Covered according to the reasonable and customary prices for the same treatment at the member levelof hospitals’ network. Subjected to Bupa Arabiaagreed prices with the network providers in the Kingdom of
Saudi Arabia.

Bupa Arabia have assigned hospital delegates and roving doctors to many providers to assist the member
in the provider premises and ensure quality services are provided to the member in appropriate time.

Understanding your scheme
The importance of approval (pre-authorisation) process
Pre-authorisation is a necessary procedure for us to check that the required tests, procedures or
medications are:
• Appropriate for your condition
• Covered by your benefits
• Within your benefit sub-limits
Don’t worry, you do not need to wait long at the hospital or even come back the next day for the
treatment. Our electronic approval process ensures that you receive the needed medical attention
fast. The hospital staff will send the request to us on your behalf. (refer to proceeding diagram)
In each instance, we inform you of the status of your request by SMS if you are enrolled in our
Msg.Bupa service. Enrol now to Msg.Bupa, by follwing simple instructions on page 3.
Should you encounter any delay or have any questions while at the hospital, call our 24/7 member
helpline 800 4400 444 or +966 12 603 6266. You can call this numbers using your mobile phone
24 hours a day, 7 days a week.
Filling for a claim
Depending on your scheme, you can file for claims (reimbursement) when you seek treatment
outside of your approved network of hospitals and clinics, or when you use your card outside of
Saudi Arabia.
To receive payment for the money you spent for all eligible medical treatment, make sure you send
your accomplished Bupa claim form which is enclosed in this guide book and can be downloaded
from our website.
To avoid any delay, make sure to submit to Bupa the following within 60 days of treatment date:
• Bupa claim form completed, signed and stamped by the treating doctor.
• Detailed medical report
• Investigations and laboratory results
• Original invoices and receipts
• Pre-authorisation approval from Bupa, if applicable

Bupa requires 10 working days to pay out a complete reimbursement claim.

هذه الوثيقة لن تغطي املطالبات التي تنشأ عما يلي:
االصابة التي يسببها الشخص لنفسه متعمدا. .١
اÏمراض التي تنشأ بفعل إساءة استعمال بعض اÏدوية .٢ئات أو بفعل تعاطي املواد الكحولية أو املنشطات أو املهد
أو اخملدرات أو ما شابه ذلك.
اجلراحة أو املعاجلة التجميلية إال إذا استدعتها إصابة جسدية .٣عرضية غ× مستثناة ¶ هذا القسم.
الفحوصات الدورية الشاملة واللقاحات أو العقاق× أو الوسائل .٤الوقائية التي ال تتطلبها معاجلة طبية مغطاة ضمن ال§نامج (باستثناء اÂجراءات الوقائية التي حتددها وزارة الصحة مثل
التطعيمات ورعاية اÏمومة والطفولة).
معاجلة احلمل والوالدة للمرأة املتعاقد معها على أساس أنها .٥غ× متزوجة.
املعاجلة التي يتلقاها الشخص املؤمن عليه بدون مقابل. .٦
االستجمام وبرامج الصحة البدنية العامة والعالج ¶ دور الرعاية .٧االجتماعية.
أي علة أو إصابة تنشأ كنتيجة مباشرة ملهنة الشخص .٨املؤمن عليه.
معاجلة اÏمراض التناسلية أو التي تنتقل باالتصال اجلنسي .٩.êاملتعارف عليها طبي
مصاريف عالج الف�ة الالحقة لتشخيص (ف×وس نقص املناعة .١٠البشرية) HIV أو العلل واÏمراض ذات الصلة بالـ HIV مبا فيها
مرض اÂيدز (نقص املناعة املكتسبة) أو مشتقاتها أو مرادفاتها أو أشكالها اÏخرى.
جميع التكاليف املتعلقة بزرع اÏسنان أو تركيب اÏسنان .١١االصطناعية أو اجلسور الثابتة أو املتحركة أو التقومي باستثناء
تلك التي تسببت عن وسائط خارجية عنيفة.
اختبارات تصحيح النظر أو السمع والوسائل البصرية أو السمعية .١٢املساعدة ما _ تكن مطلوبة بأمر الطبيب املرخص.
مصاريف انتقال الشخص املؤمن عليه بوسائل نقل غ× سيارات .١٣اÂسعاف احمللية املرخص لها أو التابعة جلمعية الهالل اÏحمر
السعودي.
تساقط الشعر أو الصلع أو الشعر املستعار. .١٤
املعاجلة النفسية أو االضطرابات العقلية أو العصبية ما عدا احلاالت .١٥احلادة.
اختبارات احلساسية مهما كانت طبيعتها بخالف تلك املتعلقة .١٦باÏدوية أو التشخيص أو العالج.
اÏجهزة والوسائل والعقاق× واÂجراءات أو املعاجلة بالهرمونات .١٧بهدف تنظيم النسل أو منع احلمل أو حصوله أو العقم أو العجز اجلنسي أو نقص اخلصوبة أو التخصيب بواسطة اÏنابيب أو أية
وسائل أخرى للتلقيح االصطناعي.
أي تكاليف أو مصاريف إضافية يتكبدها املرافق للمؤمن عليه أثناء .١٨تنوميه أو إقامته باملستشفى عدا نفقة اÂقامة واÂعاشة ¶
املستشفى ملرافق واحد للمؤمن عليه، كمرافقة اÏم لطفلها حتى سن الثانية عشرة، أو حيثما تقتضي الضرورة الطبية ذلك
حسب تقدير الطبيب املعالج.
معاجلة البثور (حب الشباب) أو أية معاجلة تتعلق بالسمنة .١٩أو البدانة.
اÏخطار الشخصية الواردة ¶ قسم التعريفات من هذه الوثيقة .٢٠
أدوية ووسائل العالج بالطب البديل. .٢١
التغ×ات الطبيعية لسن اليأس للمؤمن عليها و يشمل ذلك تغ×ات .٢٢الطمث.
االطراف الصناعية و االطراف املساعدة ماعدا تلك التي تتطلبها .٢٣حالة املؤمن له مبوجب قرار طبي باملرفق الصحي املعتمد من
اجمللس.
Conditions not covered
Consistent with the guidelines issued by the CCHI, the following cases and conditions are not covered by your healthcare scheme/s:
This list of non-covered conditions is set by the CCHI under the health regulation law
1. Intentional self-inflicted injury.
2. Sicknesses resulting from abuse of some medicines, stimulants or tranquilizers, or from use of alcohol, narcotics and the like.
3. Cosmetic treatment or surgery unless necessitated by a bodily injury not excluded in this section.
4. General checkups, inoculations, drugs or preventive measures not required for medical treatment covered under this policy (excluding preventive measures determined by the Ministry of Health, such as vaccination, maternity and child care.).
5. Pregnancy and delivery treatment of a woman identified in her contract as unmarried.
6. Treatment received by a beneficiary free of charge.
7. Rest cures, general health cures and treatment in social welfare institutions.
8. Any illness or injury resulting directly from the beneficiary's profession.
9. Medically recognized venereal or sexually transmitted diseases.
10. Costs of treatment following diagnosis of HIV or any disease related to HIV, including AIDS and its derivatives, alternatives or other forms.
11. All costs related to tooth implant, dentures, fixed or movable bridges or orthodontic treatment, unless resulting from violent external means.
12. Vision or hearing correction tests and visual or hearing aids, unless requested by a licensed physician.
13. The beneficiary's transportation expenses within and between cities in the Kingdom by other than ambulances of the Saudi Red Crescent or licensed ambulances.
14. Hair loss, baldness or artificial hair.
15. Psychological, mental or nervous disorders, unless of an acute nature as specified in the policy schedule.
16. Allergy tests of any nature, unless relating to medicines, diagnosis or treatment.
17. Equipment, means, drugs and procedures, or hormone treatment aimed at regulating
reproduction, contraception, fertility, infertility, impotence, secondary sterility, in-vitro fertilization or any other method of artificial fertilization.
18. Any costs or additional expenses incurred by the beneficiary's companion during a hospital stay, except for hospital room and board charges for one companion such as a mother companying her child aged up to twelve years or whenever medically necessary as assessed by the attending physician.
19. Treatment of acne or any treatment relating to obesity or overweight.
20. Personal risks set forth in Section – 1 (Definitions) of this Policy.
21. Alternative medicine procedures and medications.
22. Artificial and ancillary limbs except those required by the beneficiary as per a medical decision issued by the health care facility approved by the Council.
23. Natural changes related to menopause, including menstrual disorders.

This policy- will not cover the health benefits and repatriation of the remains to country of origin if claims are directly arising from the following:
1. War, invasion, foreign enemy actions, aggressive actions (whether war declared or not) and civil war.
2. Ionic radiation and contamination with radio active material resulting from nuclear fuel or any nuclear waste resulting from the burning of nuclear fuel.
3. The radioactive, poisonous, explosive properties or any other hazardous properties of any nuclear materials stored or any of their components.
4. The insured involvement or participation in the service of the armed forces, police or in any of their operations.
5. Riots, strikes terrorism or any similar acts.
