on a number of

factors. Consider these questions:

• How much health care do you anticipate needing in the

coming year?

• Would you rather keep your paycheck deductions as

low as possible and set aside tax-free money in a

Flexible Spending Account (FSA) to pay your higher

deductible costs?

• Would you rather pay more from your paycheck in order to

keep your deductible costs lower when you need care?

Now that you’ve asked yourself those questions,

remember that:

• A plan with a higher paycheck deduction will cost you more

each pay period, regardless of how much care you actually

receive, but will have lower deductibles and pay more of the

cost of services.

• A plan with a lower paycheck deduction will have higher

deductibles to be met and pay less of the cost of services.

Medical plan costs You and Havertys share the cost of your medical benefits

— Havertys pays a majority of the total cost and you pay the

remainder. The amount you pay is deducted from your paycheck.

Your specific cost is determined by the plan you choose and the

coverage level you select.

• Telemedicine – Use your electronic device with internet capabilities to access a board-certified doctor

24/7. Online visit typically costing as little as $49 depending on your plan. Go to www.livehealthonline.com

for more details.

• See in-network providers – They’ve agreed to the plan’s negotiated rates. Visit your plan website to

search for in-network providers near you.

• Use the mail-order pharmacy – It will save you time and money when refilling long-term prescriptions.

• NurseLine by Anthem – Call 24/7 and talk with a registered nurse about your health concern. Making the right call can

save you time and money. Call the toll free number for customer service on your ID card and request the NurseLine.

Removing the surcharge

If you currently pay the surcharge but have stopped using tobacco products during 2019, contact

the Benefits Service Center at 866-456-1419 during Open Enrollment to report this healthy change

and avoid the surcharge.

Programs

Tobacco Surcharge Because of the direct medical and lost productivity costs related to tobacco use, tobacco users pay more for their medical coverage

through Havertys.

When enrolling for benefits, you will be asked “Have you used tobacco products more than 5 times in the past 3 months?” If you can

honestly answer no, then you will not be required to pay the tobacco surcharge.

The surcharge is $50 per month when you enroll in the plan. When you go online or call to enroll, you can obtain the exact rate you will

pay.

If you currently use tobacco, we encourage you to quit. We offer Team Members access to tobacco cessation resources at no cost through

the myWellness program by accessing the Benefits tile on your home page in Employee Central and clicking on Wellness program. If you

enroll in the tobacco cessation program through the myWellness program, and complete one session (which consists of 5 phone calls with

a myWellness program representative) anytime in 2020, your surcharge will be removed once Havertys has been notified.

98point6 98point6 is a new kind of primary care that gives you on-demand access to a physician via private and secure in-app messaging, right

from your mobile phone.

• Private, Full-Service Care: You get primary care services — including diagnosis, treatment, ordering of prescriptions and labs, or just

questions answered from US based board-certified physicians that are employed solely by 98point6.

• On Your Time and Terms: Care is accessed through a message-based app on your mobile device. You can get care from anywhere

— during a break at work, while outdoors, or over lunch. No appointment, no travel, no waiting.

• Low or No Cost, No Appointments: With 98point6, visits are available at no cost to PPO plan participants and are only $5 per visit for

all other eligible members. Prescriptions or labs received will be processed and billed in accordance with your medical plan benefits.

Pregnant? Join Future Moms Future Moms, part of your Havertys benefits program, can answer your questions about pregnancy, help you make good choices, and

follow your health care provider’s care plan. Simply call toll-free at 1-800-828-5891 to get started and take advantage of 24/7 access

to nurse coaches, health risk screening, free call with pharmacists and nutritionists, and lactation consultations.

Health Champion Health Champion empowers you to make better health care choices by assisting with fee negotiation and payment plans, making sense

of medical bills, understanding your diagnosis, and helping you prepare for doctor visits. Call 877-327-4432

to get answers to your questions. This is a free service.

Livongo for Diabetes The Livongo for Diabetes Program provides personalized support to help employees better manage life with diabetes at no cost to

them. The program provides:

• A cellular glucose meter to help employees upload blood glucose levels to a secure online portal to share with their health care

providers.

• The ability to communicate with Certified Diabetes Educators who can help answer nutrition and lifestyle questions.

• Access to unlimited strips and lancets with no hidden costs.

Employees as well as spouses and dependents who are covered under a Havertys medical plan are eligible to participate. If you or your

covered dependent has diabetes, you will automatically receive a direct home mailing from Livongo. After enrolling on the Livongo site,

you will receive a welcome kit providing the glucose meter, strips, information and more.

Flexible Spending Accounts

Flexible Spending Accounts (FSAs) Tax-advantaged FSAs are a great way to save money. The money you contribute to these accounts comes out of your paycheck

without being taxed, and you withdraw it tax-free when you pay for eligible health care and dependent care expenses.

Havertys offers you the following FSAs:

Health Care FSA • Pay for eligible health care expenses, such as plan

deductibles, copays, and coinsurance.

• Contribute up to $2,700 in 2020.

* Grace Period – participants can submit claims toward the previous plan year with date of service through 3/15 of the current plan year.

Dependent Care FSA • Pay for eligible dependent care expenses, such as day care

for a child so you and your spouse can work, look for work,

or attend school full time.

• Contribute up to $5,000 in 2020, or $2,500 if you are

married and filing separately.

Estimate carefully Keep in mind, FSAs are “use-it-or-lose-it” accounts. You will

forfeit any amount left in the account at the end of the plan year.

The IRS requires substantiation of your FSA claims submitted to

Discovery Benefits. Please ask your doctor for a Flex Spending

receipt for services provided. Call Discovery Benefits for

additional information at 866-451-3399.

Managing your FSA(s) The website www.DiscoveryBenefits.com offers you

an easy way to track and manage your FSA money.

For even greater convenience, you can download the

Discovery Benefits mobile app for fast and easy

claims submission and to chat online with a

representative.

When you enroll in a Health Care FSA, Discovery

Benefits will send you a debit card, which you can use

to pay for eligible expenses. Depending on the

transaction, you may need to submit receipts or other

documentation to Discovery Benefits.

Vision

Vision Having vision coverage allows you to save money on eligible eye care expenses, such as periodic eye exams, eyeglasses,

contact lenses, and more for you and your covered dependents.

Changes for 2020 EyeMed will continue to provide vision benefits in 2020. However, we will be implementing a new provider network with plan design

enhancements that include lower copays for progressive lens coverage and anti-reflective lens coating. You will continue to receive

coverage for periodic eye exams, eyeglasses, contact lenses, and more for yourself and your covered dependents.

See the benefits of blue light protection Cell phones, tablets, laptops, and televisions illuminate our world. But the conveniences these digital devices provide come at a

price — exposure to blue light, the part of the light spectrum visible to the human eye. Frequent exposure to blue light can have

many negative effects, including blurry vision, difficulty focusing, dry and irritated eyes, headaches, and even macular degeneration.

Fortunately, you can obtain special lens treatments that protect against both UV rays and harmful high-energy blue light. Find a

Focus on wellness Havertys’ wellness program myWellness is designed to

encourage good health and financial well-being. You play an

important role in managing your health care costs by

maintaining or moving toward a healthy lifestyle. The

myWellness program is here to help you. The program

provides access to a variety of information, resources, and

activities to promote health, fitness, and overall well-being.

You may be able to decrease your health care expenses over

your lifetime by staying fit and healthy.

A great place to start is by taking the health assessment. It

provides tools to track and monitor your progress toward a

personalized plan focused on your goals. Visit the wellness

program by clicking on the Benefits tile on your home page

in EC or go to www.guidanceresources.com and earn

12,500 incentive points that you can use to purchase great

gift items.

Telemedicine Even if you’re in good physical health, you might not feel great. Every now and again, it can be difficult to remain positive, and taking on the daily stresses of life can seem overwhelming. Havertys understands this and wants to help you.

That’s why we’re providing access to the Anthem Behavioral Health network of providers using virtual visits. That means you can talk with a licensed therapist or substance abuse professional via video chat or phone, without leaving your home or office.

Go to livehealthonline.com to search for a video telehealth specialist. Call to make an appointment with your selected provider. Your co-pay will depend on the Anthem plan that you are enrolled in.

Employee assistance program The Havertys Employee Assistance

Program (EAP) is available throughout

the year to assist with your everyday

needs, at no cost

to you. It’s all part of our commitment to supporting

your total well-being. Get help with work-life issues,

referrals for clinical, legal, and financial services,

and more. To begin taking advantage of this

valuable benefit, visit the Benefits tile on your

home page in EC and click on myWellness or call

1-877-327-4432.

Take advantage of preventive care benefits Good preventive care can help you stay healthy and detect any

“silent” problems early, when they’re most likely to be treatable.

Most in-network preventive services are covered in full, so there’s

no excuse to skip it. It could even save your life. Use the checklist

below to make sure you maintain your total health.

• Have a routine physical exam each year. You’ll earn

2,000 wellness points, build a relationship with your doctor,

and can reduce your risk for many serious conditions.

• Get regular dental cleanings. You’ll earn 2,000 wellness

points semi-annually. Numerous studies show a link

between regular dental cleanings and disease prevention

— including lower risks of heart disease, diabetes, and

stroke.

• See your eye doctor at least once every two years.

If you have certain health risks, such as diabetes or high

blood pressure, your doctor may recommend more frequent

eye exams. You will receive 2,000 points every year for

visiting your eye doctor.

NurseLine (Anthem)

As an Anthem member, you can access the NurseLine 24/7

to speak with a registered nurse to answer your questions about

health concerns. Just call 1-877-875-9093. This is a free service.

Don’t have a personal doctor? You should. Here’s why. • Better health. Getting the right health

screenings through your doctor each year can reduce your

risk for many conditions. Preventive care is free, so there’s

no excuse to skip it.

• A healthier wallet. Having a doctor you can call helps you

avoid costly trips to the emergency room. Your doctor will

also help you decide when you really need to see a

specialist.

• Peace of mind. Advice from someone you trust more important when you’re sick. Your personal doctor gets to know you and your health history and can help coordinate any care you need.

financial Havertys offers programs to help ensure financial security for you and your family. We also provide access to benefits designed to help you save money on important services and insurance coverage.

Life and accident insurance Full-time Havertys Team Members receive company-paid

life and accident insurance and have the option to purchase

additional coverage for yourself and your family.

Employee life and AD&D insurance Havertys provides you with basic life and accidental death and

dismemberment (AD&D) insurance so that you can protect

those you love from the unexpected. There is no cost to you

for this coverage. Your benefit amount is dependent on your

years of service with Havertys.

Following your 10 years of service – your annual

earnings up to $100,000

If you want added protection, you can purchase supplemental

life and/or AD&D insurance for yourself. You may elect

coverage up to $650,000 (amounts over $300,000 are subject

to EOI). Contact our Benefit Service Center at 866-456-1419

for your specific rates.

Spouse life and AD&D insurance You may purchase life and/or AD&D insurance for your

spouse up to $100,000 (not to exceed 100% of your elected

amount). Amounts over $25,000 are subject to EOI.

When two Havertys Team Members are married to each other,

they cannot be covered both as an employee and as a spouse.

Each one may elect employee life insurance separately but

cannot elect spouse life insurance. Also, only one Team

Member may carry dependent child life insurance. Call the

Benefit Service Center at 866-456-1419 for specific rates.

Child life insurance Optional child life insurance provides $10,000 of life insurance

for children age 15 days through age 25. The rate is $1.14 per

month, regardless of the number of children covered, after tax.

Federal tax law requires Havertys to report the cost of company-paid life insurance in excess of $50,000 as imputed income. AD&D benefits are paid in addition to any life insurance if you die in an accident or become seriously injured or physically disabled.

What is AD&D insurance? speech, or use of your limb(s) in an accident, AD&D provides additional benefits to help keep your family financially secure. AD&D benefits are paid as a percentage of your coverage amount — from 50% to 100% — depending on the type of loss.

financial Short-term and long-term disability insurance

Disability insurance The loss of income due to illness or disability can cause serious financial hardship for your family. Disability insurance replaces

a portion of your income to help you continue paying your bills and meeting your financial obligations during this difficult time.

Short-term disability Short-term disability (STD) is available to full-time Team Members on the first of the month following the completion of one year of

service. If you want to enroll in this coverage, you are required to elect the STD plan when you are first eligible to enroll for benefits.

However, even though you elect coverage, STD coverage will not begin until the 1st day of the month following your completion of the

12-month waiting period. Any payroll deductions for this coverage will not start until the waiting period has been satisfied and coverage

is effective.

Long-term disability A long-term disability (LTD) program is available to full-time Team Members on the first of the month after you have completed

one year of service. Once coverage is effective, benefits will begin after 180 days of disability and will continue to your normal

Social Security retirement age, or even longer based on the age at which you became disabled.* Havertys will contribute 50% of

the cost of LTD coverage.

Legal plan The MetLaw Hyatt Legal Plan offers participants and their

eligible dependents access to legal advice and services from

a nationwide network of attorneys with coverage for many

personal legal issues. Services include telephone advice and

office consultations on an unlimited number of legal matters,

The legal plan is available for $17/month after tax. This is a

whose support goes far beyond do-it-yourself kit resolution. The

specialist will handle the recovery process behind the scenes by

401(k) thrift plan

Havertys 401(k) Thrift Plan The Havertys 401(k) Thrift Plan provides advantages you may

not get with other types of savings plans and helps you meet

one of life’s important goals — saving for a financially secure

retirement.

Eligibility You are eligible to participate if you are at least 21 years of

age and have been a Team Member for at least 90 days

of continuous employment.

Your contributions You can contribute to your 401(k) with before-tax money or

Roth after-tax money. The type of contributions you make will

depend on your financial goals and circumstances. You may

contribute up to the IRS maximum of $18,000 in 2020.

If you are age 50 or older, you may make additional catch-up

contributions — up to $6,000 in 2020.

Matching contributions Havertys will match 100% of your contributions — dollar for

dollar — for the first 4% of your pay that you contribute.

Investment elections The plan offers you a variety of investment options to choose

from. It’s important to carefully consider your investment goals,

retirement time frame, and risk tolerance when deciding how to

invest your plan contributions. Visit the Thrift Plan link by

clicking on the Benefits tile on your home page in EC to learn

more about your investment options.

Vesting Vesting refers to your ownership of the money in your account.

You are always 100% vested in your own contributions. You

become vested in Havertys’ contributions over time according to

the following schedule:

Enrolling in the plan Havertys will mail you information about the Havertys 401(k)

Thrift Plan 60 days after your date of hire. The plan has an auto

enroll feature. If you do nothing at all, you will be automatically

enrolled in the plan at 3%. You can opt out or change your

contribution percentage by calling Schwab. You will be auto

increased 1% each year until the maximum of 10% is reached. If

you do have questions about the plan or would like information

on such topics as increasing your contribution rate or making a

different investment selection, contact Schwab at 800-274-7526.

Be sure to update your 401(k) beneficiary by accessing your

Schwab account online or contacting Schwab at 800-724-7526.

What happens if you don’t enroll? As a new Team Member – If you don’t enroll in benefits within

60 days of your hire date, you will not have benefits coverage,

except for the plans and programs that are fully paid by

Havertys, such as basic life insurance.

During Open Enrollment – If you want to make changes to

your benefits or enroll in an FSA or the legal plan, you must

take action before the enrollment deadline. If you don’t enroll,

you will keep your current coverage, with the exception of your

FSA contributions and the legal plan.

Changes during the year After your enrollment opportunity ends, you won’t be able to

change your benefits coverage during the year unless you

experience a qualifying life event, such as marriage, divorce,

birth, adoption, or a change in your or your spouse’s

employment status that affects your benefits eligibility.
