NOMINATION GUIDELINES
The Hamilton Veterans Hall of Fame recognizes the efforts of Hamilton’s distinguished men and women who have worn the
uniform of our nation’s armed services and then continued to contribute to their community through acts of volunteerism, ad-
vocacy, public service, professional distinction and philanthropy. The program is not intended to focus solely on what Hamilton
military members accomplished while in uniform but what has occurred in their lives outside and beyond their military service.
Who is eligible?
The veteran nominee must meet the following criteria:
1. Must be a past or current Hamilton resident
2. Must have received an honorable discharge
3. Must be of good moral character
Definition of a Veteran
The definition of a veteran, ORC 5903.01—Veterans mean any person who has completed service in the armed forces, includ-
ing the National Guard of any state, or a reserve component of the armed forces, who has been discharged under honorable
conditions from the armed forces or has been transferred to the reserves with evidence of satisfactory service.
For the Nominator:
Information in this packet shall not be falsified in any manner.
Include final discharge papers, such as DD Form 214. If nominee received several discharges please provide the final dis-
charge with the nomination packet. Discharge papers can be requested from the National Personnel Records Center
(NPRC), 9700 Page Blvd., St. Louis, MO, 63132, http://www.archives.gov or by calling 866-272-6272. You may also reach
out to the local County Veteran Service Office for assistance.
The nomination packet must not exceed 5 pages on 8.5” x 11” single sided paper—excluding nomination form and dis-
charge papers. Packets that exceed 5 pages will be returned to the nominator.
Do not include Original documents as nomination packets and attachments become the property of the Hamilton Veteran
HOF.
Do not include documents such as newspaper articles that lose legibility when copied. Rather, send a clean, legible 8.5” x
11” copy of the document or article.
Posthumous nominations will be accepted.
The Hamilton Veterans Hall of Fame committee will review the nominations in early fall. Recommendations for induction
are submitted for review to Hamilton City Council. Finalists and their nominators will be notified in October.
Nominators of veterans who were not selected will be notified.

NOMINATION PACKET
For the submission process to uniformly portray the achievement and accomplishment of these great Hamilton vet-
erans, the nomination packet must adhere to the following format. Please label sections accordingly.
SECTION 1 Hamilton Veterans Hall of Fame Nomination Form
This form requires the signature of the nominator to validate that the information is accurate.
SECTION 2 Verification of Veteran Status
Attach a certified copy of the nominee’s final discharge papers such as a DD Form 214
SECTION 3 Summary of Post-Military Achievements
Examples include exemplary professional achievements; contributions through educational, fraternal, faith, veteran
and service organizations; political office, public service; volunteerism and more.
SECTION 4 Post military Awards and Honors
Provide a list and highlights of awards and honors. Examples include Presidential awards, Citizen of the Year , Veter-
an of the Year, significant milestones, etc.
SECTION 5 Narrative Detailing the Veteran’s Overall Accomplishment and Achievements, listing acts of volunteer-
ism, advocacy, public service, professional distinction and philanthropy within the community.
Provide a narrative detailing why this Hamiltonian should be considered for the Hamilton Veteran Hall of Fame.
Approximately 500 words.
