• เลอกขนาดยาท�เหมาะสม
• การเลอกชนดและขนาดยา
• บรหารยาแกผปวยอยางสม�าเสมอตลอด 24 ช �วโมง
• ควรใชยาท�มฤทธ 5ส 6นเสรม หากมอาการปวดรนแรงแทรกซอนข6นมาเปนชวง ๆ
• ผปวยแตละรายมความตองการยาระงบปวดแตกตางกนไป จงตองหม �นปรบยาเพ�อใหขนาดยาท�เหมาะสม

หลกการในการใชยากลม opioid
• เลอกวธบรหารยาท�เหมาะสม คอ ใชวธท�ทาใหเจบปวดนอยท�สด ปลอดภย แตสามารถระงบปวดไดเพยงพอ
• เตรยมพรอมท�จะบาบดผลขางเคยงทเกดข6นจากการใชยา
• ตดตามและประเมนผลผปวยอยางตอเน�อง

MST
• เปนยาเมดเคลอบ ทานทก 12 ช �วโมง
• เร�มออกฤทธ 5ใน 1 ช �วโมง
• ขนาด 10,30 mg
• หามเค6ยวหรอบดยาโดยเดดขาด ตองกลนท 6งเมด
Kapanol
• อยในรปแคปซล ภายในเปน pellet
• ออกฤทธ 5ใน 2-4 ช �วโมง
• ฤทธ 5อยนาน 24 ช �วโมง
• ขนาด 20,50 มลลกรม
• บรหารทาง nasogastric tube ได

• มท 6งยาฉด, ยาแผนแปะผวหนง
• หลงปดบนผวหนง ยาจะคอยดดซมชาๆ ใชเวลา 12-24 ช �วโมง
• ออกฤทธ 5ตอเน�องนาน 72 ช �วโมง
• การแปะ ควรแปะบนผวหนงท�ไมมอาการระคายเคอง ผวหนงเรยบ
• ไมควรใชมดโกนโกนขนกอนปดแผนยา

• ใชไดดในกรณผปวยมปญหาเก�ยวกบการรบประทานยา, กลนยา, มปญหาเก�ยวกบการดดซมยา

• ขนาดของยาถกกาหนดแนนอน ทาใหยงยากในการปรบขนาด
• ใชเวลานานพอสมควร กวาระดบยาจะข6นถงจดท�ตองการ
• เม�อมผลขางเคยงเกดข6น ตองเฝาตดตามนานกวาปกต
• ไมสามารถใชกบผปวยท�มโรคผวหนง หรอผวหนงผดปกต

Meperidine
• 1/10 strength of morphine
• Mild vagolytic and anti spasmodic effect
• Myocardial depression and histamine release
• Normeperidine can cause seizure.
• Interaction with MAOI : fatal reaction
• Caution in renal failure
• Not for chronic use

Tramadol
• Weak mu and kappa receptor agonist
(Approximate 1/10 as potent as morphine)
• O-desmethyl metabolite (M1 metabolite) 6
times more potent than tramadol
• Inhibit 5-HT and NE reuptake (induce nausea,
dizziness and tachycardia)
• Increase seizure risk with TCAs
• Adult dose : 400 mg/day
• Over age 75 years : 200-300 mg/day
• Renal insufficiency (CrCl < 30 ml/min)
• Reduce dosing frequency to every 12 hours
• Do not exceed 200 mg per day
Tramadol
Combination drugs
• Codeine with acetaminophen
• Codeine 15 mg + Acetaminophen 300 mg
• Tramadol with acetaminophen
• Tramadol 37.5 mg + Acetaminophen 325 mg

Acetaminophen
• Mechanism of action
• Inhibits the release of prostaglandins in the
spinal cord and has effects on serotonin
mechanism for spinal pain inhibition
• Inhibits the COX-3 isoenzyme
Acetaminophen
• บรหารไดงาย เพราะมหลายรปแบบ ไดแก ยาเมด, ยาน6า, ยาฉด, ยาเหนบ รวมท 6งแบบ Extended release หรอผสมกบ codeine หรอtramadol
• ขนาดท�แนะนาใหใชคอ 325-500 mg ทก 4 ชม. หรอ 1 g ทก 6 ชม.
• ขนาดยาสงสดไมควรเกน 4 กรม/วน
• ระดบยาในเลอดข6นสงสดใน 30-60 นาท
