Cerner 5.7 Update Fact Sheet

• New content and new features• Reduced vertical length means less scrolling• Additional content is collapsed and opens up to allow for additional details when
the clinician chooses the response• New Help Page: Offers guidance and education on answering questions or using
imbedded tools by clicking on the blue words in the profile.• Updated content to conform with ICD-10 coding
• New ICD-10 Template names are followed by (10)• OASIS-C1 (10) Discharge From Agency• OASIS-C1 (10) Recertification/Other Follow Up• OASIS-C1 (10) Start/Resumption of Care• OASIS-C1 (10) Transfer to an Inpatient Facility
• Transition to ICD-10 will begin on 9/25/2015
• Adding a new contact between 9/25/2015 and 9/30/3015 will require the clinician to select the correct note. • When adding your contact you will be prompted to provide the “Anticipated M0090
Completion Date”; date that you will finish your assessment and related documentation. (slide 1)
• The answer to the M0090 will prompt you to choose the correct version of the note required. If M0090 = 10/1/15 or later then you will choose OASIS-C1 (10) for your documentation (slide 2)
• A new Functional section has been added to the Oasis C1 (10) Note and to the Routine Note. Most Functional Assessment Tools, utilized by therapy, are now imbedded within the notes.(slide 3)• The TINETTI BALANCE ASSESSMENT TOOL will continue to be added as a
secondary profile.
• The Modified Borg Rating Scale has been added to the cardiovascular section. Perceived dyspnea can be easily tracked amongst all disciplines. (slides 4 and 5)
• Pain descriptors have been modified and “Acceptable Level of Pain” has been added. (slide 6)
• Nutritional content has been enhanced to include Alternative Nutrition: TPN, Lipids,G-tube… (slide 7 &8)
• Sensory Status & EENT has enhanced content related to swallowing, phonation and food trials for use by Speech Pathology. (slide 9, 10, 11 &12)
• The Musculoskeletal Section has been expanded. There are many new areas for therapy to easily add specific MS related content. SN is expected to complete this section through and including Gross Joint and Muscle Strength ONLY. Therapy will continue with more specific content. (slide 13)
• Medication Validation displays only selected content. (slide 14)
• A Dialysis section was added to the Urinary section.
• Documentation related to ALL Therapy Evaluation Visits and ALL Therapy Routine Visits will now be completed using the Routine Note. The TINETTI BALANCE ASSESSMENT TOOL will continue to be added as a secondary profile when appropriate.

Without scoring tools, answering the question of how much is often difficult. Using a tool, like the Borg Rating Scale for Perceived Dypsnea you will be able to communicate patient changes more accurately.
This supports a patient‐centered approach and encourages all clinicians to use standardized scoring. This creates more consistent scoring between disciplines and will lead us closer to outcome measure reporting capability.

In the Nutritional Requirements section, we modified several questions and added additional content to facilitate your ability to complete a comprehensive assessment and to provide Speech Language Pathologists with more details that can be used for their evaluation data.

The changes related to the Musculoskeletal Section represent a complete revision to the content, layout, and rules from a multidisciplinary perspective. Gross and Specific Joint and Muscle Strength questions provide both your nursing and therapy disciplines with the scoring details to complete their evaluation of the patient’s musculoskeletal health. Your nursing clinicians will typically stop after answering the Gross Joint & Muscle Strength questions.
